A genetic study of febrile convulsions.

307 probands with febrile convulsions classed as the simple type (131 children) and the complicated type (176 children) were genetically analyzed. There was a tendency toward familial aggregation of febrile convulsions, and genetic involvement was suggested. The multifactorial mode of inheritance best agreed with the observations. (1) The ratio of incidence of febrile convulsions in siblings of probands in the present study (19.9%) to the incidence in the general population (2.9%), i.e., 6.85, was rather close to the expected ratio of 5.87 from the multifactorial inheritance system. (2) The in

Role of viruses in febrile convulsions.

A disseminated viral illness was demonstrated by isolating a virus from the CSF, blood or urine in 27% of 73 children who were admitted to hospital after a first febrile convulsion. However, a viral aetiology could be implicated for 86% of the children after combining results of tissue culture, electron microscopy, mouse inoculation, complement fixation tests, and interferon assay. Parallel bacterial cultures showed a possible pathogen in 29% of children, but in only 4% was the pathogen isolated from the CSF, blood, or urine. No correlation was found between the nature of the pathogen (or evid

Hyponatraemia in children with febrile convulsions.

In a study of 23 children admitted to hospital with a febrile convulsion, mild hyponatraemia was found on 8 occasions. In 6 of these cases there was evidence of inappropriate secretion of antidiuretic hormone. The hyponatraemia is unlikely to be the cause of the convulsion, but probably predisposes the child to a subsequent convulsion during the same febrile illness.

Convulsion following intrathecal cephaloridine.

Following therapeutic intrathecal administration of cephaloridine to an 11-day-old premature infant with suspected meningitis, a generalized tonic convulsion was observed. Symptoms continued to subside over the next six hours, followed by postictal sleep. That convulsion and other symptoms were due to cephaloridine was confirmed when the second intrathecal administration produced the same symptom. Because of its possible damaging effect on CNS, it is strongly suggested that cephaloridine should not be administered directly into the cerebrospinal fluid space.

Neuropathology of the convulsive foal syndrome.

Brains from eighteen Thoroughbred foals suffering from the convulsive foal syndrome were examined neuropathologically and compared with brains from nine foals dying from different causes. In the brains of nine convulsive foals there was necrosis of the cerebral cortex, accompanied in three by severe necrosis of the diencephalon and brain stem; haemorrhage into the cerebral cortex was also present in most of this group. In the other nine brains from convulsive foals there was haemorrhage, often severe, especially in the white and grey matter of the cerebral cortex and in the cerebellum. Minimal

Convulsions immediately following althesin.

A case is reported in which a major generalised convulsive seizure followed the administration of Althesin. The patient had no clinically demonstrable predisposition to epilepsy. The fits were rapidly controlled by the intravenous injection of thiopentone.
