Management of Mandibular Condylar Sagittal Fracture Accompanied by Lateral Condylar Crest Defect.

BACKGROUND: The sagittal fracture of the mandibular condyle can be fixed with absorbable long screws. Absorbable long screws are generally inserted from the lateral crest of the condyle and are as close as possible to the medial pole of the condyle to obtain sufficient retention force. However, in clinical practice, patients with locally comminuted condylar fractures and partial defects in the lateral crest are often encountered. We validated the use of absorbable plates and long screws to fix mandibular condylar fractures with lateral crest defects, and postoperative follow-up showed good res

The application of inverted lateral clavicle locking compression plate for medial clavicle fractures: biomechanical study and clinical implementation.

BACKGROUND: There has been no absolute consensus on the optimal implant for fixation of medial clavicle fracture. The purpose of the present study was to test the biomechanical efficacy of inverted lateral clavicle locking compression plate (LCP-LC) in fixation of this specific fracture location by finite element analysis. METHODS: Transverse and comminuted medial clavicle fracture simulated models stabilized by three different devices including inverted LCP-LC, superior clavicle LCP (LCP-SC), and dual reconstruction (LCP-RP) with LCP-SC were investigated biomechanical performance under three

Successful Plate Fixation with Long Intramedullary Fibula Bone Graft for Periprosthetic Femur Fracture: A Case Report.

Background and objectives: Treatment of a displaced or comminuted periprosthetic distal femur fracture is challenging, especially in patients with osteoporosis. In this case report, we shared our successful surgical experience of using a long intramedullary fibula bone graft in a plate fixation surgery for a periprosthetic distal femur fracture in an extremely elderly patient with osteoporosis. Case report: A 95-year-old woman with severe osteoporosis (bone mineral density level: -3.0) presented with right knee pain and deformity after a fall, and a right periprosthetic distal femur fracture w

Fragment-specific fixation of posterior wall acetabular fractures.

PURPOSE: Fractures of the posterior wall (PW) of the acetabulum have a wide variety of patterns; treating them as a single entity using the standard ilio-ischial plate would be inappropriate. We are presenting our experience with a fragment-specific fixation technique in which each PW fragment is reduced and fixed with separate buttress/anti-glide plate(s) in a tailored fashion, abandoning the use of the ilio-ischial plate. PATIENTS AND METHODS: Fragment-specific fixation was applied to 46 patients with PW fractures (33 simple and 13 associated fracture types) with a mean follow-up of 34.9 ±

Dynamic limited axial compression yields favorable functional outcomes in the fixation of Pauwels type-3 femoral neck fractures: A retrospective cohort study.

PURPOSE: Pauwels type-3 femoral neck fractures are challenging injuries to manage with high rates of complications after internal fixation and no consensus has been reached regarding the optimal fixation construct. The current study aims to evaluate the effect of dynamic limited axial compression in parallel screws combined with medial buttress plate (SMBP) or lateral compression plate (LCP) fixation of Pauwels type-3 femoral neck fractures. METHODS: We performed a retrospective analysis of 51 cases of Pauwels type-3 femoral neck fractures who were fixed by SMBP or LCP. Specifically, the scre

Surgery of patellar fractures using a medial parapatellar approach.

PURPOSE: The general surgical approach for patellar fractures has a significant weakness, in that the articular facet is invisible because the fixation is performed using radiation amplifiers after exposing the fracture site through vertical or transverse dissection on the anterior patella. We report excellent outcomes in the surgical treatment of patellar fractures, using a medial parapatellar approach. METHODS: This study evaluated 20 patients who underwent surgery between August 2008 and April 2014 to correct patellar fractures with comminution or with displacement of the articular facet o

Comparison of two biological internal fixation techniques in the treatment of adult femur shaft fractures (plate-screws and locked intramedullary nail).

BACKGROUND: We aimed to compare two (plate-screws and locked intramedullary nail) biological internal fixation techniques in the treatment of adult femur shaft fractures. METHODS: Group I included 28 patients operated with biological internal fixation with plate and screws. Group II consisted of 31 patients operated with locked intramedullary nailing technique. During the follow-up period in Group I, 1 patient died and 3 patients dropped out of the study, so the results of 24 patients were evaluated. During the follow-up period in Group II, 2 patients died and 3 patients dropped out of the st

[Acute carpal tunnel syndrome after distal radius fractures--long term results of surgical treatment with decompression and external fixator application].

Acute Carpal Tunnel Syndrome (ACTS) is a relatively rare sequel of distal fractures of the radius. This paper is an analysis of the causes of ACTS and of long-term results of treatment by surgical decompression combined with use of external fixators. Out of 128 patients with distal radius fractures treated in our department 11 cases required surgical treatment because of ACTS. Fractures were classified according to the AO classification as C2 and C3. CTSA was diagnosed basing on typical clinical signs: pain numbness over the radial three and a half fingers. All patients were treated surgically

Surgical Technique - The "Derriere View" for Evaluation of Guidewire Position in Cephalomedullary Nailing of Intertrochanteric Femur Fractures.

Intertrochanteric (IT) hip fractures are common in the elderly and are frequently treated with cephalomedullary nailing. Despite advances in implant design, fixation failure, such as cut-out or cut-through, remains a concern, often related to suboptimal implant positioning and limited intraoperative visualization of the femoral neck cortex. This technical report describes the Derriere view, an additional intraoperative fluoroscopic projection applied during cephalomedullary nailing of intertrochanteric fractures with the targeting jig and instrumentation in situ. The view is obtained by rotati

Patellar fractures: To plate them all? Results of a new treatment protocol.

INTRODUCTION: In recent years, anatomically contoured locking plates have been introduced as an alternative fixation method for patellar fractures. Biomechanical studies have demonstrated superior mechanical stability and clinical studies further suggest that plating reduces complication and revision rates, especially in comminuted fracture patterns. However, there is uncertainty regarding the role of plating in less complex fracture types. Following an institutional shift to routine plating of all surgically treated patellar fractures, this study compares outcomes and costs of plate versus no

A single centre experience of pre-contoured clavicle plates by an anterior approach.

AIM: In recent years there has been increasing evidence that comminuted and shortened clavicle fractures should be treated surgically. The aim of this study was to assess the outcome of patients who had their clavicle fracture fixed using a pre-contoured, clavicle specific plate from an anterior approach. METHOD: A retrospective analysis was carried out of all patients treated with pre-contoured clavicle plates from an anterior approach between May 2014 and June 2016.Medical records were analysed, and a phone survey was carried out to assess patient function and note any complications. Interlocking nail treatment of long-bone fractures in cats: 33 cases (1995-2004).

A retrospective study between 1995 and 2004 of fracture repair in cats (n = 46) using interlocking nails (ILN) was made. Only the cases in which clinical and radiographic examination of bone healing were available are included. Five humeral and twenty eight femoral fractures were reviewed. Multiple-hole interlocking nails (MH-ILN) were used in 22 fractures, with the four-hole model (FH-ILN) in the remaining cases. In three of the cases, the FH-ILN was transformed to a three-hole model by cutting the nail between the two most distal holes. The average length of humeral ILN was 78.8 mm, and the

Biomechanical analysis of different osteosynthesis configurations in the pin and plate fixation method for distal humerus fractures.

Distal humerus fractures commonly occur in adults with low bone mineral density causing major technical challenges for orthopedic surgeons. Persian fixation method was introduced as a novel technique to stabilize small fragments in comminuted distal humerus fractures using a set of K-wires and a reconstruction plate. The present study aims to measure this technique's stiffness and stability of this technique and analyze the effect of influential parameters with numerical simulation and biomechanical testing on a cadaveric specimen. Validation of the finite element (FE) model was conducted base

Open Dislocation of Ankle without Fracture Treated with an External Fixator.

Ankle dislocations are orthopedic emergencies that require immediate treatment to avoid neurovascular impairment. They are usually accompanied by one or more comminuted fractures of the ankle mortis. In rare circumstances, such as high-energy trauma, the ankle dislocations may not be accompanied by concomitant malleolar fractures and, thus, are named "pure ankle dislocations". We presented a very rare and interesting case of an open medial dislocation of the ankle without associated fracture in an 18-year-old man with no known predisposing risk factors. The patient was admitted to the emergenc

Osteosynthesis versus arthroplasty in the management of radial head fractures: a retrospective cohort study.

BACKGROUND: Radial head fractures are among the most common elbow injuries in adults. The choice between osteosynthesis and radial head arthroplasty (RHA) remains controversial, particularly in comminuted or unstable fracture patterns. This study aims to compare functional and radiographic outcomes, complication rates, and patient satisfaction following osteosynthesis or arthroplasty in the surgical management of radial head fractures. METHODS: A retrospective cohort of 35 patients treated surgically for radial head fractures between 2018 and 2022 was analyzed. Fractures were classified using
