Enhanced recovery after surgery (ERAS) implementation in cytoreductive surgery (CRS) and hyperthermic IntraPEritoneal chemotherapy (HIPEC): Insights from Italian peritoneal surface malignancies expert centers.

BACKGROUND: Cytoreductive surgery (CRS) combined with Hyperthermic Intraperitoneal Chemotherapy (HIPEC) is a complex procedure that involves extensive peritoneal and visceral resections followed by intraperitoneal chemotherapy. The Enhanced Recovery After Surgery (ERAS) program aims to achieve faster recovery by maintaining pre-operative organ function and reducing the stress response following surgery. A recent publication introduced dedicated ERAS guidelines for CRS and HIPEC with the aim of extending the benefits to patients with peritoneal surface malignancies. METHODS: A survey was condu

Nuck Cyst, an Unexpected Amethyst Gem in the Inguinal Canal: A Case Report and Literature Review.

AIM: Nuck cyst is rare female saccular hydroceles. It appears at birth or months or years later in adulthood, resulting from imperfect closure of the canal of Nuck. Its diagnosis is often incorrect and confused with an inguinal or femoral hernia because of its extreme rarity. Consequently, surgery often leads to intraoperative surprises. We aim to increase knowledge and awareness of this rare pathology to improve the differential diagnosis of female inguinal masses that have been present for several years and to avoid intraoperative surprises. Therefore, with this study we want to highlight th

Anaphylactic Reaction to Patent Blue Dye After Supermicrosurgical Lymphaticovenular Anastomosis for the Treatment of Secondary Lymphedema.

For decades, patent blue dye (PBV) has been utilized for sentinel lymph node biopsy and lymphatic mapping in breast cancer and melanoma staging and treatment. Fistulography and intraoperative display of lymphatic vessels for lymphaticovenular anastomosis (LVA) are frequent applications. Although its anaphylactic potential is well described, PBV is used routinely. We present the case of a 71-year-old female patient, who underwent LVA for the treatment of chronic secondary lymphedema and experienced a postoperative anaphylactic reaction including a blue-colored drug-induced maculopapular exanthe

Peripherally Inserted Central Catheters as Potential Risk Factors for Failure in Pedicled Latissimus Dorsi Flaps.

Pedicled latissimus dorsi musculocutaneous flaps are reliable options for reconstruction of torso and head and neck defects. After ensuring that no kink or external compression occurs on the pedicle, the survival rate exceeds 99%. However, we experienced a complete necrosis of the flap after insertion of a peripherally inserted central catheter line. Intraoperative findings demonstrated thrombosis of the subclavian vein over a large segment, which was found to be the cause of flap failure. The clinical significance of this case report is aimed at raising awareness against insertion of peripher

Internal hernia through the foramen of Winslow: a rare cause of intestinal obstruction with imminent ischaemia of the caecum.

An internal hernia through the foramen of Winslow represents a rare surgical pathology. This report describes a case with incipient caecal ischaemia and discusses current diagnostic and therapeutic approaches. A patient in his early 60s presented at the emergency department with abdominal pain and last bowel movement three days prior. A CT scan of the abdomen suggested an internal hernia into the lesser sac. Intraoperatively, the suspected diagnosis could be confirmed laparoscopically with a twisted mobile caecum herniating through the foramen of Winslow. Due to a suspected ischaemia and lapar

Giant iris cyst with corneal perforation.

PURPOSE: To report a case of a giant iris cyst with corneal perforation. CASE REPORT: A 43-year-old woman presented with repeated episodes of pain and decreased vision in her left eye for 3 years. There was no history of trauma. Slit lamp examination showed a nasal limbal corneal perforation and a mass in the anterior chamber. Based on clinical examination, B-scan ultrasonography, and anterior segment optical coherence tomography, a presumed diagnosis of anteriorly subluxated lens was made. However, intraoperatively, the patient's own crystalline lens was found behind the anterior chamber mas

Intraoperative management of renal allograft venous-calyceal fistula and incidental renal cell carcinoma during renal transplantation: a case report.

This case report describes rare intraoperative complications during renal allotransplantation. A 59-year-old man underwent an expanded criteria deceased donor renal transplantation. A wedge biopsy, as per institution protocol, was performed prior to surgery. After vascular anastomoses, the kidney was reperfused; immediate significant hematuria was noted from the ureter. After exploration, compression of the wedge biopsy site stopped the bleeding, suggesting a venous-calyceal fistula. An incision at the wedge biopsy site was made to do an open repair, yielding a small suspicious lesion. Frozen

Hidden Treasure: Congenital Adhesions Necessitating an Alternative Approach to Laparoscopic Appendicectomy.

Congenital adhesions are rare findings in adults. We present a case of appendicitis in a middle-aged male with extensive congenital adhesions of the terminal ileum to the right lateral abdominal wall. The small bowel mesentery completely obscured the inflamed appendix. Alternative techniques were required as a result of these intraoperative discoveries, and substantial adhesiolysis was carried out before a successful appendicectomy. Although the congenital adhesions described here are extremely uncommon, the authors suggest that practitioners should be aware of them because they could change t

Ergonomic Considerations in Urologic Surgery.

PURPOSE OF REVIEW: This paper aims to discuss the various work-related musculoskeletal disorders (WRMDs) among urologists and provide an overview of the latest recommendations to improve awareness of ergonomic principles that can be applied in the operating room, with special consideration of challenges faced during pregnancy. RECENT FINDINGS: Urologists suffer from a large burden of WRMDs. The main drivers of pain associated with the various surgical approaches include repetitive movements, static and awkward body positions, and the use of burdensome equipment. Pregnant surgeons are at an ev

Role of preoperative CT angiography with multimodality imaging reconstruction to perform laparoscopic Complete Mesocolic Excision (CME) and Central Vascular Ligation (CVL) in right-sided colon cancer: Is it really useful? A prospective clinical study.

BACKGROUND: The concept of complete mesocolic excision (CME) and central vascular ligation (CVL) in right colonic resections appears to improve the oncological outcomes. The highest rate of complications reported in the literature in patients undergoing CME is related to difficult surgical manuevers and intraoperative bleeding due to the central vascular dissection. METHODS: We used preoperative findings obtained with the CT angiography, multiplanar reformation (MPR), maximum intesity projection (MIP) and 3D volume rendering (VR) technique to verify if this preoperative radiological assessmen

5th National Audit Project (NAP5) on accidental awareness during general anaesthesia: patient experiences, human factors, sedation, consent, and medicolegal issues.

The 5th National Audit Project (NAP5) of the Royal College of Anaesthetists and the Association of Anaesthetists of Great Britain and Ireland into accidental awareness during general anaesthesia (AAGA) yielded data related to psychological aspects from the patient, and the anaesthetist, perspectives; patients' experiences ranged from isolated auditory or tactile sensations to complete awareness. A striking finding was that 75% of experiences were for <5 min, yet 51% of patients [95% confidence interval (CI) 43-60%] experienced distress and 41% (95% CI 33-50%) suffered longer term adverse effec

Histopathologically Confirmed Nuck's Cyst Mimicking Lymphadenopathy in a Young Female: A Case Report.

Hydrocele of the canal of Nuck is an exceedingly rare congenital anomaly in females, often mimicking inguinal hernias and frequently misdiagnosed due to limited awareness. This report presents a 26-year-old woman with cyclical inguinal swelling, ultimately diagnosed intraoperatively as a Nuck's cyst, confirmed by histopathology revealing endometrial tissue within the cyst. The condition arises from persistent communication between the peritoneal cavity and the inguinal canal along the round ligament, leading to cyst formation. Surgical excision remains the definitive treatment, with approaches
