Selective contrast enhancement of anterior spinal nerve roots on magnetic resonance imaging: a suggestive sign of Guillain-Barré syndrome and neurobrucellosis.

BACKGROUND: Recent studies suggest that cardiac resynchronization therapy through biventricular pacing (BVP) may be a promising new treatment for patients with advanced congestive heart failure (CHF). This method involves implantation of pacer leads into the right atrium (RA), right ventricle (RV), and coronary sinus (CS) in patients with ventricular dyssynchrony as evidenced by a bundle branch block pattern on electrocardiogram (ECG). Clinical trials are enrolling stable patients with ejection fractions (EF) </= 35%, left ventricular end-diastolic diameters (LVIDd) >/= 54 mm, and QRS duration…

The Effect of Sacubitril/Valsartan on Supraventricular and Ventricular Arrhythmias in Patients With Heart Failure.

BACKGROUND: Patients with heart failure with reduced ejection fraction (HFrEF) frequently experience electrical disturbances, such as ventricular or atrial fibrillation (AF). Sacubitril/Valsartan (SV) therapy has been linked to lower rates of mortality, ventricular tachycardia (VT), and ventricular fibrillation (VF), with decreased reliance on implantable cardioverter-defibrillator (ICD) therapy. However, studies on the antiarrhythmic effects of SV in patients with ICD or cardiac resynchronization therapy defibrillator (CRT-D) devices are limited. This study aimed to evaluate the impact of SV …

Post cardiac injury syndrome after initially uncomplicated CRT-D implantation: a case report and a systematic review.

BACKGROUND: Post cardiac injury syndrome (PCIS) is an inflammatory process that occurs in the setting of injury to the pericardium, epicardium or myocardium. It may follow cardiac surgery, myocardial infarction, trauma, intracardiac ablation, percutaneous coronary intervention or implantation of a pacemaker or cardioverter-defibrillator. METHODS: In this paper, we report the first case of PCIS after implantation of a "Cardiac Resynchronization Therapy Defibrillator" (CRT-D) device and review systematically the available literature. To obtain information on PCIS after implantation of heart rhy…

Chronic active myocarditis with spatially and temporally heterogeneous lesions causing severe heart failure and intractable atrial arrhythmia: An autopsy case report.

UNLABELLED: The pathogenesis of chronic active myocarditis remains unclear. A 65-year-old man underwent permanent pacemaker implantation for sick sinus syndrome and pulmonary vein isolation for paroxysmal atrial fibrillation. Four years later, the left ventricular ejection fraction decreased from 51 % to 35 %, and the apical left ventricular inferior wall developed akinesis. Isolated cardiac sarcoidosis was suspected; however, prednisolone and optimal medical therapy failed to improve the symptoms. Even after cardiac resynchronization therapy followed by atrioventricular junction ablation for …

Treatment of Cheyne-Stokes respiration reduces arrhythmic events in chronic heart failure.

OBJECTIVE: This study aimed to investigate whether adequate treatment of Cheyne-Stokes respiration (CSR) reduces the risk of arrhythmic events in patients with chronic heart failure (CHF). METHODS: A cohort of 403 registry patients with CHF (LVEF≤45%, NYHA-class≥2) and implanted cardioverter-defibrillator devices (ICD) was studied. They underwent overnight polygraphy, with 221 having mild or no CSR (apnea-hypopnea index [AHI]<15/h), and 182 having moderate to severe CSR (AHI>15/h). Latter ones were offered therapy with adaptive servoventilation (ASV), which 96 patients accepted and 86 rejecte

RR interval variability in the evaluation of ventricular tachycardia and effects of implantable cardioverter defibrillator therapy.

BACKGROUND: An implantable cardioverter defibrillator (ICD) is the most reliable therapeutic device for preventing sudden cardiac death in p … g to the RR interval variability. METHODS: We analyzed 821 VT episodes in 69 patients with ICDs or cardiac resynchronization therapy defibrillators. VTs were classified as irregular when the difference between two successive beats was >20 ms in at least one of 10 RR intervals; otherwise, they were classified as regular. We evaluated successful termination using anti-tachycardia p …

Bifascicular pacing: A novel multipoint left conduction system pacing method for cardiac resynchronization.

BACKGROUND: Pacing a single fascicle of the left bundle branch leads to a delay of the segments served by the contralateral fascicle. Bifascicular (BiF) pacing may offer better cardiac resynchronization therapy (CRT) than left inferior fascicle (LIF) or left superior fascicle (LSF) pacing alone. OBJECTIVE: This study aimed to compare synchrony between BiF and single-fascicle pacing and present the procedural, clinical, electrocardiographic, and echocardiographic outcomes in CRT candidates who received BiF pacing. METHODS: The following parameters were investigated to assess acute differences…

Comparison between endocardial and epicardial cardiac resynchronization in an experimental model of non-ischaemic cardiomyopathy.

AIMS: Pacing from the left ventricular (LV) endocardium might increase the likelihood of response to cardiac resynchronization therapy. However, experimental and clinical data supporting this assumption are limited and controversial. The aim of this study was to compare the acute response of biventricular pacing from the LV epicardium and endocardium in a swine non-ischaemic cardiomyopathy (NICM) model of dyssynchrony. METHODS AND RESULTS: A NICM was induced in six swine by 3 weeks of rapid ventricular pacing. Biventricular stimulation was performed from 16 paired locations in the LV (8 epica…

Resynchronization therapy optimization by intracardiac impedance.

AIMS: For successful cardiac resynchronization therapy (CRT), an optimization of left ventricular (LV) lead position and stimulation timing is required. The feasibility of optimizing LV lead position, atrioventricular delay (AVd), and interventricular delay (VVd) in CRT using intracardiac impedance measurement was evaluated. METHODS AND RESULTS: Heart failure patients (n = 14, NYHA 13×III, 1×II, ejection fraction: 26 ± 6%, QRS: 165 ± 30 ms) were stimulated by AAI and biventricular (DDD-BiV) pacing in turn. Left ventricular lead site, AVd, and VVd were varied. An external pacemaker measured im…

Cardiac Resynchronization With or Without Defibrillator in Nonischemic Cardiomyopathy: A Nationwide Cohort Study (DECIDE-CRT).

BACKGROUND: With evolving therapy for heart failure with reduced ejection fraction, the benefits of adding a defibrillator in patients undergoing cardiac resynchronization therapy (CRT) remain unclear, particularly for nonischemic cardiomyopathy (NICM). This study evaluates long-term survival among patients with NICM treated with CRT defibrillator (CRT-D) versus CRT pacemaker (CRT-P). METHODS: DECIDE-CRT (Cardiac Resynchronization With or Without Defibrillator in Non-Ischemic Cardiomyopathy) is a multicenter cohort study across 170 US Veterans Affairs hospitals. We identified patients with NI…

Surgery for severe mitral regurgitation and left ventricular failure: what do we really know?

In ischemic and nonischemic cardiomyopathy, functional mitral regurgitation (MR) results from geometric abnormalities of the ventricle, whic … lities can be favorably influenced by antiremodeling pharmacologic therapy and in selected cases by cardiac resynchronization therapy. Surgical repair of functional geometric MR with an undersized complete rigid annuloplasty ring can abolish MR and is associated with improved functional status and left ventricular remodeling. It is unclear if surgery is associated with improved su …

A tale of two QRS: Misleading irregular tachycardia in an ICD carrier.

We report a 52-year-old man with coronary artery disease, reduced left ventricular ejection fraction, and a cardiac resynchronization therapy-defibrillator (CRT-D), admitted to the emergency department for palpitations. Surface ECG showed a wide-complex tachycardia with a 2:1 ventricular-to-atrial (V:A) relationship. Device interrogation revealed episodes detected in the ventricular tachycardia monitor zone, according to the atrioventricular discrimination algorithms. Detailed analysis of surface ECG and device intracardiac electrograms was performed, followed by an electrophysiological study …

Continuous Pacing and Recording Technique: A Real-Time Feedback Approach for Left Bundle Branch Pacing.

Left bundle branch pacing demonstrates significant clinical value in both prevention of right ventricular pacing-induced cardiomyopathy and resynchronization therapy following left bundle branch block. The current intermittent recording technique requires repeated interruptions during implantation to test parameters, increasing procedural complexity and time. In recent years, the application of stylet-driven leads and rotatable connectors combined with lumenless leads has enabled a continuous pacing and recording technique. This approach facilitates beat-by-beat monitoring of electrocardiogram

Transcatheter Mitral Valve Repair Improves Conduction and Function in a CRT Candidate.

Dilated cardiomyopathy (DCM) with severe mitral regurgitation (MR) and left bundle branch block (LBBB) typically indicates cardiac resynchronization therapy (CRT). However, device-related complications can limit CRT's applicability in some patients. A 77-year-old woman with DCM, severe MR, and LBBB underwent mitral valve transcatheter edge-to-edge repair (M-TEER) because of high risk for CRT-related complications. Post-procedure, she showed marked improvement in symptoms, left ventricular function, and QRS duration. This case highlights M-TEER as a potential initial strategy in selected patien…
