Extensive Cochlear Otosclerosis: CT-MRI Correlation and the Diagnostic Value of Contrast Enhancement.

Otosclerosis is a chronic disorder of the otic capsule. It is characterized by abnormal bone remodeling, which may extend beyond the fenestral region and involve the cochlea and other structures of the bony labyrinth. There are two main types of otosclerosis-retrofenestral (cochlear) and fenestral, the latter of which is much more common. We present multimodal imaging findings of extensive bilateral cochlear otosclerosis in a 59-year-old woman with chronic bilateral sensorineural hearing impairment, classified as grade 3 in the right ear and complete deafness in the left ear. High-resolution c

Laser versus conventional fenestration in stapedotomy for otosclerosis: a systematic review.

OBJECTIVES/HYPOTHESIS: To assess hearing results and complications following primary stapedotomy in otosclerosis patients comparing the use of laser and conventional techniques for fenestration. STUDY DESIGN: Systematic literature review. METHODS: A systematic bibliographic search was conducted in PubMed, Embase, the Cochrane Library, Cumulative Index to Nursing and Allied Health Literature, and Scopus. Studies reporting original data on the effect of laser fenestration, compared to conventional techniques, on closure of air-bone gap in patients undergoing primary stapedotomy were included.

Fenestration of the middle cerebral artery associated with an aneurysm--case report.

A 36-year-old female suddenly developed a severe headache. Computed tomography demonstrated subarachnoid hemorrhage in the left Sylvian fissure. Left carotid angiography revealed a fenestration at the distal portion of the M1 portion of the left middle cerebral artery, and an aneurysm arising from the proximal end of the fenestration. These findings were confirmed when the aneurysm neck was clipped. The embryonic origin of this lesion indicates that congenital factors may result in aneurysm formation.

Microdrill versus perforator for stapedotomy.

The purpose of this study was to compare the hearing results of performing stapedotomy using either a microdrill or a perforator. Two groups of patients, otherwise similar, were followed up for at least 2 years after the stapedotomy. It was possible to create 0.8 mm fenestra in 91.6% of our patients. There was no significant difference between the two groups in hearing results. The results showed a continuous improvement in the first 6 post-operative months after which the hearing stabilized. In our hands, the microdrill has not proved to be more traumatic than the perforator to the inner ear.

Selective chemical vestibulectomy: preliminary results with human application.

The involved membranous labyrinth of patients with medically intractable Meniere's disease is being accessed by means of a fenestration of the bony horizontal semicircular canal in preparation for the application of a 125 or 250 mcg flake of solid streptomycin. Sensory information originating in the affected vestibular end organ is selectively blocked from reaching the central nervous system. This uniformly results in the rapid elimination of vertiginous symptoms, and at these calculated doses yields preservation of (or in some cases improvement in) preoperative cochlear thresholds. The ration

Endolymphatic hydrops after fenestration: a temporal bone study with implications on the function of the utriculo-endolymphatic valve.

A temporal bone specimen demonstrating endolymphatic hydrops 13 years after fenestration of the lateral semicircular canal is presented. Fibro-osseous tissue extending from the lateral semicircular canal and reaching the vestibule produced fixation of the membranous wall of the utricle to the bony wall. Fixation and retraction of the utricule appears to have resulted in a permanently open utriculo-endolymphatic valve leaflet. Similar findings of fibro-osseous changes arising from the area of the crus commune and an open valve were found in a Meniere's disease specimen. The implications of thes

A comparison of fenestration of the horizontal canal and stapedectomy in the opposite ear.

The Lempert one stage horizontal semicircular canal fenestration was the surgical procedure of choice for otosclerosis from 1939 to 1954. Subsequently, mobilization and stapedectomy replaced fenestration. In the offices of Drs. Day, Jordan, and Caparosa, the horizontal semicircular canal fenestration was performed from 1940 to 1955; the mobilization, two years; and, thereafter, the stapedectomy. A significant number of patients have had fenestration on one side and stapedectomy on the opposite ear. Fifty patients seen consecutively were reviewed. Reliable office audiological testing has been a
