Utility of Acoustic Change Complex as an Objective Tool to Evaluate Difference Limen for Intensity in Cochlear Hearing Loss and Auditory Neuropathy Spectrum Disorder.

Purpose This study aimed to investigate usefulness of acoustic change complex (ACC) as an objective measure of difference limen for intensity (DLI) in auditory neuropathy spectrum disorders (ANSD) and cochlear hearing loss (CHL). Method The study used a multiple static group comparison research design. Twenty normal-hearing individuals (NH), 19 individuals with ANSD, and 23 individuals with CHL underwent DLI measurement using behavioral (psychoacoustic) techniques and ACC. For eliciting ACC, a 500-ms, 1,000-Hz pure tone was presented at 80 dB SPL. Additionally, six variants of this stimulus wi

Distorted sound perception and subjective benefit after stapedotomy - a prospective single-centre study.

OBJECTIVE: To evaluate the quality of perceived sound after stapedotomy over a 1-year follow-up period focussing on incidence of dysacusis, particularly distorted sound perception (DSP). DSP was assessed by (i) determination of the frequencies and hearing level that such perceptions are elicited by pure tones (pure-tone-evoked distorted sound perception, PTE-DSP), a novel psychoacoustic measurement introduced in this paper, and (ii) assessment of patient-reported occurrence of DSP using the Amsterdam Post Operative Sound Evaluation (APOSE) questionnaire (APOSE-DSP). DESIGN: Prospective study.

A State-of-the-Art Review: Personalization of Tinnitus Sound Therapy.

Background: There are several established, and an increasing number of putative, therapies using sound to treat tinnitus. There appear to be few guidelines for sound therapy selection and application. Aim: To review current approaches to personalizing sound therapy for tinnitus. Methods: A "state-of-the-art" review (Grant and Booth, 2009) was undertaken to answer the question: how do current sound-based therapies for tinnitus adjust for tinnitus heterogeneity? Scopus, Google Scholar, Embase and PubMed were searched for the 10-year period 2006-2016. The search strategy used the following key wo

The effect of Transcranial Direct Current Stimulation in addition to Tinnitus Retraining Therapy for treatment of chronic tinnitus patients: a study protocol for a double-blind controlled randomised trial.

BACKGROUND: Currently, there still is no treatment that eliminates tinnitus in all patients. Recent studies have shown that Tinnitus Retraining Therapy (TRT) significantly improves quality of life for tinnitus patients. Also, several studies have reported that transcranial Direct Current Stimulation (tDCS) has a positive effect on attention, working memory, long-term memory and other cognitive processes. The aim of this randomised placebo-controlled double-blind study is to evaluate the added effect of tDCS to TRT in chronic tinnitus patients. To our knowledge, this is the first study to combi

Auditory Processing in Specific Language Impairment (SLI): Relations With the Perception of Lexical and Phrasal Stress.

PURPOSE: We investigated whether impaired acoustic processing is a factor in developmental language disorders. The amplitude envelope of the speech signal is known to be important in language processing. We examined whether impaired perception of amplitude envelope rise time is related to impaired perception of lexical and phrasal stress in children with specific language impairment (SLI). METHOD: Twenty-two children aged between 8 and 12 years participated in this study. Twelve had SLI; 10 were typically developing controls. All children completed psychoacoustic tasks measuring rise time, in

Bilateral transverse sinus stenosis in patients with tinnitus.

Tinnitus is a frequent complaint in patients affected by intracranial hypertension (IH). Recently, some studies have reported an association between idiopathic intracranial hypertension (IIH) and bilateral transverse sinus stenosis (BTSS). We investigated the relationship between BTSS and monosymptomatic tinnitus, regardless of its clinical characteristics, in subjects without clinical evidence of IH. We selected 78 subjects (all women, mean age 49.5 ± 10.36) affected by tinnitus, without clinical history of audiological and otological pathologies, enrolled among outpatients of the Institute o

On the role of the olivocochlear bundle in hearing: 16 case studies.

Earlier we presented data (Scharf et al. (1994) Hear. Res. 75, 11-26) from a young patient (S.B.) who had undergone a vestibular neurotomy, during which the olivocochlear bundle (OCB) was severed. Those data are complemented by measurements on 15 other patients-some like S.B. with normal audiometric thresholds, none with a loss greater than 35 dB at experimental frequencies. Comparisons of performance for the same ear before and after surgery or between the operated and healthy ears do not provide evidence that the lack of OCB input impairs the following psychoacoustical functions: (1) detecti

Broadband auditory stream segregation by hearing-impaired and normal-hearing listeners.

PURPOSE: To investigate the effects of hearing loss on auditory stream segregation of broadband inharmonic sounds. METHOD: Auditory stream segregation by listeners with normal and impaired hearing was measured for 6-component inharmonic sounds ("A" and "B") using objective and subjective methods. Components in the A stimuli ranged between 1000 and 4000 Hz, whereas B stimuli were generated at the same frequency ratio but scaled upward in frequency relative to the A stimuli. In Experiment 1, streaming was measured by having listeners detect a delay inserted into a sequence of A and B stimuli (A

Effects of Sensorineural Hearing Loss on Cortical Synchronization to Competing Speech during Selective Attention.

When selectively attending to a speech stream in multi-talker scenarios, low-frequency cortical activity is known to synchronize selectively to fluctuations in the attended speech signal. Older listeners with age-related sensorineural hearing loss (presbycusis) often struggle to understand speech in such situations, even when wearing a hearing aid. Yet, it is unclear whether a peripheral hearing loss degrades the attentional modulation of cortical speech tracking. Here, we used psychoacoustics and electroencephalography (EEG) in male and female human listeners to examine potential effects of h…

A loudness model for time-varying sounds predicts asymmetries in estimates of loudness change for sounds that increase or decrease in level.

Previous research has demonstrated a robust asymmetry in loudness judgements for rising and falling level tone stimuli, showing greater loudness and loudness change for rising sounds. In the literature, this has been interpreted to reflect an adaptive perceptual bias, resulting from an evolutionary advantage that increases the margin of safety when reacting to an approaching sound-producing object. The current experiment tested whether a loudness model was able to predict asymmetries in loudness-change judgments for rising and falling sounds, providing an alternative explanation to the adaptiv

Randomized Controlled Trial of a Perceptual Training Game for Tinnitus Therapy.

OBJECTIVE: Abnormal executive top-down control of attention may result in, or be a consequence of, tinnitus. On the basis of a study indicating the feasibility of a game designed to treat tinnitus, a Phase II controlled trial of an auditory attention training game was undertaken. MATERIALS AND METHODS: Measures of tinnitus, as well as behavioral and electrophysiological measures of attention, were compared before and after 20 consecutive days of 30-minute training sessions using a game developed with LabVIEW™ software (National Instruments Corp., Austin, TX). Fifteen participants played an ex

The differential effect of low- versus high-frequency random noise stimulation in the treatment of tinnitus.

Tinnitus is the sensation of a ringing, buzzing, roaring or hissing sound in the absence of an external sound. As tinnitus has been related to hyperactivity and synaptic plasticity changes in the central auditory system, invasive and noninvasive neuromodulation methods have been used to interfere with this underlying mechanism to reduce tinnitus loudness and distress. Recently, transcranial random noise stimulation applied over the auditory cortex induced a more pronounced effect on tinnitus loudness than transcranial direct current and alternating current stimulation. We performed tRNS over t

Effects of Long-Term Speech-in-Noise Training in Air Traffic Controllers and High Frequency Suppression. A Control Group Study.

OBJECTIVE: To evaluate 1) if air traffic controllers (ATC) perform better than non-air traffic controllers in an open-set speech-in-noise test because of their experience with radio communications, and 2) if high-frequency information (>8000 Hz) substantially improves speech-in-noise perception across populations. MATERIALS AND METHODS: The control group comprised 28 normal-hearing subjects, and the target group comprised 48 ATCs aged between 19 and 55 years who were native Spanish speakers. The hearing -in-noise abilities of the two groups were characterized under two signal conditions: 1) s

The Interplay of Preference, Familiarity and Psychophysical Properties in Defining Relaxation Music.

BACKGROUND: The stress response has been well documented in past music therapy literature. However, hypometabolism, or the relaxation response, has received much less attention. Music therapists have long utilized various music-assisted relaxation techniques with both live and recorded music to elicit such a response. The ongoing proliferations of relaxation music through commercial media and the dire lack of evidence to support such claims warrant attention from healthcare professionals and music therapists. OBJECTIVE: The purpose of these 3 studies was to investigate the correlational relat

Precursor effects on behavioral estimates of frequency selectivity and gain in forward masking.

The experiments presented in this paper explore the hypothesis that cochlear gain is reduced, in a frequency-specific manner, over the course of a sound (called a "precursor") which was designed to activate the medial olivo-cochlear reflex (MOCR). Psychophysical tuning curves (PTCs) and off-frequency growth of masking (GOM) functions were measured with two precursors. The on-frequency precursor condition, which was hypothesized to activate the MOCR at the signal frequency, produced a PTC with a lower best frequency in all subjects consistent with less gain. This same condition produced a GOM f

Two-tone distortion on the basilar membrane of the chinchilla cochlea.

Basilar membrane responses to pairs of tones were measured, with the use of a laser velocimeter, in the basal turn of the cochlea in anesthetized chinchillas. Frequency spectra of basilar membrane responses to primary tones with frequencies (f1, f2) close to the characteristic frequency (CF) contain prominent odd-order two-tone distortion products (DPs) at frequencies both higher and lower than CF (such as 2f1-f2, 3f1-2f3, 2f2-f1 and 3f2-2f1). For equal-level primaries with frequencies such that 2f1-f2 equals CF, the magnitude of the 2f1-f2 DP grows with primary level at linear or faster rates

Speaking rate, voice-onset time, and quantity: the search for higher-order invariants for two Icelandic speech cues.

The temporal structure of speech has been shown to be highly variable. Speaking rate, stress, and other factors influence the duration of individual speech sounds. The highly elastic nature of speech would seem to pose a problem for the listener, especially with respect to the perception of temporal speech cues such as voice-onset time (VOT) and quantity: How does the listener disentangle those temporal changes which are linguistically significant from those which are extrinsic to the linguistic message? This paper reports data on the behavior of two Icelandic speech cues at different speaking
