Perceptions of vascular access for intravenous systemic therapy and risk factors for lymphedema in early-stage breast cancer-a patient survey.

BACKGROUND: The choice of vascular access for systemic therapy administration in breast cancer remains an area of clinical equipoise, and pa … catheter (port) in 34%. Anthracycline-based regimens were associated with a greater use of central vascular access devices (cvads- that is, a picc or port; 86/97, 89%). Trastuzumab use was associated with greater use of ports (49/64, 77%). Although few patients (7%) reported being involved in the decisions about vascular access, most were satisfied or very satisfied (88 …

Central venous accesses in prone position during the pandemic period: A narrative review.

The unprecedented challenges posed by the global COVID-19 pandemic have magnified the significance of managing intensive care patients in pr … establishing central venous access. Before the pandemic, scant literature detailed the insertion of vascular access devices in prone or unconventional positions. Limited case reports and letters highlighted the feasibility of procedures like ultrasound-guided central catheter placement in patients undergoing surgery or with specific clinical needs. During the pandemic, a …

Recurrent and Relapsing Catheter-Related Bloodstream Infections in Patients Requiring Long-term Vascular Access.

Patients requiring long-term vascular access are at risk of experiencing recurrent or relapsing infections. Decisions to remove and replace these devices when infected depend on their clinical status, the pathogen involved, options for alternative access sites, the risk of mechanical complications or loss of access with attempted catheter replacement, and interruptions of care plans. This State-of-the-Art Review focuses on a team approach to the management of such complex patients. Management involves empiric and then directed systemic antimicrobial therapy, antimicrobial lock therapy, decisio

Medical adhesive-related skin injuries associated with vascular access.

Establishing vascular access and preventing infection, both at insertion and during ongoing care is generally the top priority; the maintenance of optimal skin integrity is often a distant secondary consideration. Skin can react to different types of dressings or adhesives, or problems can arise relating to the securement of lines or the development of sensitivities to cleaning solutions. Clearly, these scenarios are not limited to the securement of vascular access devices; however, a patient with a long-term vascular access device may not have other options for vascular access, which makes th…

Single versus double use of a suture-based closure device for transfemoral aortic valve implantation.

BACKGROUND: The most common method of percutaneous closure in transfemoral transcatheter aortic valve implantation (TAVI) employs two obtusely oriented ProGlide devices. The aim of this study was to assess the feasibility and safety of using a single ProGlide system for primary access site closure in comparison with the double ProGlide approach in an all-comers TAVI population. METHODS: Between March 2016 and December 2018, a total of 1105 patients underwent transfemoral TAVI for severe aortic stenosis at our center. Application of two ProGlide systems was standard until April 2017 (n = 432),

Incidence of vascular complications for electrophysiology procedures in the ultrasound era: a single-centre experience over 10,000 procedures in the long term.

BACKGROUND: Ultrasound (US) is being increasingly used to guide vascular access for electrophysiology (EP) procedures in many centres. Nonetheless, the incidence and predictors of vascular complications in the US era are limited. In this study, we describe our experience of vascular access-related complications associated with EP procedures which were performed with the routine use of US-guided vascular access. METHODS: A total of 10,158 consecutive EP procedures in 8361 patients performed from April 2014 (when our centre moved to a policy of routine US-guided vascular access for EP procedure

Propensity Matched Analysis of 14 French Suture- Versus Plug-Based Vascular Closure During Transfemoral Transcatheter Aortic Valve Replacement (TAVR).

BACKGROUND: Plug-based vascular closure devices (Pb-VCD) and suture-based vascular closure devices (Sb-VCD) are used for percutaneous vascular access site closure during transcatheter aortic valve replacement (TAVR). Until now, no clear superiority of either device was shown in studies comparing 18 F VCDs solely. However, there is no data exclusively comparing the 14 F Pb-VCDs against Sb-VCDs after novel 14 F low-profile third-generation heart valve delivery sheath use with focus on vascular complications. AIMS: This study aimed to compare the safety and efficacy of 14 F Pb-VCD to Sb-VCD foll

Fatal Budd-Chiari syndrome associated with Hemodialysis Reliable Outflow (HeRO) graft.

Population aging and prolonged time on dialysis pose challenges to vascular access for nephrologists and vascular surgeons. HeRO grafts are an option used for patients with central venous obstruction and without the possibility of other vascular access on upper limbs. Some long-term complications, namely access thrombosis, infection, and limb ischemia, have already been reported. There are few data on thromboembolic complications associated with this device. We report the first case in the literature of Budd-Chiari Syndrome associated with the HeRO graft.

Stent exteriorization facilitates surgical repair for large-bore sheath complications.

Endovascular therapy for peripheral vascular and valvular disease has rapidly expanded and yet also produced new challenges for vascular access and closure. Current generation thoracic endograft (TEVAR) outer diameter sheath sizes range from 18 to 25 French, and the options for vascular access closure include off-label use of a closure device versus surgical cutdown. We describe a patient with a complex, post-dissection, descending aortic aneurysm who required TEVAR and had a vascular access complication repaired with stent exteriorization with open repair.

Vascular access in the newborn: a position paper of Neonatal European Vascular Access Teams (NEVAT).

Vascular access is a cornerstone of neonatal intensive care, yet current practice remains heterogeneous due to the lack of harmonized international guidelines. In this article, we present a position paper developed by a group of European experts - the Neonatal European Vascular Access Teams (NEVAT) - designed as a guide in planning, inserting, and maintaining vascular access in neonates. The position paper was developed using a not anonymous consensus method. Seven working groups prepared preliminary drafts on seven specific topics: peripheral venous devices, umbilical catheters, epicutaneo-ca

Retrieval strategy for ruptured balloon with circumferential tear during angioplasty for arteriovenous fistula in hemodialysis patients.

INTRODUCTION: Balloon angioplasty is a common endovascular procedure. The balloon for angioplasty sometimes ruptures (incidence, 3.6%-10%), and it is constructed such that it ruptures in a longitudinal direction and complications related to rupture are rare. However, on rare occasions, retrieval is challenging, especially in the case of ruptures with a circumferential tear. There is no established method for retrieval and careful retrieval is required due to the risk of embolization by the residual balloon fragment. TECHNIQUE: We describe two cases of balloon rupture in the transverse directi

Next Generation Temporary Mechanical Circulatory Support for Cardiogenic Shock.

Cardiogenic shock remains the leading cause of death in patients with acute myocardial infarction and severe acute heart failure. Temporary mechanical circulatory support devices have emerged as important adjuncts for restoring systemic perfusion, unloading the failing ventricle, and stabilizing patients for cardiogenic shock and during high-risk percutaneous coronary interventions. However, currently available devices are associated with important limitations, including vascular complications, hemolysis, and uncertain survival benefit. Technological innovation has led to the development of ne

Vascular ultrasound in patients with mechanical circulatory support devices: from pre-implant assessment to post-decannulation surveillance.

Mechanical circulatory support (MCS) devices, including extracorporeal membrane oxygenation (ECMO), left ventricular assist devices (LVADs), and intra-aortic balloon pumps (IABP), are increasingly used in patients with severe cardiac or cardiorespiratory failure. Despite major technical advances, vascular complications related to cannulation, device support, and decannulation remain frequent. Duplex ultrasound (DUS) is the primary vascular imaging modality because it is fast, can be performed at the patient’s bedside, is repeatable, and provides functional data via Doppler imaging. However, th

Outcome of access site in patients treated with platelet glycoprotein IIb/IIIa inhibitors in the era of closure devices.

The most consistent procedural predictor of vascular access site complications thus far has been the intensity and duration of anticoagulant therapy during and after percutaneous coronary interventions (PCI). Several devices have been developed to aid in the closure of the femoral arteriotomy. This report describes the clinical outcome of unsuccessful deployment of femoral closure devices in a cohort of 285 consecutive patients who underwent PCI and were treated with platelet glycoprotein (GP) IIb/IIIa inhibitors. Manual femoral artery compression was used in 123 patients, Perclose in 123 pati

Yoghurt (dahi): a probiotic and therapeutic view.

UNLABELLED: Deep vein thrombosis is an uncommon yet significant complication of percutaneous coronary interventions. Prolonged manual compression at the access site, a common practice for achieving hemostasis, can inadvertently exacerbate thrombosis risks, including venous stasis and endothelial injury. We present the case of an 84-year-old female with coronary artery disease who underwent staged percutaneous coronary intervention with stent placement in the left circumflex and left anterior descending arteries. After the procedure, manual pressure was applied to the femoral access site for ov

Predictors, Morbidity, and Costs Associated with Pneumothorax during Electronic Cardiac Device Implantation.

BACKGROUND: Pneumothorax (PTX) is a major cause of morbidity associated with cardiac implantable electronic devices (CIEDs). We sought to evaluate predictors of PTX at our centers during CIED implantations, including the venous access technique utilized, as well as to determine morbidity and costs associated with PTX. METHODS: We reviewed records of all patients undergoing cardiac device implant or revision with new venous access at our institutions between 2008 and 2014. Common demographic and procedure characteristics were collected including age, sex, body mass index (BMI), comorbidities,

Late onset iatrogenic limb ischaemia after deployment of an Angio-Seal vascular closure device.

It is common practice to deploy a vascular closure device for access site closure after percutaneous angiography or cardiovascular interventions for immediate haemostasis and to facilitate early discharge. We encountered two octogenarian women who underwent and had subsequent vascular access site closure with Angio-Seal (St Jude) and who later presented with limb ischaemia needing surgical revascularisation. Our patients had undergone uneventful deployment of the Angio-Seal vascular closure device (VCD) at the right common femoral artery (CFA) access site with successful haemostasis. About 3 w

Acute lower limb ischemia caused by repeat deployment of vascular closure device.

Vascular closure devices are used to facilitate faster hemostasis and earlier ambulation, improve patient comfort, and reduce length of hospital stay after percutaneous endovascular procedures. However, their use may rarely be associated with limb ischemia due to endothelial damage and arterial thrombosis. This report illustrates the case of a patient who experienced acute lower limb ischemia due to superficial femoral artery occlusion caused by repeat closure with Angio-Seal vascular closure device within 30 days in a small-caliber superficial femoral artery. The patient was surgically treate

Rates and impact of vascular complications in mechanical circulatory support.

BACKGROUND: Mechanical circulatory support (MCS) devices are increasingly used for hemodynamic support in cardiogenic shock or high-risk percutaneous coronary interventions. Vascular complications remain a major source of morbidity and mortality despite technological advances with percutaneous techniques. Little is known about the rates and predictors of vascular complications with large-bore access MCS in the contemporary era. METHODS: The study cohort was derived from National Inpatient Sample using data from 2015 to 2019 for cardiac hospitalizations with the use of: intra-aortic balloon pu
