Anatomic evaluation of the vertical face-lift in cadavers.

IMPORTANCE: This study investigates how different orientations of tension vectors affect the amount of soft-tissue lift in specific cervicofacial regions. OBJECTIVES: To compare differences in cosmetic neck and face changes generated by 3 different face-lift techniques, to quantify the amount of lift across different points on the face, and to quantify changes in platysmal dehiscence in each of 3 standard superficial musculoaponeurotic system plication face-lift techniques applied to fresh-frozen cadaver heads. DESIGN, SETTING, AND PARTICIPANTS: Ten cadaver heads in an academic tertiary care

Safety of cranial fixation in endoscopic brow lifts.

INTRODUCTION: The endoscopic brow lift technique relies on brow mobilization and often soft fixation to the underlying calvarium. While the endoscopic brow lift has been used safely, there are anecdotal reports of cerebrospinal fluid leak. We sought to measure calvarial thickness to improve the safety of cranial fixation. METHODS: A retrospective review was performed of T2 weighted MRIs of the face of 28 patients. Calvarial thickness was measured on 10 coronal planes, from 3 centimeters (cm) anterior to 6 cm posterior to coronal suture. Fifteen points were measured on each coronal plane, star

The web lift and posterior pull for the aging face.

There are many factors that contribute to the aging neck. We have recently begun employing a percutaneous suture spanning the submentum combined with a posterior platysma pull. We present our initial results with this technique. In this retrospective study, subjects underwent the combined procedure with and without concomitant rhytidectomy. Cephalometric analysis was used to compare the preoperative cervicomental angle (CMA) and hyomental distance (HMD) with postoperative values at 4 weeks. Subjects also were queried on their postoperative satisfaction. Twenty-five subjects were included in th

Sinus lift procedures: an overview of current techniques.

For more than 30 years the maxillary sinus augmentation graft has been a mainstay of implant-directed maxillary reconstruction. The purpose of this article is to review the fundamentals of maxillary sinus reconstruction including anatomy and physiology of the sinus, indications for surgery, preoperative evaluation, surgical techniques, and management of complications. While there are some relative contraindications for the procedure, there are almost no absolute contraindications. With preparation, education, and experience, the maxillary sinus augmentation/elevation graft is a procedure that

An analysis of unsolved problems of face-lift procedures.

The face-lift/rhytidectomy, considered a premier surgical procedure in the specialty, is designed to reverse the effects of aging on the face. Yet our understanding of aging is incomplete, for example, only recently has it been recognized that there are different fat compartments in the face: how aging affects these different compartments is not known. Some surgeons consider the baggy lower eyelid to be due to excess fat and remove the fat. Others consider the problem to be due to a weak orbital septum and recommend septal tightening, while recent work suggests that facial bone retrusion is re

Searching for ski-lift injury: an uphill struggle?

Injuries arising from ski-lift malfunction are rare. Most arise from skier error when embarking or disembarking, or from improper lift operation. A search of the literature failed to uncover any studies focusing specifically on ski-lift injuries. The purpose of this study was to identify and characterise ski-lift injury resulting in hospitalisation and comment on barriers to reporting and reporting omissions. New Zealand hospitalised injury discharges 2000-2005 formed the primary dataset. To aid case identification these data were linked to ACC compensated claims for the same period and the da

Anatomy of tympanoparotid fascia relating to neck lift.

The aim of this study is to elucidate anatomical detail of the tympanoparotid fascia (TPF), deployed anteroinferiorly to the tragus, in relation to neck lift and platysmaplasty. Forty-one hemifaces of 25 Korean adult cadavers (age range: 43-101 years, 19 males and 6 females) were used for the study. Thirty-seven were dissected. Twenty-one were used for tension measurement and 4 for the histologic study. Tympanoparotid fascia was found in almost 36 hemifaces (100%). It was white-colored dense connective layer anteroinferior to tragus. The whitish fascia was a dense connective tissue layer deplo

Update on minimally invasive face lift technique.

BACKGROUND: The aging face is characterized by loss of skin elasticity, fat resorption, loss of muscle tone and volume, and loss of bone volume. Restorative procedures should be based on the condition of the soft tissues and the relationship between these tissues and the existing skeletal volume and can be performed through open, endoscopic, or minimally invasive approaches. OBJECTIVE: A minimally invasive approach to the face lift is presented, updated with useful details that the author has incorporated into his clinical technique on the basis of 15 years of experience. METHODS: The minima

Transblepharoplasty forehead lift and upper face rejuvenation.

A complete upper face rejuvenation via blepharoplasty incisions is feasible. This technique is indicated in: (1) patients with male pattern baldness, (2) patients with a history of hair transplants, (3) patients with excessively high foreheads, (4) patients in which upper blepharoplasty is planned simultaneously with the brow lift, (5) patients with spastic frontalis syndrome, and (6) patients needing periorbital or bicularis muscle repositioning. Standard upper blepharoplasty incisions are made. Dissection is proceeded to the superior orbital rim. All the soft tissues, including the retro-orb

The Role of SMAS Lifts in the "Deep Plane" Age.

The deep plane facelift has achieved near-universal prominence in contemporary surgical discourse, yet the evidence supporting its superiority over well-executed SMAS techniques remains limited. These authors contend that SMAS lifts offer distinct anatomic and practical advantages that are increasingly relevant in contemporary practice. SMAS techniques mobilize the mobile SMAS with a greater degree of excursion than composite techniques permit, preserve rather than sacrifice the structural volume of the SMAS layer, and maintain independent control of skin and soft tissue vectors, allowing for

The Hydraulic Sinus Lift Technique: Opportunities and Limitations.

The sinus lift procedure, first introduced in the 1970s to address insufficient bone height in the posterior maxilla for dental implants, has greatly evolved over the years. Traditional invasive techniques, such as the lateral window approach, posed a high risk of complications, including membrane perforation. Recent developments have led to the creation of the hydraulic sinus lift, introduced in 2005, which uses hydraulic pressure to gently lift the sinus membrane, lowering risks and increasing precision. Studies have shown its high success rates, minimal postoperative discomfort, and improve

The Interscapular Lift: Unfolding the Tortuous Subclavian Artery.

OBJECTIVE: Transradial cardiac catheterization has gained popularity because of its reduced risk of access-site complications, yet procedural failures remain frequent, often caused by subclavian artery tortuosity. We propose an alternative maneuver to improve procedural success rates, addressing the challenge of subclavian artery tortuosity more efficiently than existing time-consuming and resource-intensive techniques. KEY STEPS: Place a towel between the patient's scapulae to retract the shoulders before advancing the catheter. POTENTIAL PITFALLS: Aside from the increased time taken to pla

A New Simplified Autogenous Sinus Lift Technique.

Oral maxillofacial rehabilitation of the atrophic maxilla with or without pneumatization of the maxillary sinuses routinely presents limited bone availability. This indicates the need for vertical and horizontal bone augmentation. The standard and most used technique is maxillary sinus augmentation using distinct techniques. These techniques may or may not rupture the sinus membrane. Rupture of the sinus membrane increases the risk of acute or chronic contamination of the graft, implant, and maxillary sinus. The surgical procedure for maxillary sinus autograft involves two stages: removal of t

Sinonasal Complications Following the Sinus Lift Procedure.

Background: Although the incidence of postoperative acute and chronic rhinosinusitis in patients undergoing a sinus lift procedure is relatively high, a paucity of rhinology literature examines the management of and outcomes for this patient population. The objective of this study was to review the management and postoperative care of sinonasal complications and identify possible risk factors that should be considered prior to and following sinus augmentation. Methods: We identified sequential patients who had undergone a sinus lift procedure and were referred to the senior author (AK) at a te

LIFT: a feasible option for primary and recurrent fistulas.

BACKGROUND: The development of a surgical solution for anal fistulas with minimal recurrence and morbidity remains a challenge. Ligation of the intersphincteric fistula tract (LIFT), however, is a sphincter sparing technique that provides complete resolution of the transsphincteric fistula as well as minimizing recurrence and fecal incontinence. Our goal is to evaluate the efficacy of fistula tract ligation for primary and recurrent fistulas. METHODS: A retrospective single-center case study was conducted from data between 2015 and 2019 in a large district hospital in Antwerp, Belgium. Patien

Prostatic Urethral Lift: Does Size Matter?

OBJECTIVES: To assess if prostatic urethral lift (PUL) can be as effective on larger prostates. PUL is an endoscopic device that retracts prostatic tissue to relieve benign prostatic hypertrophy (BPH) obstruction. In 2013, PUL was approved for patients with a prostate size of <80 g and no median lobes. The approval was primarily based on the L.I.F.T. study, which only evaluated patients with prostates between 30 and 80 g in size. MATERIALS AND METHODS: Seventy-four patients underwent PUL between April 2, 2014, and December 2, 2015, for BPH management. Fifty-one patients were in the <80 g pros
