Healthcare workers' views on the use of continuous positive airway pressure (CPAP) in neonates: a qualitative study in Andhra Pradesh, India.

BACKGROUND: Continuous Positive Airway Pressure (CPAP) is a form of non-invasive ventilatory support which is increasingly used in low- and middle-income countries to treat neonates with acute respiratory distress. However, it may be harmful if used incorrectly. We aimed to explore the experiences of doctors and nurses using CPAP in neonatal units in India and their views on enablers and barriers to implementation of CPAP. METHODS: Participants from 15 neonatal units across Andhra Pradesh were identified through purposive sampling. Eighteen in-depth interviews (IDI) with doctors and eight foc

Effects of the COVID-19 pandemic on the outcomes of HIV-exposed neonates: a Zimbabwean tertiary hospital experience.

INTRODUCTION: The COVID-19 pandemic has globally impacted health service access, delivery and resources. There are limited data regarding the impact on the prevention of mother to child transmission (PMTCT) service delivery in low-resource settings. Neotree ( www.neotree.org ) combines data collection, clinical decision support and education to improve care for neonates. Here we evaluate impacts of COVID-19 on care for HIV-exposed neonates. METHODS: Data on HIV-exposed neonates admitted to the neonatal unit (NNU) at Sally Mugabe Central Hospital, Zimbabwe, between 01/06/2019 and 31/12/2021 we

Examining the newborn with an open spinal dysraphism.

Spinal dysraphism includes a constellation of frequent and potentially complex malformations of the spine and spinal cord. Open spinal dysraphisms, the majority of which are myelomeningoceles, have a number of associated morbidities that require both immediate and lifelong medical care. The role of the neonatal nurse includes the immediate stabilization of the affected infant, systematic examination of the malformation, and implementation of evidence-based protocols to ameliorate the associated medical problems. Coordination of care and communication and support of the family are essential asp

"Try not to judge": mothers of substance exposed infants.

PURPOSE: To describe the hospital experiences of mothers who give birth to substance-exposed infants. STUDY DESIGN AND METHODS: Secondary analysis of data from a larger study that was focused on the experiences of Mexican-American mothers in the neonatal intensive care unit (NICU) was conducted. Semistructured interviews with five women who were recovering addicts on methadone were analyzed. Each of their infants spent time in an NICU following birth. The transcribed interviews were analyzed using qualitative content analysis. RESULTS: Four themes were identified: (a) "try not to judge," (b)

A descriptive survey on knowledge gap related to retinopathy of prematurity and its prevention and management among nurses in a tertiary care hospital.

PURPOSE: Retinopathy of prematurity (ROP) is a leading cause of severe visual impairment of childhood affecting preterm babies. The disease is sensitive to the quality of neonatal nursing care provided to preterm neonates in any neonatal intensive care unit. (NICU). The aim of this study was to assess the knowledge gap among nurses related to ROP and its prevention and management working in NICU in a tertiary care hospital. METHODS: In a cross-sectional descriptive survey, 53 nurses working in a selected NICU of a tertiary care hospital were enrolled. A pretested and validated self-administer

Types of intensive care units with the healthiest, most productive work environments.

BACKGROUND: The quality of nurses' work environments in hospitals is of great concern. The American Association of Critical-Care Nurses has specified 6 standards essential to a healthy (ie, satisfying and productive) work environment. These standards are sufficiently aligned to the Essentials of Magnetism processes to make this tool suitable for measuring healthy work environments. OBJECTIVES: To identify differences in staff nurses' perceptions of the work environment by type of intensive care unit. METHODS: A cross-sectional descriptive design with strategic sampling was used in this secon

Neonatal mortality in the United States is related to location of birth (hospital versus home) rather than the type of birth attendant.

BACKGROUND: Planned home births have leveled off in the United States in recent years after a significant rise starting in the mid-2000s. Planned home births in the United States are associated with increased patient-risk profiles. Multiple studies concluded that, compared with hospital births, absolute and relative risks of perinatal mortality and morbidity in US planned home births are significantly increased. OBJECTIVE: To explore the safety of birth in the United States by comparing the neonatal mortality outcomes of 2 locations, hospital birth and home birth, by 4 types of attendants: ho

Nurses' Perceived Barriers and Strategies to Promote Parental Involvement in Newborn Pain Management.

PURPOSE: To explore neonatal intensive care unit (NICU) nurses' perspectives on strategies and barriers to parental involvement in newborns' pain management (PM). BACKGROUND: Newborns undergo several painful procedures in the NICU that affect their neurodevelopment. When properly prepared and guided, parents can play an active role in their newborn's PM. Nurses play a crucial role in facilitating parental involvement. DESIGN: An exploratory, qualitative, and descriptive design was used. METHODS: Online focus group interviews were conducted with 24 nurses from three Portuguese NICUs. Inducti

Ethical issues related to caring for low birth weight infants.

BACKGROUND: Currently preterm births are the leading causes of newborn deaths and newborn mortality in developed countries. Infants born prematurely remain vulnerable to many acute complications and long-term disabilities. There is a growing concern surrounding the moral and ethical implications of the complex and technological care being provided to extremely low birth weight infants in neonatal intensive care units in the developed nations. RESEARCH PURPOSE: The purpose of this study was to describe the ethical and moral issues that neonatal intensive care nurses experience when caring for

Differences in preferences for neonatal outcomes among health care professionals, parents, and adolescents.

CONTEXT: In neonatal intensive care, parents make important clinical management decisions in conjunction with health care professionals. Yet little information is available on whether preferences of health care professionals and parents for the resulting health outcomes differ. OBJECTIVE: To measure and compare preferences for selected health states from the perspectives of health care professionals (ie, neonatologists and neonatal nurses), parents of extremely low-birth-weight (ELBW) or normal birth-weight infants, and adolescents who were either ELBW or normal birth-weight infants. DESIGN:

Hyperbilirubinemia.

Hyperbilirubinemia is the most common condition requiring evaluation and treatment in newborns. The clinical manifestation of hyperbilirubinemia-jaundice-occurs in 60% of normal newborns and nearly all preterm infants. Compared with conditions that require advanced pharmacologic and technologic treatment strategies, hyperbilirubinemia seems to be overshadowed and may lose the attention it deserves as a condition that has potentially devastating effects. Nurses must be vigilant when caring for babies with "just jaundice" by monitoring bilirubin levels, identifying infants at risk for developing

Study on the value of KMC combined with blue light irradiation in improving the therapeutic effect of neonatal jaundice.

Neonatal jaundice is a common problem that affects newborns. We aim to investigate the effect of Kangaroo Mother Care (KMC) combined with blue-light irradiation on enhancing the therapeutic effect in the treatment of neonatal jaundice. From May 2020 to August 2022, 89 neonates with pathological jaundice and their mothers were selected as participants. All neonates received blue-light irradiation. The participants were divided into two groups: 46 neonates cared for by KMC as the research group (RG) and 43 neonates receiving routine nursing as the control group (CG). The first lactation time, fi

Breastfeeding Mothers' and Nurses' Perceptions of Breast Milk Sharing and Donation for Research in Bhutan: A Qualitative Study.

INTRODUCTION: Breast milk is globally recognized as the optimal source of infant nutrition due to its immunological and neurodevelopmental benefits. In low-and middle-income countries like Bhutan, limited availability of donor milk and absence of milk banking infrastructure hinder neonatal nutrition. While breast milk sharing for feeding has been explored elsewhere, little is known about maternal and nursing perspectives of milk donation for research use within Bhutanese sociocultural context. OBJECTIVE: To explore breastfeeding mothers' perceptions of breast milk sharing and donation for bot

Continuous labor support by doulas and nurses and its impact on birth outcomes: a systematic review.

BACKGROUND: Continuous labor support provided by doulas, nurses, or midwives is increasingly recognized as an important component of respectful, woman-centered maternity care. Evidence suggests that continuous support during labor may improve maternal and neonatal outcomes by enhancing emotional support, facilitating labor progress, and reducing unnecessary obstetric interventions. However, variations in intervention models and healthcare settings have resulted in inconsistent findings regarding the effectiveness of different support providers. This systematic review aimed to evaluate the impa

Neonatal hematologic disorders.

Neonatal hematology is a complex subspecialty of pediatric hematology, combining the unique aspects of the maternal/fetal relationship, the delicate balance of coagulation factors, and the distinctive physiologic conditions of the newborn period. The objective of this article is to briefly review specific hematologic disorders that commonly present in the newborn period. Alloimmune cytopenias, polycythemia, thrombosis and bleeding associated with vitamin K deficiency will be discussed through a focus on pathophysiology, signs and symptoms, current treatment strategies, and implications for nur

Tin-metalloporphyrins: an answer to neonatal jaundice?

Hyperbilirubinemia is a common problem, affecting 45-60 percent of term infants and up to 80 percent of premature neonates. Phototherapy, the standard treatment for neonatal hyperbilirubinemia, is effective but has the potential for adverse effects. Prevention of bilirubin formation, rather than dependence on therapeutic measures to remove excess bile pigment, is a logical alternative. Metalloporphyrins--specifically tin-protoporphyrin (SnPP) and tin-mesoporphyrin (SnMP)--are being used experimentally to prevent and treat neonatal hyperbilirubinemia. This article reports the findings of studie
