Complications in aphakic eyes after vitreous loss.

In our 114 patients with vitreous loss during a cataract operation there were 21 cases (18.4%) which resulted in severe after-effects leading to the conclusion that vitreous loss during a cataract operation provokes a more frequent development of postoperative complications. Therefore a careful surgical procedure with exact repositioning of the vitreous-free anterior chamber, as well as regular and careful follow-up examinations are required. At our hospital we regularly carry our a three-mirror-contact lens examination after four weeks to enable us to perform a preventive treatment of the ret

Retinal detachments and topical ocular miotics.

There is increasing suspicion that topical ocular miotic therapy in selected persons may precipitate retinal detachments. This is a rare event and probably does not occur in patients free of retinal pathology. Data supporting a possible cause and effect relationship, obtained from the National Registry of Drug-Induced Ocular Side Effects, a survey of the Retina Society, medicolegal decisions, and a review of the literature, is presented.

[Trabeculotomy and cataract-extraction: simultaneously or step by step? (author's transl)].

In 64 eyes the combined procedure (group I) and in 67 aphacic eyes a trabeculotomy (group II) was performed. There were different types of glaucoma. Re-examinations wer done 6 months and 1--10 years postoperatively. No definite differences of operative or postoperative complications existed between the two groups. In eyes suffering from primary open angle glaucoma intraocular pressure was controlled in 65--91% in both groups. In group I visual acuity was slightly better. On the other hand in these eyes more often a deterioration of the visual field was observed inspite of i. o. pressure not ex

Early Patching Behaviors That Improve the Chances of Good Visual Acuity in Children Treated for Unilateral Congenital Cataract.

PURPOSE: Achieving near normal vision after unilateral congenital cataract (UCC) surgery is possible but requires early surgery, optical correction, and consistent patching. Patching is often challenging for children and their caregivers. The goal of these analyses is to examine the association between reported consistency in patching during the first year after surgery and visual acuity (VA). DESIGN: Data from the Infant Aphakia Treatment Study (IATS) are used in a cohort design. PARTICIPANTS: A total of 101 children whose caregivers completed a prospective, 7-day patching diary 2 months af…

Experiences of Treatment With Contact Lenses in Aphakic Children With Unilateral Congenital Cataract: A Retrospective Study.

BACKGROUND AND OBJECTIVES: Congenital cataract is a condition compromising the eye's crystalline lens in infants and is usually diagnosed at birth. It can lead to irreversible vision loss if not promptly detected and treated, especially in unilateral cases. Nowadays, children with congenital cataracts can undergo surgical removal of their opacified crystalline lenses, and visual rehabilitation is mandatory to prevent deep amblyopia. Contact lenses, predominantly of rigid gas-permeable (GP) type, are gaining more popularity for this matter. METHOD: In this retrospective cohort study, unilatera

Suture clamping of intraocular lenses without a scleral flap: A new implantation method for patients with an aphakic capsule.

We aim to describe a technique for intraocular lens (IOL) implantation with suture clamping for aphakic capsules. This prospective, noncomparative, interventional case study included three eyes of three patients with an aphakic capsule after vitrectomy, in which the IOL was clamped by suturing. The suturing procedure was transconjunctival or subconjunctival performed without scleral flaps. The IOLs were implanted without difficulty, and visual acuity improved after IOL implantation. There were no severe complications, and the position of each IOL remained correct through 18 months of follow-up

Transfixion technique for the treatment of silicone oil pupillary block glaucoma refractory to Nd-YAG iridotomy.

BACKGROUND: Pupillary block induced by silicon oil in vitrectomized patients is a common condition usually treated with Nd-YAG laser iridotomy or with surgical removal of silicon oil. CASE REPORT: A case of silicone oil pupillary block glaucoma successfully treated with a 30 G needle transfixion technique is described. We performed a non-complicated vitrectomy surgery for retinal detachment with proliferative vitreoretinopathy that included lensectomy, inferior peripheral iridectomy, and silicone oil injection. After surgery, the iridectomy became occluded with fibrous tissue and the intraocu

A new surgical technique using steel suture for trans-scleral fixation of posterior chamber intraocular lenses.

BACKGROUND: A new emerging complication of trans-scleral fixation of posterior chamber (PC) intraocular lens (IOL) with polypropylene suture is high rates of spontaneous dislocation of the IOL due to disintegration or breakage of suture. MATERIALS: We report a new surgical technique of trans-scleral fixation of posterior chamber intraocular lens (SF PCIOL) with steel suture to eliminate the complication of dislocation of IOL fixed with polypropylene suture in one adult and a child. RESULTS: We successfully achieved stable fixation and good centration of IOL after SF PCIOL with steel suture i

Selection of an initial contact lens power for infantile cataract surgery without primary intraocular lens implantation.

PURPOSE: To provide guidelines for the selection of an initial contact lens (CL) power based on the preoperative characteristics of the patient in eyes undergoing infantile cataract surgery without primary intraocular lens (IOL) implantation. DESIGN: Cohort study. PARTICIPANTS: Eyes were included if cataract surgery was performed without primary IOL implantation before 1 year of age, a SilSoft CL (Bausch & Lomb, Rochester, NY) was placed immediately after surgery, and postoperative refraction data were available within 1 month after surgery. METHODS: The target CL power was calculated by us

Outcome of angle surgery in children with aphakic glaucoma.

PURPOSE: To investigate the outcome of trabeculotomy and/or goniotomy for pediatric aphakic glaucoma. METHODS: Retrospective chart review of consecutive children who had congenital cataract surgery between 1990 and 2006 and required goniotomy and/or trabeculotomy for aphakic glaucoma. Treatment success was defined as postoperative intraocular pressure of <or=24 mm Hg despite topical medication use, avoidance of trabeculectomy or shunt placement, and no visually significant complications in the follow-up period. Exclusion criteria included a diagnosis of anterior segment dysgenesis, microcorne

Incidence of early-onset glaucoma after infant cataract extraction with and without intraocular lens implantation.

AIMS: To determine the incidence of glaucoma, with onset within 1 year after the date of cataract surgery (early onset) performed in the first year of life, with or without intraocular lens (IOL) implantation. METHODS: A retrospective review of a single surgeon's cohort from 1999 to 2006. Glaucoma onset risk, comparison of aphakic/pseudophakic eyes and IOL type were analysed together with microcornea, persistent fetal vasculature (PFV) and age < or =4 weeks at surgery. RESULTS: Ninety-eight eyes (62 patients; mean age 2.88 months) were included with 61 eyes (36 patients) aphakic (57 planned

Contact lens rehabilitation following repaired corneal perforations.

BACKGROUND: Visual outcome following repair of post-traumatic corneal perforation may not be optimal due to presence of irregular keratometric astigmatism. We performed a study to evaluate and compare rigid gas permeable contact lens and spectacles in visual rehabilitation following perforating corneal injuries. METHOD: Eyes that had undergone repair for corneal perforating injuries with or without lens aspiration were fitted rigid gas permeable contact lenses. The fitting pattern and the improvement in visual acuity by contact lens over spectacle correction were noted. RESULTS: Forty eyes o

Primary rhegmatogenous retinal detachment: 20 years of change.

AIM: To compare characteristics, management, and outcome of two groups of patients with primary rhegmatogenous retinal detachment (RRD) presenting to the same vitreoretinal unit approximately 20 years apart. METHODS: 124 patients in 1979-80 and 126 cases in 1999 were compared. RESULTS: More cases were pseudophakic and fewer aphakic in 1999 than 1979-80. More cases of giant retinal tear and fewer dialyses were operated on in 1999. Vitrectomy was a primary procedure in 63% of cases in 1999 but only 1% in 1979-80. Anatomical success rates were statistically similar: 79.8% primary and 88.8% fina

Aphakic hydrogel contact lens fitting on a monocular canine: a case report.

BACKGROUND: Though plano bandage contact lenses used for therapeutic purposes are not uncommon for dogs, no literature regarding contact lenses to correct aphakic canines currently exists. CASE REPORT: Oliver, a 7-year-old terrier mix, was aphakic in his left eye and essentially blind in his right eye as the result of a large retinal detachment. Surgical complications and endothelium damage contraindicated an intraocular lens implant in his left eye. While co-managed with his veterinary ophthalmologist, Oliver was prescribed an aphakic hydrogel contact lens to improve his monocular vision. C
