Operative versus nonoperative treatment of acute undisplaced and minimally displaced scaphoid waist fractures--a systematic review.

INTRODUCTION: Undisplaced and minimally displaced scaphoid waist fractures can be managed either operatively (percutaneous or ORIF) or non-operatively with both methods obtaining high rates of fracture union and subsequent return of function. The aim of this systematic review is to identify and evaluate the best available evidence to determine whether they should be managed operatively or non-operatively. MATERIALS AND METHODS: A Medline and journal hand search was performed with strict eligibility criteria to obtain the highest quality evidence from meta-analyses, randomised controlled trial

[Focused High-Energy Extracorporeal Shock Wave Therapy (ESWT) for Bone healing Disorders of the Forearm and the Hand].

BACKGROUND: ESWT is a non-surgical treatment option but can also be used in addition to surgical treatment (stabilisation, freshening, defect filling, removal of discomforting osteosynthesis material) for the treatment of delayed bone healing (DBH) and non-union (NU). Its value as well as influencing factors on the upper extremity have not been adequately quantified so far. PATIENTS AND METHODS: Sixty cases were retrospectively studied after application of focused high-energy ESWT with regard to healing rate and consolidation time. The influence of age, location, time of treatment and treatme

Functional outcome of open reduction of chronic perilunate injuries.

PURPOSE: Perilunate injuries are complex and occasionally go unrecognized acutely. Open reduction and internal fixation is a valid treatment option for these injuries. The purpose of this study was to evaluate the functional outcome of treating chronic perilunate injuries with open reduction and internal fixation. METHODS: Between 1998 and 2007, we treated 24 patients for chronic perilunate injuries. We excluded 5 patients from this study because they underwent proximal row carpectomy or limited wrist arthrodesis. We treated the remaining 19 patients with open reduction and internal fixation.

Results after radioscapholunate arthrodesis with or without resection of the distal scaphoid pole.

PURPOSE: To evaluate the differences between radioscapholunate (RSL) arthrodesis alone versus RSL arthrodesis with additional distal scaphoidectomy. METHODS: We retrospectively evaluated 61 patients who were treated with RSL arthrodesis for painful posttraumatic osteoarthritis. Thirty patients had an RSL arthrodesis with additional resection of the distal scaphoid pole (group A), and 31 had RSL arthrodesis alone (group B). Six patients in group A and 8 in group B had the RSL arthrodesis converted to a complete wrist arthrodesis during follow-up. Those patients were excluded from the survey. O

Influence of screw design, sex, and approach in scaphoid fracture fixation.

BACKGROUND: Screw fixation of scaphoid fractures has gained popularity. A long central screw has been shown to be biomechanically advantageous. QUESTIONS/PURPOSES: We compared the ability of different screw designs to obtain this goal and determined the influence of sex and approach on screw length. METHODS: We performed all measurements on three-dimensional reconstructions of 20 CT scans of normal scaphoids (10 men and 10 women) with the use of software. The three-dimensional computer models were analyzed, the central axis was defined, and the screws were placed along this axis. We compared

Percutaneous transtrapezial fixation of acute scaphoid fractures.

Percutaneous screw fixation of undisplaced fractures of the scaphoid waist has gained popularity but remains technically demanding. This study describes a transtrapezial modification of the volar percutaneous technique and reports the results in 41 patients. The patients were evaluated at a mean of 36 months (range 14-68 months) after surgery. All fractures healed within 10 weeks (mean 6.4 weeks). Functional ranges of wrist motion and grip strength were achieved in all patients. Radiographs showed accurate central placement of the screw in all patients and no degenerative changes were seen at

Distal radius morphometry of volar curvature along with scaphoid and lunate facet inclinations and ulnar variance in the Anatolian population.

This study evaluates the morphometric characteristics of the volar distal radius, specifically volar curvature, scaphoid and lunate facet inclinations, and ulnar variance, in the Anatolian population. By identifying these population specific morphological variations, the study seeks to support improved anatomical compatibility of volar implants and enhance surgical planning for distal radius fracture fixation. Morphometric measurements were analyzed retrospectively on three-dimensional computed tomography images of 103 intact distal radii. Standardized sagittal sections were created through th

Scapholunate Ligament Reconstruction with Suture Augmentation: a Biomechanical Analysis.

PURPOSE: To evaluate the biomechanical performance of the modified Brunelli scapholunate interosseous ligament (SLIL) reconstruction with and without suture augmentation, using a cadaveric model under cyclic loading conditions. METHODS: Eight matched pairs of fresh-frozen cadaveric wrists underwent sequential sectioning of the SLIL, dorsal intercarpal ligament, long radiolunate ligament, and scaphotrapeziotrapezoid ligament to induce dorsal intercalated segment instability (DISI). Each wrist was reconstructed using the modified Brunelli technique; one in each pair received augmentation with 2

Cost-effectiveness of early surgical fixation versus cast immobilization for adults with a scaphoid waist fracture: five-year follow-up of the Scaphoid Waist Internal Fixation for Fractures Trial.

AIMS: The conclusion of the one-year analysis of the Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) was that initial cast immobilization with surgical fixation for nonunion was the optimal treatment for patients with a fracture of the waist of the scaphoid. However, there remained significant uncertainty about the medium-term outcomes and how these could influence the patient's health-related quality of life (HRQoL), care requirements, and costs. The aim of this study was to explore how additional data from the five-year follow-up influenced the cost-effectiveness of the treatme

Simple Excision of Proximal Scaphoid Nonunion: A Viable Option When the Scapholunate Ligament Is Intact.

PURPOSE: Because of the biological, anatomical, and biomechanical characteristics of the scaphoid, treating some proximal pole nonunions remains challenging. The location of the nonunion (proximal or distal to the dorsal apex) may influence its natural history. Based on this concept, patients with a proximal pole nonunion located proximal to the dorsal apex and maintaining an intact insertion of the dorsal scapholunate ligament in the distal fragment of the scaphoid may benefit from simple excision of the proximal pole. This study evaluated the clinical and functional outcomes of this treatmen

The epidemiology of scaphoid fractures and non-unions in Switzerland: a nationwide analysis of the socioeconomic impact.

Scaphoid fractures are the second most common wrist injury after distal radius fractures and primarily affect young individuals in their most productive working years. Some scaphoid fractures fail to heal, potentially resulting in chronic pain, functional impairment, and long-term osteoarthritis in the wrist, which may require salvage procedures and can have a significant impact on work capacity. Considerable debate exists around the biological and injury-related risk factors for scaphoid non-unions, as well as the criteria for surgical intervention. However, epidemiological data are still lim

Proximal Pole Scaphoid Fracture Nonunion Treated With Ipsilateral Hamate Transfer: A Case Series.

PURPOSE: The scaphoid's retrograde blood flow renders it vulnerable to fracture nonunion proximally. As proximal fragmentation occurs, reconstructive options become challenging with little consensus regarding treatment. Proposed surgical techniques include vascularized or nonvascularized grafts and fragment excision with scapholunate (SL) ligament advancement. We hypothesized that ipsilateral proximal hamate with volar capitohamate ligament autograft would provide a viable treatment for proximal pole scaphoid nonunion in a sample drawn from a military population. METHODS: Eleven patients with
