Left Ventricular Outflow Tract Obstruction After Transcatheter Mitral Valve-in-Ring Implantation: A Word of Caution.

Left ventricular outflow tract obstruction is a well-known, albeit rare, adverse event that can occur after mitral valve replacement. With the current increase in transcatheter valve interventions, new and unique adverse events may be expected to occur. We present a case of severe left ventricular outflow tract obstruction that developed after transcatheter mitral valve implantation.

Accessory mitral valve without subaortic obstruction of left ventricular outflow tract in a middle-aged male.

Accessory mitral valve (AMV) is a rare congenital abnormality with a usually early-age clinical onset, being potentially a cause of subvalvular obstruction of the left ventricular outflow tract. This report describes the case of a 60-year-old patient presented with palpitations and chest pain. Primary evaluation revealed a ventricular tachycardia episode while transthoracic echocardiography showed an intracardiac additional structure at the level of the left ventricular outflow tract. After transoesophageal echocardiography and paraclinical investigations this structure was proven to be an AMV

Intermediate results following complex biventricular repair of left ventricular outflow tract obstruction in neonates and infants.

OBJECTIVES: Although the majority of infants with severe left ventricular outflow tract obstruction (LVOTO) can be managed with balloon or surgical aortic valvotomy, a more complex biventricular repair may be required in a subset of infants with multi-level obstruction, failure of or complication to prior intervention. In the presence of normal left ventricle size and inflow, the Ross procedure is applied in patients with/without ventricular septal defect (VSD), while the Yasui procedure is applied only in those with VSD. We report mid-term outcomes in a single institution. METHODS: Thirty-fi

[Obstruction of the left ventricular outflow tract caused by bicuspid aortic valve and discrete subaortic membrane].

Bicuspid aortic valve is the most common congenital cardiac anomaly and it may often coexist with other congenital cardiac anomalies. Its coexistence with discrete subaortic membrane, causing obstruction of the left ventricular outflow tract is very rare. A 21-year-old male patient presented with complaints of exertional dyspnea and dizziness. On transthoracic echocardiography, the parasternal short-axis view showed a bicuspid aortic valve, and parasternal long-axis color Doppler view showed a mosaic pattern in the subaortic region. A more careful examination of the parasternal long-axis views

[Dynamic obstruction to left ventricular outflow during dobutamine stress echocardiography: the probable mechanisms and clinical implications].

We observed the development of left ventricular outflow tract dynamic obstruction in some patients during dobutamine stress echocardiography. The purpose of this study was to identify the possible mechanisms and to consider the clinical implications. From 11/04/94 to 01/09/97 we studied 547 patients; 42 patients developed dynamic obstruction, defined as a late peak Doppler velocity profile that exceeded baseline outflow velocity by at least 1 m/s. The encountered mechanisms were: increased myocardial contractility; systolic anterior motion of the mitral valve; decreased venous return to the le

Relief of left ventricular outflow tract obstruction in infants and small children with valved extracardiac conduits.

Six infants and small children underwent placement of a porcine valved apical-aortic conduit for left ventricular outflow tract obstruction. Five survived operation and continue to do well without evidence of marked narrowing or valve regurgitation in the conduit. The operative technique is described.

Resolution of left ventricular and asymmetric septal hypertrophy after resection of left ventricular outflow obstruction in a patient with troponin-positive hypertrophic obstructive cardiomyopathy: a case report.

We report the case of a young woman with a troponin mutation of C to T nucleotide substitution in exon 17 of troponin 2 (TNNT2; c.868C.T; p.Arg288Cys) leading to hypertrophic obstructive cardiomyopathy. Following surgical resection of the outflow obstruction, she had near-complete resolution of her asymmetric left ventricular hypertrophy, such that cardiomyopathy could no longer be diagnosed on echocardiographic grounds. We believe that this unusual case shows important aspects relating to the interplay between genetic and environmental mechanisms and the overlap in the phenotypic spectrum bet

Caseous Mitral Annular Calcification Causing Left Ventricular Outflow Tract Obstruction and Embolic Stroke in Hypertrophic Cardiomyopathy.

BACKGROUND: Caseous mitral annular calcification is a variant of annular calcification that may alter mitral valve geometry and represent a potential embolic source. CASE SUMMARY: A 50-year-old woman with exertional dyspnea was diagnosed with hypertrophic cardiomyopathy with left ventricular outflow tract obstruction. Transesophageal echocardiography revealed caseous calcifications of the posterior mitral annulus, causing leaflet displacement, promoting systolic anterior motion. Despite optimized medical therapy, she developed ischemic stroke, with imaging suggesting liquefactive degeneration

Direct Visualization of External Outflow Graft Obstruction in Durable Left Ventricular Assist Device Using Transthoracic Echocardiography.

External outflow graft obstruction (OGO) in durable left ventricular assist devices (LVADs) is a rare but critical complication. Detecting external OGO can be challenging because of its nonspecific symptoms and a wide variety of differential diagnoses. We report the case of a 45 year old man with LVAD admitted with complaints of recurrent syncope and frequent low-flow alarms. Laboratory tests and chest radiography revealed no specific findings. Transthoracic echocardiography via the left subcostal approach revealed an outflow graft compressed by a low-echo area within a bend relief. This findi

How Would We Treat Our Own Left Ventricular Outflow Tract Obstruction in Transcatheter Mitral Valve Replacement?

This article aims to provide a comprehensive review of the current knowledge on the prevention and treatment of left ventricular outflow tract obstruction (LVOTO) associated with transcatheter mitral valve replacement-a novel and evolving treatment alternative for mitral regurgitation-encompassing both surgical and pharmacological interventions. LVOTO is a potentially catastrophic complication of transcatheter mitral valve replacement. Therefore, identifying patients at high risk for LVOTO and implementing a carefully tailored medical and surgical strategy are essential for optimizing perioper

A surgeon's toolkit for mitral valve-induced left ventricular outflow tract obstruction with minimal septal hypertrophy.

OBJECTIVE: Even in the absence of significant left ventricular hypertrophy, abnormalities of the mitral valve leaflets and subvalvular apparatus can contribute to left ventricular outflow tract obstruction. We present a toolkit of techniques for mitral valve repair and replacement in this setting. METHODS: Our approach to mitral valve repair for left ventricular outflow tract obstruction follows 2 key principles: modification of the abnormal effects of flow vortices that displace the leaflets into the left ventricular outflow tract and restoration of a more posterior leaflet coaptation zone.

Dynamic left ventricular outflow tract obstruction induced by intra-aortic balloon pump in patient with angioedema.

BACKGROUND: Intra-aortic balloon pump is used for temporary mechanical support of failing left ventricle. It works by reducing the arterial afterload during ventricular systole to reduce myocardial work and increasing diastolic proximal aortic pressure to improve coronary perfusion. Rarely, intra-aortic balloon pump (IABP) can become the cause of severe haemodynamic compromise, causing dynamic left ventricular outflow tract obstruction. CASE PRESENTATION: An 88-yo man presented with angiotensin converting enzyme inhibitor (ACEI) - induced angioedema. He received steroids and adrenaline, but p

[Redo surgical treatment of a patient with hypertrophic cardiomyopathy and recurrent left ventricular outflow tract obstruction].

Septal myectomy is the gold standard for patients with obstructive hypertrophic cardiomyopathy. Nevertheless, some patients develop postoperative recurrence of left ventricular outflow tract obstruction and clinical symptoms of heart failure. Such ones require redo surgery if optimal medical therapy is ineffective.

Acute Left Ventricular Ballooning: Tools to Differentiate Hypertrophic Cardiomyopathy with Outflow Obstruction from Neurohumoral Takotsubo Syndrome.

Despite considerable interest in the syndrome of acute left ventricular (LV) ballooning, its pathophysiology has remained ill-defined. In this review, we explore observational data describing two etiologies of acute LV ballooning: neurohumoral classic Takotsubo Syndrome (TTS), and acute severe unrelenting left ventricular outflow tract (LVOT) obstruction in patients with obstructive hypertrophic cardiomyopathy (HCM). We describe the clinical presentation and varying pathophysiology of these presentations, explore how echocardiography and cardiac catheterization may help differentiate between t

Tissue tracking of segmental strain as a predictor of provoked dynamic left ventricular outflow tract obstruction.

BACKGROUND: Left ventricular outflow tract obstruction (LVOTO) is commonly observed in patients with hypertrophic cardiomyopathy (HCM) or left ventricular hypertrophy (LVH). Some patients develop LVOTO provoked by physical exertion, and hence termed dynamic LVOTO (DLVOTO). However, its precise prevalence and mechanism are still unclear. AIM: Two-dimensional speckle tracking echocardiography (2D STE) seems to be helpful for the detection of early LV structural abnormalities. This study aimed to examine the possible role of segmental as well as global longitudinal strain in identifying DLVOTO n

Usefulness of Alcohol Septal Ablation in the Left Ventricular Outflow Tract Obstruction in Fabry Disease Cardiomyopathy.

Fabry disease (FD) is an X-linked linked genetic disorder caused by α-galactosidase A deficiency. The typical clinical manifestation is left ventricular hypertrophy, often mimicking hypertrophic cardiomyopathy (HC). In contrast to sarcomeric HC, left ventricular outflow tract obstruction (LVOTO) is less frequent. We describe 6 male patients with genetically confirmed FD and symptomatic LVOTO. All of them underwent a transcatheter alcohol septal ablation with an immediate effect on the obstruction in all cases and without any serious complications. The median LVOT maximal pressure gradient was
