Closing Radial Wedge Osteotomy for Preiser Disease: Clinical Outcomes.

PURPOSE: To determine clinical outcomes of Preiser disease treated with closing radial wedge osteotomy (CRWO). METHODS: Seven patients with Preiser disease underwent CRWO. Two wrists had stage 2 disease, 3 had stage 3, 2 had stage 4 (Herbert-Lanzetta classification). Magnetic resonance imaging (MRI) showed that 4 wrists had stage 1 and 3 had stage 2 indicating complete and incomplete necrosis, respectively (Kalainov criteria), before surgery. Two wrists had concomitant Kienböck disease. The range of motion (ROM) of the wrists was restricted owing to pain, and median Modified Mayo Wrist Score

Osteoarthritis of the scaphoidtrapezium joint: an early sign of calcium pyrophosphate dihydrate disease.

The aim of this study was to determine the value of scaphoidtrapezium osteoarthritis (ST osteoarthritis) as an early sign of calcium pyrophosphate dihydrate disease (CPDD) in a cohort of patients undergoing surgery for osteoarthritis of the first carpometacarpal joint. We examined whether patients with cartilage calcification of the wrist at the time of operation had ST osteoarthritis, indicating CPDD at an earlier time (retrospective study), and whether patients with ST osteoarthritis but without cartilage calcification at the time of surgery develop radiological or clinical signs of CPDD at

Reconstruction of longitudinal stability of the forearm after disruption of interosseous ligament and radial head excision (Essex-Lopresti lesion).

We have presented a technique for reconstruction of the central band of the interosseous membrane in conjunction with surgical repair of the distal radial joint and radial head prosthesis. With this technique, we address all three anatomic structures that provide longitudinal stability of the forearm, specifically, (1) radial head replacement, (2) interosseous membrane reconstitution, and (3) TFCC repair.

Using a TightRope® to treat a complex fracture of the trapezium.

We present the case of a 23-year-old man with a combined scaphoid fracture and comminuted trapezium fracture, treated surgically with percutaneous fixation of the scaphoid fracture and concomitant Arthrex Mini TightRope(®) stabilisation of base of thumb metacarpal to base of index finger metacarpal. The patient made a good functional recovery, returning to usual activities within six weeks. We suggest that this technique could be used to treat complex trapezium fractures that cannot be reconstructed with surgery.

Role of SPECT/CT compared with MRI in the diagnosis and management of patients with wrist trauma occult fractures.

BACKGROUND: The diagnosis of wrist fractures, especially scaphoid fractures, remains a challenge because of nonunion risk. Currently, new hybrid technologies are emerging such as SPECT/CT systems, which combine functional and anatomical data sets. So, we wanted to evaluate the utility of SPECT/CT in the management of occult carpal fractures. METHODS: In this study, all patients addressed at the orthopedic department at Brest University Hospital for wrist pain after trauma with initial normal plain radiographs were prospectively included. Patients with normal radiographs but with signs strongl

Preliminary lunate reduction and pinning facilitates restoration of carpal height when treating perilunate dislocation, scaphoid fracture and nonunion, and scapholunate dissociation.

Carpal collapse potentially accompanies greater and lesser arc perilunate injuries; unstable, acute scaphoid fracture; scaphoid nonunion with humpback deformity; and scapholunate dissociation. When surgical treatment is elected, preliminary reduction of the lunate from extension to neutral--to correct dorsal intercalated segment instability deformity--facilitates restoration of carpal height and decreases the risk of carpal malalignment and scapholunate advanced-collapse arthritis. This simple technique is described.

ACR Appropriateness Criteria® Osteonecrosis: 2022 Update.

Osteonecrosis is defined as bone death due to inadequate vascular supply. It is sometimes also called "avascular necrosis" and "aseptic necrosis" when involving epiphysis, or "bone infarct" when involving metadiaphysis. Common sites include femoral head, humeral head, tibial metadiaphysis, femoral metadiaphysis, scaphoid, lunate, and talus. Osteonecrosis is thought to be a common condition most commonly affecting adults in third to fifth decades of life. Risk factors for osteonecrosis are numerous and include trauma, corticosteroid therapy, alcohol use, HIV, lymphoma/leukemia, blood dyscrasias

Multiple de novo gene variations in a progeroid phenotype case report: haploinsufficiency mechanisms.

We are presenting the case of a 6-year-old male patient with progeroid phenotype and severe developmental delay referred to Genetic clinic. Given the complex phenotype an extensive metabolic and genetic evaluation was performed including a whole exome sequencing analysis that showed genetic variants in TTR, RELN, MYH6, PHIP, and SYNE2 genes. Patients' mother and brother were analyzed for the genetic variants in MYH6, PHIP and RELN. Both had same variants on PHIP and RELN as our patient, with no apparent phenotypical consequences. Physical examination was remarkable for dysmorphism including pl

Application of artificial intelligence in the diagnosis of scaphoid fractures: impact of automated detection of scaphoid fractures in a real-life study.

PURPOSE: We evaluated the diagnostic performance of two AI software programs (BoneView and RBfracture) in assisting non-specialist radiologists (NSRs) in detecting scaphoid fractures using conventional wrist radiographs (X-rays). METHODS: We retrospectively analyzed 724 radiographs from 264 patients with wrist trauma. Patients were classified into two groups: Group 1 included cases with a definitive diagnosis by a specialist radiologist (SR) based on X-rays (either scaphoid fracture or not), while Group 2 comprised indeterminate cases for the SRs requiring a CT scan for a final diagnosis. Diagnosis of occult scaphoid fractures: an exploratory study to assess whether clusters of clinical features are diagnostic in comparison to reduced sequence magnetic resonance imaging.

BACKGROUND: Occult scaphoid fractures are difficult to diagnose radiographically. Evidence regarding prevalence and diagnostic accuracy of clinical tests is growing; however, gaps in knowledge remain and further research is needed. PURPOSE: This study aimed to determine the prevalence of occult scaphoid fractures and other hand/wrist fractures, plus any clinical/demographic findings diagnostic of scaphoid fractures. METHODS: Prognostic prospective cohort study. Patients referred with diagnosis of occult scaphoid fractures, non-diagnostic radiographs, and one or more positive provocative test

Interobserver variability among radiologists for diagnosis of scaphoid fractures by computed tomography.

PURPOSE: To determine the interobserver variability among radiologists for computed tomography (CT) diagnosis of scaphoid fractures. METHODS: Four specialized musculoskeletal radiologists evaluated the CT scans of 150 consecutive patients who were clinically suspected of having sustained a scaphoid fracture but whose scaphoid-specific radiographs were normal. The radiologists were asked to determine the presence or absence of a scaphoid fracture and to localize the fracture. Interobserver agreement was calculated using the kappa statistic. RESULTS: The radiologists diagnosed between 11 (7%)

Association between lunate morphology and carpal collapse in cases of scapholunate dissociation.

PURPOSE: Type II lunate morphology has recently been shown to decrease the incidence of dorsal intercalated segment instability (DISI) deformity in patients with scaphoid nonunions. A similar association has been suggested for scapholunate dissociation, but a formal comparison has yet to be performed. The purpose of this study was to determine if an association exists between lunate morphology and DISI in cases of scapholunate dissociation. METHODS: A retrospective review was performed on 58 patients with the diagnosis of scapholunate dissociation as determined by use of radiographs, magnetic

Observer variation in MRI for suspected scaphoid fractures.

The aim of this study was to evaluate prospectively the observer variation in the detection of pathology on MRI for suspected acute scaphoid fracture. 79 consecutive MR scans were included to calculate the inter-observer variation. All patients were suspected of having a scaphoid fracture but had no fracture on radiographs. 38 randomly chosen MR scans were used to calculate the intra-observer variation. Four observers, with varying levels of expertise, blinded scored three items: (i) scaphoid fracture, (ii) localization of a scaphoid fracture, and (iii) another fracture. The observer variation
