Recombinant Staphylococcus aureus exfoliative toxins are not bacterial superantigens.

Staphylococcal scalded-skin syndrome is an exfoliative dermatitis characterized by the separation of the epidermis at the stratum granulosum. This disruption is mediated by one of two Staphylococcus aureus exotoxins, exfoliative toxins A and B (ETA and ETB). Both ETA and ETB have been reported to be bacterial superantigens. A controversy exists, however, as other data indicate that these exotoxins are not superantigens. Here we demonstrate that recombinant exfoliative toxins produced in Escherichia coli do not act as T-cell mitogens and thus are not bacterial superantigens. These data fit the

Drug eruption induced by cyanamide (carbimide): a clinical and histopathologic study of 7 patients.

BACKGROUND: In some countries, cyanamide is used as an alcohol intake inhibitor with few harmful side effects. So far 3 cases of allergic contact dermatitis and only 1 of lichenoid drug eruption due to cyanamide have been reported. OBJECTIVES AND METHODS: We describe the clinical manifestations, course, histopathologic and immunohistochemical findings of 7 cases of cyanamide-induced drug eruption. RESULTS: Of the 7 patients, 6 developed exfoliative dermatitis and 1 a lichen-planus-like eruption. Three patients with exfoliative dermatitis showed high fever, severe itching, anorexia, insomnia

Implant infection caused by dermatitis: a report of two cases.

We report 2 cases of implant infection with Staphylococcus aureus associated with dermatitis. In both cases, the skin at the surgical site was normal and full antiseptic measures were taken. One patient had a history of chronic asteatotic eczema complicated by cellulitis; the other had generalised exfoliative dermatitis with an atopic aetiology. Dermatitis at sites remote from the operative site may be a contributing factor in implant infection.

Staphylococcal scalded skin syndrome in an immunocompromised adult.

Staphylococcal scalded skin syndrome, a generalized exfoliative dermatitis complicating infections by exfoliative toxin-producing strains of Staphylococcus aureus, is rarely observed in adults. In contrast to mortality in infants, mortality in adults is usually high. A case of generalized staphylococcal scalded skin syndrome in an immunocompromised woman is reported. Culture of skin biopsy and pleural fluid yielded identical strains of staphylococcus aureus belonging to phage group II. Exfoliative toxins A and B were detected in both isolates. As far as can be determined, this is the first rep

Omenn's syndrome: lessons from a red baby.

Exfoliative dermatitis and erythroderma in infancy are rare. Clinicians need to be alert to the possible diagnosis of Omenn's syndrome (OS), a rare form of combined immunodeficiency in infants presenting with exfoliative dermatitis, erythroderma, recurrent infections, eosinophilia and raised IgE. OS is fatal unless treated by bone-marrow transplantation (BMT). We describe a 3-week-old girl who presented with a widespread scaly erythematous rash and stomatitis, and was initially treated for presumed atopic eczema and primary herpes stomatitis. Aged 3 months, she developed erythroderma, diarrhoe

Erythroderma: review of a potentially life-threatening dermatosis.

Erythroderma, or generalized exfoliative dermatitis, is a disease characterized by erythema and scaling of greater than 90% of the body's surface. The resultant dysmetabolism is potentially life threatening. A detailed history is to identify and treat the underlying cause of this dermatitis. We present two cases of erythroderma in African patients and review this important disease.

Multisystemic eosinophilic epitheliotropic disease in three donkeys.

Multisystemic eosinophilic epitheliotropic disease (MEED) is a rare condition of equids characterized by eosinophilic infiltration of multiple organs. Clinical signs are variable depending on the affected organs. The most common clinical signs include chronic weight loss, diarrhoea and exfoliative dermatitis. Respiratory distress and raised liver enzymes are less frequently seen. The cause is unknown and the pathogenesis is poorly understood. There are less than 50 reported cases of horses with MEED. We now document the lesions in three donkeys with fluctuating or chronic loss of weight, letha

Erythroderma: a study of incidence and aetiopathogenesis.

Thirty cases of exfoliative dermatitis (22 male and 8 female) were selected at random for study. In addition to routine history and clinical examination, investigations like histopathology of skin and lymph nodes, peripheral blood smear for abnormal cells and bone marrow examinations were done. Aetiologically, the cases could be classified into 4 categories namely exfoliation in per existing dermatoses, drug-induced exfoliation, idiopathic exfoliation and exfoliation secondary to internal malignancy. Two cases showed abnormal monocytoid cells (sezary cells) in peripheral blood smear.

A real-world pharmacovigilance analysis of hepatitis B vaccine using the U.S. Vaccine Adverse Event Reporting System (VAERS) database.

Hepatitis B vaccines (HBVs) are widely used duo to their high clinical use and mild effects. However, as post-marketing data accumulate, several serious adverse events (SAEs) following HBV have been reported. Currently, quantitative studies based on real-world data are lacking, and information on their adverse events is limited. Adverse reaction signals of the HBV were mined and analyzed using the U.S. Vaccine Adverse Event Reporting System (VAERS) to provide a reference for the safe clinical use of this vaccine. Multiple statistical methods, including the reporting odds ratio (ROR) method, th

Papulosquamous diseases.

The papulosquamous diseases have certain common morphologic appearances which may at times lead to confusion in diagnosis. They do not have any common etiologic factor. The most common diseases in this category are psoriasis, seborrheic dermatitis, lichen planus, pityriasis rosea, and secondary syphilis. Exfoliative erythroderma is included here since it rarely may be a complication of a pre-existing papulosquamous disease such as psoriasis, seborrheic dermatitis, and lichen planus.

Unilateral Psoriasis on the Left Leg and Poliomyelitis of the Right Leg: A Case Report and Review of the Etiology.

Psoriasis is a common immune-mediated, inflammatory skin disease. However, unilateral psoriasis is rare and few cases have been reported. A 59-year-old male with a polio-affected right leg presented with exfoliative skin on the left leg for three months. He had been treated under the diagnosis of contact dermatitis with secondary infection. After the exfoliative skin lesions improved, psoriatic papules were noted, which was ultimately diagnosed as psoriasis and successfully treated with topical steroid and vitamin D agents. We propose that the Koebner phenomenon has acted as the triggering and

Erythroderma, hypogammaglobulinemia, and T-cell lymphocytosis. Occurrence following therapy with phenytoin.

Erythroderma and exfoliative dermatitis developed in a 17-year-old boy following therapy with phenytoin sodium. Immunologic studies performed early in the course of the dermatitis disclosed panhypogammaglobulinemia and a marked increase in T lymphocytes that responded poorly in vitro to T-cell mitogens. After therapy with prednisone, the dermatitis improved somewhat, and the patient's lymphocytes proliferated in vitro when exposed to phenytoin. We speculate that our patient's acute hypersensitivity reaction may have been mediated by an excessive number of phenytoin-sensitized suppressor-cytoto

Overcoming recurrent cutaneous reactions from imatinib using once-weekly dosing.

OBJECTIVE: To describe the use of once-weekly imatinib in a patient who developed recurrent cutaneous reactions associated with imatinib. CASE SUMMARY: A 65-year-old Asian woman received investigational treatment with imatinib 300 mg/d as maintenance therapy for acute lymphoblastic leukemia. Exfoliative dermatitis developed after 3 weeks of administration. Attempts to resume imatinib at lower dosages, with or without a low-dose corticosteroid, resulted in recurrent reactions. Once-weekly administration of imatinib, however, produced a tolerable, less severe reaction. After the woman had exper

Patch testing in cutaneous reactions caused by carbamazepine.

The usefulness of patch testing in the diagnosis of carbamazepine-induced allergic skin eruptions was studied in 18 patients with previous histories of skin eruptions caused by carbamazepine. The etiological role of carbamazepine was ascertained by peroral or topical provocation in 15 (out of 18) patients. The clinical reactions caused by the drug were classified as maculopapular exanthema with general symptoms (7 patients), other type of exanthema (3), exfoliative dermatitis (erythroderma) (3), fixed drug eruption (3), erythema multiforme (1) and urticaria (1). Patch testing showed positive r

Recurrent staphylococcal scalded skin syndrome in a 20-month old-A case report.

We present a case of a 20-month-old child with a history of atopic dermatitis that exhibited recurrent erythematous-bullous lesions consistent with Staphylococcal Scalded Skin syndrome (SSSS). SSSS is an exfoliative toxin-mediated skin disorder most commonly found in children. In this paper, we discuss the importance of recognizing the clinical symptomatology and progressive nature of SSSS, particularly in patients with a history of atopic dermatitis, to ensure prompt treatment and resolution of the syndrome.
