Laparoscopic repair of hepatic herniation through a ventral incisional hernia: a case report.

BACKGROUND: Ventral incisional hernia is a common problem after abdominal surgery. Most patients with these hernias present with greater omentum and gastrointestinal prolapse. However, hepatic herniation through a ventral incisional hernia is a rare phenomenon that has been seldom reported in the literature. We report the case of a ventral incisional hernia with hepatic herniation treated with laparoscopic repair. CASE PRESENTATION: A 68-year-old Japanese women with a history of myocardial resection for hypertrophic cardiomyopathy 1 year earlier was admitted to our hospital with symptoms of v…

Role of imaging in managing lateral trocar site incisional hernia after abdominoperineal resection.

Trocar site incisional hernia (TSIH) is an unpleasant complication of laparoscopic surgery. A 70-year-old male with low rectal carcinoma underwent a laparoscopic abdominoperineal resection after completion of neoadjuvant radiotherapy. The postoperative recovery was smooth; however, he developed abdominal distension and pain over the previous drain site after removal on day 3. In view of diagnostic ambiguity, an imaging tool was requested as an adjunct to further management. Computed tomography of the abdomen showed small bowel obstruction secondary to herniated ileal loops passing through the …

Incidence and predictors of incisional hernia after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy.

Background and objectives: The incidence of incisional hernia (IH) after cytoreductive surgery plus hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) is largely unknown. Methods: We conducted a retrospective study to identify patients who underwent CRS/HIPEC from 2001 to 2016. Patients were followed postoperatively for a minimum of two years. The primary outcome was the occurrence of an IH identified either on CT scan or physical examination. Univariate and multivariable logistic regression models were used to test associations with IH. Results: We identified 155 patients who underwent CRS…

Small bowel obstruction from metastatic breast cancer masquerading as an obstructed incisional hernia.

Small bowel metastatic disease from primary breast cancer is rare. With this case report we illustrate a distinctly rare presentation of small bowel metastatic disease from breast cancer masquerading as an obstructed incisional hernia. This case is also an example of small bowel metastasis from an invasive ductal primary breast carcinoma as opposed to the more common pattern of small bowel metastasis from lobular carcinoma.

Migration of endoscopic biliary stent and small bowel perforation within an incisional hernia.

Long-term complications from biliary stents, such as migration and perforation, are unusual but may occur. Distal perforations most commonly involve the colon or distal ileum. This report describes the case of a perforated small bowel, secondary to a stent trapped within an incisional hernia. Patients with comorbid abdominal pathologies, including colonic diverticulae or abdominal herniae, may be at increased risk of perforation from migrated stents.

S116: Impact of incisional negative pressure wound therapy on surgical site infection after complex incisional hernia repair: a retrospective matched cohort study.

INTRODUCTION: Incisional negative pressure wound therapy (iNPWT) may reduce surgical site infections (SSI), which can have devastating consequences after incisional hernia repair. Few comparative studies investigate the effectiveness of this wound management strategy in this population. The objective of this study is to determine the effect of iNPWT on the incidence of SSI after complex incisional hernia repair. METHODS: All adult patients undergoing open incisional hernia repair at a single center from 2016 to 2019 were reviewed. A commercial iNPWT dressing was used at the discretion of the …

Laparoscopic or Robotically Assisted Colectomy with a Pfannenstiel Incision Reduces the Incisional Hernia Incidence.

OBJECTIVES: The present study examined the incidence of incisional hernia by comparing patients from whom a specimen was extracted either through a Pfannenstiel incision (PI) with an intracorporeal anastomosis or via a midline incision (MI) with an extracorporeal anastomosis. METHODS: The records of 370 consecutive patients who underwent a laparoscopic or robotically-assisted colectomy were retrospectively analyzed. Regardless of the clinical symptoms, incisional hernia was objectively diagnosed based on abdominal computed tomography findings. The surgical outcomes and incisional hernia incid…

[A low risk of ileus, incisional hernia and impact on female fertility after appendectomy].

Appendicitis is a common condition, which is often seen by the general practitioners as well as in the emergency departments. It is normally treated with laparoscopic appendectomy. This review gives an overview of the long-term surgical complications ileus and incisional hernia and of the impact of appendicitis and appendectomy on female fertility. The literature shows, that there is a low risk of ileus and incisional hernia after appendectomy, and that there is no change in female fertility after appendectomy, not even in cases where the appendix was perforated.

Postoperative-treatment following open incisional hernia repair: A survey and a review of literature.

INTRODUCTION: Incisional hernias of the abdominal wall are frequent complication after laparotomy (9-20%). Open incisional hernia repair with sublay mesh placement (SMP) on the posterior rectus sheath is described as being a sufficient method for repairing incisional hernia. In order to ensure wound healing and to therefore prevent recurrence, carrying an abdominal binder (AB) or a pressure dressing (PD) and physical rest for a certain time is the common postoperative recommendation, though the evidence for post-operative treatment is low. Hence, we conducted a survey to reveal the different r…

The partial underlay preperitoneal with panniculectomy repair for incisional abdominal hernia in the morbidly obese.

BACKGROUND: Because of high frequency, high morbidity, and difficulty of repair, incisional hernias in obese patients represent a particularly vexing and common problem for surgeons. The objective of this study was to describe a highly selective technique for incisional hernia repair with panniculectomy in the morbidly obese. We also describe perioperative characteristics and preliminary outcomes for a limited series of patients who underwent this procedure. METHODS: We performed a preperitoneal partial mesh underlay with a panniculectomy (PUPP) on 10 patients with incisional hernias and a bo…

Results of laparoscopic repair of primary and recurrent incisional hernias at a single UK institution.

In this study incisional hernia repairs at a single UK institution between 1994 and 2008 were analyzed with respect to short-term and long-term results. Prospectively collected data were analyzed retrospectively to ascertain outcomes, complications, and recurrences. Two hundred and twenty-seven operations were performed with 35% of the operations being for recurrent hernias. A self-centering suture technique was used. Median operating time was 55 minutes. There were 8 conversions and median hospital stay was 1 night. There were 52 complications (23%) including 3 postoperative bleeds, 3 mesh in…

[Use of synthetic materials for plasty of postoperative incisional hernias in interventions on the "open" colon].

Single-stage plasty of ventral hernias was performed in 40 patients with the use of synthetic materials and closure of colostomy (in 35 of them), resection of the sigmoid gut (in 3) or anterior resection of the rectum (in 2). Postoperative complications in the wound of the abdominal wall were found in 7 patients. There were no recurrences in the long-term (within 3-34 months) periods. Such single-stage interventions are thought to be possible.

[Morphology and biological integration of autodermoplasties in large incisional hernias. Results of an experimental study].

UNLABELLED: Their aim of the experimental study was to investigate the morphology and wound healing of "Kutisplastik"-technique, originally introduced by Loewe (1913) and Rehn (1914), from different conditions. We want to clarify the mechanism and development of revascularisation. RESULTS: The well known phases of wound healing also appear in the situation of a cutisplasty. In the exsudative reaction the cutis is covered from fibrin fibers. This leads to the development of granulation tissue. The cellular reaction demonstrates first the in-growth of capillary bundles and vessels in the collag

The volume-outcome relationship for incisional hernia repair: analysis of an administrative dataset for England.

PURPOSE: Incisional hernia is a common complication following abdominal surgery and repair can be complex. We aimed to use an administrative dataset for England to investigate the relationship between surgeon and provider volumes and outcomes. METHODS: We used data from the Hospital Episodes Statistics database for adult incisional hernia repair conducted in England from 1st April 2013 to 31st March 2025. The exposure variables were annual surgeon and provider volume defined as the number of procedures conducted in the 12 months prior to the index procedure. The primary outcome was reinterven…

EHS Guidelines on the Management of Primary Ventral and Incisional Hernias Under Emergency Conditions.

INTRODUCTION: Emergent primary ventral or incisional hernias (PVIHs) are a common cause of surgical admission, leading to significantly higher rates of morbidity and mortality compared to elective hernia repairs. Despite this, management varies widely due to a lack of evidence-based consensus. This article presents the new European Hernia Society (EHS) guidelines for the emergency treatment of adult patients with PVIH. MATERIAL AND METHODS: This project was developed by the EHS Science Committee following AGREE-S, GRADE, and GIN standards. A guideline panel, composed of general and emergency …
