Revised criterion for diagnosis of ineffective esophageal motility is associated with more frequent dysphagia and greater bolus transit abnormalities.

BACKGROUND: Ineffective esophageal motility (IEM) has been defined by the presence of > or = 30% liquid swallows with contraction amplitude < 30 mmHg (ineffective swallows) in the distal esophagus ("old" IEM). A recent study with combined multichannel intraluminal impedance and manometry (MII-EM) raised the question whether the manometric diagnosis of IEM should be based on a new definition: > or = 50% ineffective liquid swallows ("new" IEM). The aim of this study was to evaluate the association between the number of ineffective liquid swallows and symptoms and bolus transit in patients with "

[Botulism and dysphagia].

BACKGROUND: Botulism is a severe neuroparalytic, of an acute characteristic, afebrile and is caused by the action of a toxin produced by Clostridium botulinum. This toxin links itself to the receptors of the axon membrane of the motor neurons, preventing the release of acetylcholine in the neuromuscular junction, causing flaccid paralysis of the cranial nerves and skeleton musculature. AIM: To present the speech therapy procedures adopted with a patient with botulism and who was presenting dysphagia. METHOD: A male adult, with botulism, sent for a speech-language evaluation due to the presen

A mathematical model of human oesophageal motility function.

Recent advances in various observation methods revealed several unique characteristics of oesophageal peristalsis and its disorders. However, a framework for understanding the oesophageal motility pattern is lacking. Here, we propose a simple mathematical model of the human oesophageal motility function. The model comprises central nervous system signals, enteric nervous system neurons (interneurons and motoneurons) and oesophageal smooth muscles. The neural function implements excitable dynamics at the oesophageal body and toggle-switch dynamics at the lower oesophageal sphincter. The local s

EVALUATION OF ESOPHAGEAL ACHALASIA: FROM SYMPTOMS TO THE CHICAGO CLASSIFICATION.

INTRODUCTION: The diagnosis of achalasia may be suggested by clinical features but a complete work-up is required not only to confirm the diagnosis but also to grade the disease by severity or clinical subtype. OBJECTIVE: To review the current evaluation of esophageal achalasia and its correct comprehension. METHOD: The literature review was based on papers published on Medline/Pubmed, SciELO and Lilacs, crossing the following headings: "esophageal achalasia"; "deglutition disorders"; "diagnostic techniques", "digestive system"; "endoscopy, digestive system"; "manometry". RESULTS: The diagn…

In addition, we performed cricopharyngeal myotomy to prevent postoperative deglutition disorders. 6 months postoperatively the patient is symptom-free and up to now there is no local recurrence of the tumour.

Functional disorders of swallowing.

Swallowing involves complex coordination of the neuromuscular anatomy and physiology of the oropharynx and esophagus, controlled by the enteric and central nervous systems. Dysphagia is classified as either oropharyngeal or esophageal and results from mechanical or structural disturbances. Videofluoroscopy, fiberoptic endoscopic evaluation of swallowing, barium swallow, manometry, and endoscopy are common modalities utilized in diagnosis, but none is as important as a patient's history. Functional dysphagia is a diagnosis of exclusion and is based on Rome criteria. Its mechanism is unknown but

Dysphagia: Thinking outside the box.

Dysphagia is a common symptom that is important to recognise and appropriately manage, given that causes include life threatening oesophageal neoplasia, oropharyngeal dysfunction, the risk of aspiration, as well as chronic disabling gastroesophageal reflux (GORD). The predominant causes of dysphagia varies between cohorts depending on the interplay between genetic predisposition and environmental risk factors, and is changing with time. Currently in white Caucasian societies adopting a western lifestyle, obesity is common and thus associated gastroesophageal reflux disease is increasingly diag

Association of Bolus Transit Time on Barium Esophagram With Esophageal Peristalsis on High-resolution Manometry and Nonobstructive Dysphagia.

GOAL: The goal of this study was to correlate upright and prone bolus transit time (BTT) on barium esophagography (BE) with esophageal peristalsis on high-resolution manometry (HRM) and self-reported dysphagia in patients with normal lower esophageal sphincter parameters on HRM. BACKGROUND: BTT on BE could be the gold standard for assessing the effectiveness of esophageal peristalsis if it can be quantified. MATERIALS AND METHODS: Patients with normal lower esophageal sphincter parameters and standard-protocol BE from 2017 to 2020 were included. Patients were divided, based on the number of

[Treatment and management for severe dysphagia: medication, rehabilitation and surgery].

Treatment and management for severe dysphagia consist of medication, rehabilitation and surgery. It is important to understand the prognosis of causative disease and dysphagia. The use of drug for dysphagia is restrictive and not so effective. Rehabilitation is widely applied. Restorative training is for acute cerebrovascular disorders and compensatory methods are used for chronic stage and/or progressive dysphagia. Surgical intervention, such as cricopharyngeal myotomy and laryngeal suspension is directed to get functional recovery. On the other hand tracheoesophageal diversion and laryngeal

Understanding non ulcer dyspepsia.

Non ulcer dyspepsia is one of the most common problems encountered in primary care practice. The underlying pathophysiology of non ulcer dyspepsia is not fully understood, but it is known that this condition is associated with H. pylori infection and motility disorder. The presenting abdominal symptoms are non specific: they include bloating, belching, flatulence, excessive fullness after eating and nausea. Psychological condition such as anxiety, depression and stress do play a role in the recurrence of symptoms. Upper GI endoscopy is necessary in patients who presents with alarm symptoms sug

Characteristics of Swallowing Function in People with Parkinson's Disease: A Scoping Review.

BACKGROUND: Most individuals with Parkinson's disease (PD) develop dysphagia during the course of their disease. It is crucial to comprehensively understand swallowing characteristics specific to PD for effective treatment. OBJECTIVES: To systematically analyze and synthesize swallowing characteristics in people with PD compared with healthy controls and to summarize the assessment methods used. METHODS: This scoping review was conducted in accordance with the Joanna Briggs Institute's (JBI) methodology. Systematic searches were conducted across six databases and one clinical trial registry.

A Sensory Method to Evaluate Food Texture for Oropharyngeal Dysphagia by Emulating Oral Tasks.

Dysphagia involves impairments in chewing and swallowing, requiring foods with controlled textural properties to ensure safe and efficient oral processing. Despite standardized frameworks like the International Dysphagia Diet Standardisation Initiative (IDDSI), reproducible sensory evaluation tools are limited. This study proposes a methodological framework for the sensory evaluation of texture-modified foods (TMFs) designed to mimic the oral processing patterns of individuals with oropharyngeal dysphagia (OD). A multidisciplinary expert panel developed a structured sensory protocol incorporat
