Current and emerging MR imaging techniques for the diagnosis and management of CSF flow disorders: a review of phase-contrast and time-spatial labeling inversion pulse.

This article provides an overview of phase-contrast and time-spatial labeling inversion pulse MR imaging techniques to assess CSF movement in the CNS under normal and pathophysiologic situations. Phase-contrast can quantitatively measure stroke volume in selected regions, notably the aqueduct of Sylvius, synchronized to the heartbeat. Judicious fine-tuning of the technique is needed to achieve maximal temporal resolution, and it has limited visualization of CSF motion in many CNS regions. Phase-contrast is frequently used to evaluate those patients with suspected normal pressure hydrocephalus

The lack of relationship between intracranial pressure and cerebral ventricle indices based on brain computed tomography in patients undergoing ventriculoperitoneal shunt.

BACKGROUND: In this study we investigated whether cerebral ventricle indices based on brain computed tomography (CT) scans are reliable for predicting intracranial pressure (ICP) in hydrocephalic patients. METHODS: Electronic medical records of 221 patients undergoing ventriculoperitoneal shunt due to hydrocephalus were retrospectively reviewed. Cerebral ventricle indices including Evans' index, third ventricle index, cella media index, and ventricular score were calculated from transverse diameters measured at various levels on preoperative brain CT scans. ICP was considered as CSF opening p

Obstructive hydrocephalus secondary to an ectatic basilar artery.

A 57 year-old white hypertensive male presented with a five-month history of progressive dementia and a clinical picture suggestive of normal pressure hydrocephalus. A CT scan and vertebral angiogram demonstrated an ectatic basilar artery obstructing the posterior third ventricle; the resulting hydrocephalus was relieved by a ventriculo-peritoneal shunt and the patient's symptoms resolved completely.

Ventriculoperitoneal shunt malfunction due to chronic cholecystitis: A case report.

RATIONALE: Ventriculoperitoneal shunt (VPS) is the most common treatment for idiopathic normal pressure hydrocephalus, a subtype of hydrocephalus characterized by gait disturbance, dementia, and urinary incontinence. However, while the malfunction of VPS is reported at a high rate, the involvement of chronic cholecystitis in shunt malfunction is rare. PATIENT CONCERNS: A 73-year-old woman with idiopathic normal pressure hydrocephalus who received a VPS but subsequently developed chronic cholecystitis. The patient suffered from drowsiness and was unable to walk. Her family found that she prese

Novel therapeutics for hydrocephalus: Insights from animal models.

Hydrocephalus is a cerebrospinal fluid physiological disorder that causes ventricular dilation with normal or high intracranial pressure. The current regular treatment for hydrocephalus is cerebrospinal fluid shunting, which is frequently related to failure and complications. Meanwhile, considering that the current nonsurgical treatments of hydrocephalus can only relieve the symptoms but cannot eliminate this complication caused by primary brain injuries, the exploration of more effective therapies has become the focus for many researchers. In this article, the current research status and prog

Epidemiology and practice variations of shunt surgery for hydrocephalus: a nationwide registry-based study.

OBJECTIVE: Indications for surgical treatment of hydrocephalus (HC) can vary across centers. The authors sought to investigate the frequencies of surgically treated HC disorders and to study variations in the practice of shunt surgery in Norway, a country with universal and free healthcare. METHODS: This is a nationwide registry-based study using data from the Norwegian Patient Registry. Four neurosurgical centers serve exclusively in 4 defined geographic regions. All patients who underwent shunt surgery in Norway between January 1, 2008, and December 31, 2021, were included and regional diff

Subdural effusion protects the aging brain from harmful ventriculomegaly.

The human brain loses its volume and its function during aging. The solid part of the brain within the intracranial space, the brain parenchyma, decreases in volume with age; while the cerebrospinal fluid (CSF) volume increases. With progressive loss of brain parenchymal volume (BPV), CSF may shift from cerebral ventricles to the subdural space, forming subdural effusion (SDE), whose role in the brain aging process remains unclear. We hypothesize that damages associated with ventriculomegaly can be lessened after formation of SDE. As the BPV decreases, the enlarged ventricular surface area cau

A critical evaluation of continuous intracranial pressure monitoring in pediatric hydrocephalus.

An evaluation of continuous intracranial pressure (ICP) monitoring in 42 hydrocephalic infants and children helps to establish the role of this method in comparison with other clinical techniques for estimating the occurrence of progressive hydrocephalus. Transducer monitoring via ventricular catheters in term and premature infants revealed average ICP in the 5--12 mm Hg range. The higher values were recorded from patients with noncommunicating hydrocephalus but did not correlate with the extent of ventriculomagaly. The method has very litte predictive value in estimating the progressive natur

[Case of hydrocephalus associated with vestibular schwannoma, treated by tumor removal].

A 57-year-old male presented with gait disturbance, urinary incontinence, dementia resembling symptom of normal pressure hydrocephalus (NPH) and was admitted to our institusion. A computerized tomographic scan (CT) and magnetic resonance imaging (MRI) revealed hydrocephalus as well as a tumor in the right cerebellopontine angle. We diagnosed communicating-type hydrocephalus associated with the vestibular schwannoma and tumor removal was performed prior to ventriculo-peritoneal (V-P) shunt. After surgery, symptoms of the patient disappeared promptly and cerebrospinal fluid protein elevation was

[Indications for the intrathecal infusion test in disorders of cerebrospinal fluid dynamics].

Indications for and results of the intrathecal infusion test including 50 patients with chronic disturbances of the circulation of the cerebrospinal fluid are presented and discussed. The computer-aided infusion test permits in 93 per cent a reliable differentiation between patients with a normal-pressure hydrocephalus and those with cerebro-atrophic processes. Functional checking of the valve systems is a further indication for the intrathecal infusion test. In patients with a classical pressure hydrocephalus or subdural hygromas, however, we do not see an indication for this invasive examina

Otitic hydrocephalus: a report of 2 cases.

Otitic hydrocephalus is a rare intracranial complication of otitis media. It is characterized by elevated cerebrospinal fluid (CSF) pressure with normal CSF biochemistry and without any focal neurologic abnormality other than those related to the increased intracranial pressure. The pathophysiology of this rare condition is unknown. The incidence of intracranial complications was reduced dramatically with the introduction of newer antibiotics, but complicated cases still occur. Serious consequences can ensue if complications are not identified and treated properly. The author reviews the liter

Long-standing overt ventriculomegaly in adults (LOVA) as a distinct entity on the neurologist's differential: a narrative review.

Long-standing overt ventriculomegaly in adults (LOVA) has been posited as a form of progressive hydrocephalus, with similar clinical and radiographic features to normal pressure hydrocephalus (NPH), but which should be understood as a distinct clinical entity. We conducted a narrative review analysing the literature into LOVA as a distinct form of hydrocephalus with its own clinical and radiographic characteristics and treatment modalities. LOVA is characterised by triventriculomegaly, an Evans' index of ≥0.4, presenting with progressive symptoms of elevated intracranial pressure after an init

Secondary communicating hydrocephalus management by implantation of external ventricular shunt and minimal gradual increase of cerebrospinal fluid pressure.

BACKGROUND: This paper presents a new management strategy explaining the process used by implantation of external ventricular drainage (EVD) and very gradual increase of intracranial pressure for treatment of acute hydrocephalus. During the last 30 years' experience in professional practice, the senior author (M.S.) discovered that there are some options of regulations between cerebral spinal fluid (CSF) production and resorption. This theory shows that minimal continuous increase of the CSF pressure as long as the patient general neurological condition is unchanged and appears as normal can l

Serial CT scanning in childhood tuberculous meningitis: prognostic features in 198 cases.

Serial cranial computed tomographic (CT) scanning and intracranial pressure monitoring were performed on 198 children with stage II and III tuberculous meningitis. The aims of the study were to document the course of tuberculous hydrocephalus during medical and surgical treatment, as well as the prognostic significance of parenchymal changes in the brain as demonstrated by CT. Lumbar cerebrospinal fluid pressure was monitored continuously for a 1-hour period in all patients on admission and at weekly intervals in patients with communicating hydrocephalus for the 1st month of treatment. Vertebrobasilar artery ectasia presenting as hydrocephalus.

Symptomatic hydrocephalus in association with basilar artery ectasia is a rare occurrence. We report an 81-year-old woman with progressive dementia, gait disturbance, and sphincter incontinence. Brain computed tomography (CT) and magnetic resonance imaging (MRI) scans showed obstructive hydrocephalus with compression of the fourth ventricle by a large ectatic basilar artery. After ventriculoperitoneal shunting, the patient showed rapid resolution of symptoms. This case illustrates that ventricular obstruction due to basilar artery ectasia can mimic the classical triad of normal pressure hydroc

Parkinsonian syndromes associated with hydrocephalus: case reports, a review of the literature, and pathophysiological hypotheses.

We present nine cases of obstructive hydrocephalus (OH) associated with marked parkinsonism. Four patients had noncommunicating OH (NCOH) [three nontumoral aqueductal stenosis (AS), one tumoral AS]. The presentation was that of acute or subacute parkinsonism, usually at the time of acute recurrent ventricular obstruction. Three had a marked response to levodopa and required short-term treatment after shunting. However, one has remained levodopa dependent after 2 1/2 years. Three of the five patients with communicating OH (COH) presented with shunt-responsive normal pressure hydrocephalus (NPH)

Dementia and hydrocephalus in Paget's disease: a case report.

Dementia and hydrocephalus in an elderly female were related to basilar impression caused by Paget's disease of the skull. Ventricular and lumbar thecal pressures were normal, but isotope cisternography suggested obstructive hydrocephalus. The patient improved after ventricular shunting. The importance of prompt investigation of dementia occurring in patients with Paget's disease is emphasised.

[Computer tomographic diagnosis of a mega-dolicho basilar artery (author's transl)].

Four mega-dolicho basilar arteries are described, which were found by computer tomography and confirmed by angiography. Two patients showed additional abnormalities or anomalies of other vessels. The computer tomographic features are described. The elongated vessels can produce so-called normal pressure hydrocephalus by impressing the floor of the third ventricle, although the hydrocephalus cannot be confirmed by computer tomographic cisternography.

Modeling CSF circulation and the glymphatic system during infusion using subject specific intracranial pressures and brain geometries.

BACKGROUND: Infusion testing is an established method for assessing CSF resistance in patients with idiopathic normal pressure hydrocephalus (iNPH). To what extent the increased resistance is related to the glymphatic system is an open question. Here we introduce a computational model that includes the glymphatic system and enables us to determine the importance of (1) brain geometry, (2) intracranial pressure, and (3) physiological parameters on the outcome of and response to an infusion test. METHODS: We implemented a seven-compartment multiple network porous medium model with subject speci
