Distal tibial fractures are challenging injuries because of limited soft-tissue coverage, frequent high-energy mechanisms and the potential for soft-tissue compromise. Extensive surgical exposure may further disturb the local soft-tissue and biological environment. Ilizarov circular external fixation permits stable fixation with limited exposure of the fracture site. This prospective observational study evaluated fracture union, bone and functional outcomes and complications following closed or indirect reduction and Ilizarov circular external fixation in patients with open and closed distal t

BACKGROUND/AIM: As one of the common clinical diseases, fractures have many causes, mechanisms, healing and influencing factors; especially fracture healing is a long-term and complex process. Animal fracture models can simulate the various states of human fractures, and on this basis, the prevention, mechanism, and treatment of fractures can be studied to further guide clinical practice.
MATERIALS AND METHODS: Here, we developed a novel and portable device to create a closed fracture model in mice. We then compared this novel closed fracture model with the traditional open model in multiple

BACKGROUND: Zygomatic arch fractures can occur either as isolated fractures or as a part of multiple facial fractures. There are several techniques available for reduction of zygomatic arch fractures, which include closed reduction and open reduction with fixation using metallic splints, closed reduction via Gillies or a gingivobuccal approach is preferred for minimal invasiveness, short operative time, and low complication rate. The aim of the present study is to present a simple, easy, and reliable method for closed reduction of isolated zygomatic arch fractures.
PATIENTS AND METHODS: This

BACKGROUND: Various treatment modalities have been used in treating femoral shaft fractures, i.e., open intramedullary nailing, plating and external fixators but these does not always produce stable fixation and there is increased risk of infection, closed locked intramedullary nailing technique has being the gold standard and is a routine procedure but it requires proper orthopedic operation table with traction apparatus and the use of image intensifier. The use of open interlocking nailing technique doesn't require any special orthopaedic table nor it requires use of Image intensifier. The o

OBJECTIVE: The aim of this study was to assess muscle injury associated with upper and lower closed limb fracture using ultrasound, and to develop ultrasound classification criteria for muscle injury.
PATIENTS AND METHODS: Thirty patients with limb fracture and muscle injury participated in this study. Ultrasonography was used to assess muscle fibre, hematoma, vascular injury, and diameter growth rate. Injury was classified into three grades according to the ultrasound imaging: scores of less than, equal to, or greater than 9.
RESULTS: Of 30 patients, focal fibre rupture was observed in 11 c

PURPOSE: Distal radius fractures comprise the majority of hand- and wrist-related malpractice claims. We hypothesized that a majority of lawsuits would be for malunions resulting from nonsurgical treatment. Additional goals of this study were to quantify costs associated with claims, determine independent risk factors for making an indemnity payment, and illustrate trends over time.
METHODS: Seventy closed malpractice claims filed for alleged negligent treatment of distal radius fractures by orthopedic surgeons insured by the largest medical professional liability insurer in New York State (N

Pure tibiotalar dislocation without an associated fracture is an extremely rare injury. We present three cases of closed posteromedial tibiotalar dislocation without any associated fractures to the foot, ankle, or leg. All patients were treated conservatively with immediate closed reduction under general or local anaesthesia and immobilised in a short leg cast for six weeks without weight-bearing resulting in a satisfactory outcome at the final follow-up. A review of the literature is also presented in this paper.

INTRODUCTION: Only 12 cases of musculoskeletal tuberculosis (TB) that occurred at the site of recent blunt trauma have been previously described.
MATERIALS AND METHODS: A 43-year-old previously healthy man experienced an uncomplicated bursting fracture at the L2 vertebra without any spinal tuberculosis (ST)-associated evidence. During the conservative treatment, he was in good physical condition with back pain totally subsiding and occasional ambulation in a spinal brace, whereas 7 months after the original trauma, he presented with a characteristic manifestation of ST at the same vertebra.

Adequate pain relief is a requisite for a successful closed manipulative reduction (CMR) of fractures and dislocations. This prospective study was undertaken to assess the mode and adequacy of pain relief given to patients undergoing such procedures at Seremban Hospital from the 1st April to the 31st May 2001. All patients with fractures and dislocations scheduled to undergo CMR were included in this study. The type of sedative agents and analgesia administered were recorded. Demographic data and the type of fracture or dislocation of the selected patients were documented. A visual analogue sc

We present the outcome of non-operative management of fractures of the shaft of the femur in children. Thirty children with non-operatively managed unilateral fractures of the shaft of the femur were studied. The mean age was 6.5 years (range: 1.2 to 12). Axial, angular and rotational malunion were assessed clinically and radiologically by plain films and computed tomography (CT). All children had shortening of the fractured limb with a mean shortening of 1.8 cm (range: 0.5 to 3.5). A significant level of shortening was seen in children over 7 years. In the sagittal plane, all had anterior ang

PURPOSE: Most mandibular fractures occur because of blunt trauma. In some cases, the mandible can be weakened by the presence of an unerupted or impacted tooth. The fate of this tooth in the fracture line and its effects on bone healing remain an important outcome event.
MATERIALS AND METHODS: In this study, we analyzed the records of 117 patients with a total of 121 fractures having an unerupted tooth remaining in the fracture line who were treated and followed by the Dental Faculty of Ankara University between 1986 and 1996. All were open fractures, and none of the patients had a history of

STANDARDS: There is insufficient evidence to support treatment standards.
GUIDELINES: There is insufficient evidence to support treatment guidelines. Early closed reduction of cervical spine fracture-dislocation injuries with craniocervical traction is recommended to restore anatomic alignment of the cervical spine in awake patients. Closed reduction in patients with an additional rostral injury is not recommended. Patients with cervical spine fracture-dislocation injuries who cannot be examined during attempted closed reduction, or before open posterior reduction, should undergo magnetic res

Forty-four patients with 44 femoral shaft fractures were treated by AO interlocking nail technique at Ratchaburi Hospital. All cases were comminuted, segmental or rotational unstable fractures caused by traffic accidents. All patients had more than 90 degrees flexion of the knee within 2 weeks and resumed full range of motion of the knee within 2 months post-operatively. There were no serious complications found in the period of 29 months follow-up. We conclude that the closed interlocking nail is one of the treatments of choice for comminuted, segmental or rotational unstable femoral shaft fr

In four missed posterior shoulder dislocations, a trial of closed reduction was successful, but severe instability required further therapy. The glenohumeral joints were immobilized by 3 to 4 percutaneously inserted threaded pins. Removal of the pins after 4 to 6 weeks was followed by physiotherapy. After a follow-up of 4 to 42 months, the functional results of this simple procedure are good and there is no recurrence. Bending of the ends of the pins, a very careful follow-up to deal with pin migration, and adequate prevention of subsequent convulsions in epileptic patients are mandatory.

Background Supracondylar fracture is one of the most frequent pediatric traumas and surgically managed fractures. Multiple factors can contribute to delaying surgical management of supracondylar fracture, which is thought to lead to difficult reduction and more complications. Surgical treatment during the nighttime shift (from 20:00-8:00) might increase the complication rate including vascular injury, nerve injury, and the need to convert closed reduction to open due to multiple reasons including nontrained staff, exhausted on-call team, and other reasons. Objectives We are looking into the ef

Complete transection of the sciatic nerve following femoral fracture is extremely rare. In the setting of closed injury it has only been reported in two other cases. Here we present a teenage motorcyclist who sustained a closed left, mid-femoral fracture following a road traffic collision with complete transection of the sciatic nerve. Despite being a closed injury, the obvious limb deformity of the patient and extreme pain prompted immediate nerve block during the primary survey making formal neurological assessment difficult. This case highlights the possibility of complete major nerve trans
