Assessing the Insurance Deductible Effect on Outcomes After Elective Spinal Surgery.

BACKGROUND: Private insurers use the calendar deductible system, placing pressure on patients and medical personnel to perform medical services before the end of the year to maximize patient savings. The impact of the deductible calendar on patient-reported outcomes (PROs) after spine surgery is poorly understood. The objective of our study was to investigate if patients undergoing surgery in December had different PROs and demographics compared with all other months. METHODS: The Quality Outcome Database, a national spine registry, was queried for patients who underwent elective spine surger

Insurance status predicts acuity of thoracic aortic operations.

OBJECTIVE: Nonelective case status is the strongest predictor of mortality for thoracic aortic operations. We hypothesized that underinsured patients were more likely to require nonelective thoracic aortic surgery because of reduced access to preventative cardiovascular care and elective surgical services. METHODS: Between June 2005 and August 2011, 826 patients were admitted to a single aortic referral center and underwent 1 or more thoracic aortic operations. Patients with private insurance or Medicare (insured group, n=736; 89%) were compared with those with Medicaid or no insurance (under

Timing of Alveolar Bone Graft and Barriers to Care.

ObjectiveThe current standard timing for alveolar bone grafting (ABG) occurs during mixed dentition, typically between the ages of six and twelve. A delay in receiving this operation is associated with an increase in graft loss and an overall thinner maxilla. This study aims to determine whether socioeconomic barriers are associated with a delay in timely ABG.DesignA retrospective analysis of patients who received ABG at our institution since 2012. Patient demographics, cleft classifications, operative details, and surgical dates were examined. A logistic regression model was created using soc

A retrospective analysis of risk-reducing salpingo-oophorectomy performed in women diagnosed with hereditary breast and ovarian cancer at our institution.

BACKGROUND: Hereditary breast and ovarian cancer (HBOC) confers a markedly increased lifetime risk of breast and ovarian cancers. As no effective surveillance method for early detection of ovarian cancer has been established, risk-reducing salpingo-oophorectomy (RRSO) is recommended for patients with HBOC to prevent disease onset. We evaluated the clinical characteristics of patients with HBOC and the surgical outcomes of RRSO performed at our institution. METHODS: We retrospectively reviewed women diagnosed with HBOC at our institution between 2018 and 2024. For analyses of surgical outcomes

Herbal decoction and lumbar spine surgery in patients with lumbar disc herniation: a real-world study using linked electronic health records and claims data.

INTRODUCTION: Lumbar disc herniation (LDH) is a major cause of low back pain and may require surgical intervention when conservative or non-surgical treatment fails. In Korea, herbal medicine is commonly used as part of non-surgical care; however, its long-term effect on the risk of lumbar surgery has been difficult to assess because herbal decoctions have historically not been recorded in national insurance claims data. METHODS: We conducted a retrospective cohort study by linking electronic health records (EHRs) from four Korean medical hospitals with National Health Insurance claims data.

Factors Influencing Patient Satisfaction with Total Joint Replacement Surgery.

BACKGROUND: Replacement arthroplasty surgery is a surgical procedure that is needed to restore the activity of a joint. Patient satisfaction regarding arthroplasty surgery is influenced by post-operative complication rate, ability to afford care, and quality of interaction. In this study, we assessed patients' postoperative complications, mobility, and pain management as proxies for their overall satisfaction with total joint replacement (TJR). METHODS: An anonymous web-based survey was conducted for patients who had undergone any total joint replacement surgery in the US. Respondents were ad

Inhospital Surgical Disparities.

Access to and quality of health care are fundamental determinants of surgical outcomes, yet disparities remain prevalent due to a mix of patient, provider, and systemic factors. Rural residents often face longer travel times and reduced hospital access, exacerbating inequities. Inhospital disparities are influenced by race, socioeconomic status, and insurance coverage, with Black patients, those of lower socioeconomic status, and the uninsured experiencing higher rates of surgical complications and mortality. Socioeconomic disadvantage further limits access to health-promoting resources. Famil

Variability in technical fee billing for cardiac CT across congenital cardiac centers.

BACKGROUND: Cardiac Computed Tomography (CCT) is increasingly used to provide 2D, 3D and 4D information in patients with congenital heart disease of all ages. Historically, negotiated rates for professional and technical fees associated with cardiac imaging were confidential, with variability in professional, technical and global charges, reimbursement and cost to patients for the same current procedural terminology (CPT) code at different institutions. Billing transparency is a key component of both the CARE act passed in 2020 and the Health Care PRICE Transparency Act 2.0 passed in 2021. Ins

Rates, costs, return to work and reoperation following spinal surgery in a workers' compensation cohort in New South Wales, 2010-2018: a cohort study using administrative data.

BACKGROUND: Internationally, elective spinal surgery rates in workers' compensation populations are high, as are reoperation rates, while return-to-work rates following spinal surgery are low. Little information is available from Australia. The aim of this study was to describe the rates, costs, return to work and reoperation following elective spinal surgery in the workers' compensation population in New South Wales (NSW), Australia. METHODS: This retrospective cohort study used administrative data from the State Insurance Regulatory Authority, the government organisation responsible for reg

[Self-mutilation as insurance fraud].

The case of a self-amputation of the left-sided forefinger is presented. It was an isolated smooth amputation near the basic ankle of the finger without any accompanying injury of other fingers. The victim (a physician) claimed financial compensation from his accident-insurance; his contract included special disability taxes for finger injuries. However, the insurance company did not pay but was able to demonstrate--by means of a medico-legal reconstructive expertise--that the amputation was voluntary and self-inflicted. The argumentation concerning self-mutilation is presented (including the

HOUT-29. HEALTHCARE COSTS FOR HIGH-GRADE GLIOMAS: A POPULATION-BASED STUDY

Abstract Clinical management of patients with high-grade gliomas (HGGs) is very challenging both clinically and financially. As costs of cancer care in the US are expected to increase and oncology care is switching from volume-based to value-based pattern, efforts are needed to establish effective and efficient management of patients with HGGs. However, relevant data are limited. In this study, we retrospectively identified 88 primary HGGs patients diagnosed and treated at our institution between January 1, 2011 and February 28, 2017 and linked patient clinical information from electronic heal

Comparing Clinical and Patient-Reported Outcomes After Anterior Cervical Discectomy and Fusion Among Patients Aged 65 and Older Based on Insurance Type.

STUDY DESIGN: Retrospective cohort. OBJECTIVE: Evaluate the impact of insurance type on outcomes of ACDF among patients 65 years or older. SUMMARY OF BACKGROUND DATA: Despite a 38.7% inflation-adjusted decrease in reimbursement for ACDF procedures in Medicare patients aged 65 years and older over the past decade, ACDF volume has increased by 24%. Value-based care studies have investigated Medicare insurance as a predictor of postoperative outcomes. However, literature examining PROMs by Medicare status, especially for elderly patients, remains limited. METHODS: Patients aged 65 years or old

Surgical Scheduling Errors During Manual Data Transfer.

BACKGROUND AND OBJECTIVES: Retrospective studies examining errors within a surgical scheduling setting do not fully represent the effects of human error involved in transcribing critical patient health information (PHI). These errors can negatively impact patient care and reduce workplace efficiency due to insurance claim denials and potential sentinel events. Previous reports underscore the burden physicians face with prior authorizations which may lead to serious adverse events or the abandonment of treatment due to these delays. This study simulates the process of PHI transfer during surgic

Socioeconomic differences between medically and surgically treated prolactinomas: a retrospective review of 598 patients.

OBJECTIVE: Socioeconomic status (SES) is known to affect presentations and outcomes in pituitary neuroendocrine tumor resections, but there is a paucity of literature examining its impact specifically on patients with prolactinomas, who may be treated medically or surgically. The authors sought to determine whether SES was associated with differences in treatment choice or outcomes for prolactinoma patients. METHODS: The authors retrospectively reviewed patient records at a high-volume academic pituitary center for prolactinoma diagnoses. Patients were split into medically and surgically trea
