Extrathoracic approach in surgical treatment of subclavian steal.

Between 1979 and 1985 11 selected patients underwent axilloaxillary and carotid-subclavian bypass grafting [dacron or Gore-Tex (USA), 8 mm inner diameter]. Nine patients had lesions in the left subclavian artery and two patients presented with innominate artery obstruction with subclavian-carotid recovery phenomenon. All patients were symptomatic before surgery, and all were relieved of both cerebrovascular and upper extremity ischemic symptoms following surgery. Early graft patency has been excellent. There has been no mortality or serious postoperative complications. Two patients developed l

[Limits of preventive extra-intracranial anastomoses. A propos of a meningioma of the clinoid process with stenosis of the internal carotid artery].

In the case reported here a voluminous meningioma attached to the anterior clinoid process had invaded the cavernous sinus, involving and constricting the internal carotid artery and its terminal branches. To prevent occlusion of the carotid-sylvian system during surgery, an extra-intracranial anastomosis was carried out and its permeability was controlled and confirmed. However, excision of the meningioma, which entailed resection of the carotid segment, resulted in hemiplegia. The authors emphasize the importance of haemodynamic and anatomical factors in such cases.

Cerebrospinal fluid hydromechanics in adult patients with benign noncommunicating hydrocephalus: one-hour test shunting and balanced cerebrospinal fluid infusion test to select patients for intracranial bypass operation.

Cerebrospinal fluid (CSF) pressure and absorption were determined in 12 patients with benign noncommunicating hydrocephalus. An extracorporeal communication of plastic tubing between the ventricular system and the lumbar subarachnoid space was established to imitate the hydromechanical condition after an intracranial bypass operation. This extracorporeal shunt, which in effect was an externalized Torkildsen shunt, served first as a test shunt for 1 hour and then as a route for the constant rate CSF infusion test. The total CSF absorptive capacity was thus determined under a condition of balanc

Efficacy of STA-MCA bypass surgery in moyamoya angiopathy: long-term follow-up of the Caucasian Krupp Hospital cohort with 81 procedures.

BACKGROUND: Despite the consensus on the efficacy of revascularizing surgery in moyamoya angiopathy (MA) in Asia, the indication in Caucasian moyamoya patients is controversially discussed. OBJECTIVE: The efficacy of revascularizing surgery in adult European patients with MA should be clarified. METHODS: This study retrospectively analyzed the rate of further strokes and hemorrhages as well as MRI and Duplex ultrasound features during long-term follow up after STA-MCA bypass. RESULTS: Eighty-one STA-MCA bypass procedures in 54 patients with MA operated in one single German institution were

Extra-intracranial standard bypass in the elderly: perioperative risk, bypass patency and outcome.

BACKGROUND: Patients with chronic atherosclerotic vessel occlusion and cerebrovascular hemodynamic insufficiency may benefit from extra-intracranial (EC-IC) bypass surgery. Due to demographic changes, an increasing number of elderly patients presents with cerebrovascular hemodynamic insufficiency. So far, little data for EC-IC bypass surgery in elderly patients suffering occlusive cerebrovascular disease are available. We therefore designed a retrospective study to address the question whether EC-IC bypass is a safe and efficient treatment in a patient cohort ≥70 years. METHODS: 50 patients u

Xenon/CT CBF measurements as valuable diagnostic tool in a case of bilateral occlusive cerebrovascular disease associated with intracranial aneurysm.

A patient with the coincidental occurrence of a right internal carotid artery (ICA) stenosis and asymptomatic ipsilateral infraclinoidal ICA aneurysm is presented. CBF measurements including testing of the cerebrovascular reserve capacity (CRC) demonstrated a compromise of cerebral blood flow (CBF) within the anterior circulation of the affected side. We treated the patient in one operation by aneurysm clipping and subsequent carotid endarterectomy (CEA). 10 months later she presented with an occlusion of the contralateral ICA. CBF measurements showed sufficient resting flow and CRC on both si

Perioperative ASL-derived CBF changes are associated with complications after Moyamoya bypass surgery.

BACKGROUND: Postoperative hyperperfusion and insufficient perfusion augmentation are important contributors to morbidity after revascularization surgery for Moyamoya disease (MMD). Arterial spin labeling (ASL) enables noninvasive assessment of cerebral blood flow (CBF), but the relationship between perioperative ASL-derived CBF changes and specific postoperative complications after direct bypass remains incompletely defined. This study evaluated whether perioperative ASL-derived CBF changes were associated with postoperative complications after direct bypass surgery in patients with MMD. METH

Computed tomography perfusion imaging in the selection of acute stroke patients to undergo emergent carotid endarterectomy.

Severe acute stroke patients with critical carotid stenosis or occlusion without intracranial thrombus typically do not undergo emergent carotid thromboendarterectomy (CEA) because of the risk of reperfusion-related intracranial hemorrhage. Past studies have not consistently demonstrated benefit of early operative intervention. Cerebral computed tomography (CT), cervical and cerebral CT angiography (CTA), and cerebral CT perfusion (CTP) imaging may identify a subset of acute stroke patients without intracranial thrombus who may benefit from emergent CEA. Acute stroke patients underwent unenhan

Common Carotid-Greater Saphenous-M2 Bypass for Myoepithelial Carcinoma Encasing the Carotid Artery.

Revascularization of the anterior circulation using a high-capacitance graft is sometimes necessary for treatment of cervical tumors encasing the internal carotid artery (ICA). In this surgical video, we aim to demonstrate the technical nuances of high-flow extra-to-intracranial bypass using a saphenous vein graft. The patient is a 23-year-old female who presented with a 4-month history of an enlarging left-sided neck mass, dysphagia, and 25-lb weight loss. Computed tomography and magnetic resonance imaging demonstrated an enhancing lesion encasing the cervical ICA. The patient underwent an op

Traumatic Pseudoaneurysm of the Distal Anterior Cerebral Artery Following Penetrating Brain Injury Caused by a Crossbow Bolt: A Case Report.

Traumatic intracranial aneurysms are one possible complication after penetrating brain injury. A 25-year-old man with a history of major depression presented with a crossbow bolt penetrating the head. On arrival, Glasgow Coma Scale score was E4V5M6, with no apparent neurological deficit. Computed tomography (CT) of the head showed the crossbow bolt passing near the corpus callosum, with surrounding contusion. Three-dimensional rotational angiography showed no anterior cerebral artery injuries. The crossbow bolt was removed after bifrontal craniotomy, with no postoperative infection. Postoperat

The Susceptibility Pathogenesis of Moyamoya Disease.

Moyamoya disease (MMD) is a cerebrovascular disease characterized by progressive stenosis of the intracranial internal carotid arteries and their proximal branches. Epidemiologically, MMD is more prevalent in East Asia than any other region worldwide, and has been estimated at 0.94 per 100,000 in the Japanese and 0.43 per 100,000 in the Chinese population. The etiology of this rare disease, however, remains unknown. Regarding biomarkers, MMD is characterized by an increased expression of angiogenic factors and proinflammatory molecules such as vascular endothelial growth factors and matrix met

Moyamoya disease and syndromes: from genetics to clinical management.

Moyamoya angiopathy is characterized by a progressive stenosis of the terminal portion of the internal carotid arteries and the development of a network of abnormal collateral vessels. This chronic cerebral angiopathy is observed in children and adults. It mainly leads to brain ischemic events in children, and to ischemic and hemorrhagic events in adults. This is a rare condition, with a marked prevalence gradient between Asian countries and Western countries. Two main nosological entities are identified. On the one hand, moyamoya disease corresponds to isolated moyamoya angiopathy, defined as

Trends in the management of adult moyamoya disease in the United States: results of a nationwide survey.

BACKGROUND: Moyamoya disease is a rare, progressive, steno-occlusive disease of the distal branches of the carotid arteries associated with the development of a profuse basal collateral vascular network. In the United States, the disease occurs sporadically and literature on the topic in limited. With data lacking relative to moyamoya epidemiology and management, we report our survey results among a sample of neurovascular surgeons. METHODS: A 16-question survey was sent as a hyperlink by electronic mail to 46 vascular neurosurgery centers and physicians who have published on this topic. Data

Clinical characteristics and outcomes of tandem lesions involving internal carotid artery occlusion and distal middle cerebral artery occlusion.

BACKGROUND: Tandem lesions involving internal carotid artery occlusion (ICAO) and distal middle cerebral artery occlusion (MCAO) are rare, and evidence regarding their clinical characteristics, management strategies, and outcomes remains limited. We aimed to describe real-world treatments and clinical outcomes in patients with this condition at a single comprehensive stroke center. METHODS: We reviewed consecutive patients included in our stroke registry between 2009 and 2022 and identified those with ICAO and distal MCAO (M3 or M4 segments). We described the use of thrombolysis, mechanical t

Percutaneous transluminal angioplasty for vertebral artery stenosis due to primary central nervous system vasculitis: A case report.

We report a rare case of high-grade vertebral artery stenosis caused by biopsy-proven primary central nervous system vasculitis, successfully treated with percutaneous transluminal angioplasty. Primary central nervous system vasculitis with large-vessel involvement is uncommon, and reports of effective endovascular intervention remain limited. This case is notable for 3 reasons: (1) the diagnosis was histologically confirmed, (2) standard immunosuppressive therapy failed to halt disease progression, and (3) endovascular revascularization resulted in durable recovery without complications.
