Anatomic consideration of C2 pedicle screw placement.

STUDY DESIGN: This anatomic study tested placement of C2 pedicle screws using cadaver specimens. OBJECTIVES: To further assess the safety of transpedicular screw placement in the axis by comparing two surgical techniques. SUMMARY OF BACKGROUND DATA: Transpedicular screw fixation of traumatic spondylolisthesis of the axis has been described in the literature. Recently, anatomic studies and clinical applications of transpedicular screw fixation for traumatic lesions of middle and lower cervical spine have been described. No previous study assessing the safety of C2 pedicle screw placement is a

Bow Hunter's Syndrome: Complete Microsurgical Decompression of Vertebral Artery by Far Lateral Retrocondylar Approach: 3-Dimensional Operative Video.

This 50-yr-old man had a 15-yr history of presyncopal episodes that were precipitated by turning his head to the right, and had worsened recently. Cerebral angiogram demonstrated complete cessation of anterograde flow in left vertebral artery (VA) at the level of the C1 sulcus arteriosus while turning head to right, indicating dynamic compression at the C1 level.  Patient underwent left extreme lateral retrocondylar approach, partial C1 laminectomy and opening of the C1 foramen with complete microsurgical decompression of the VA. After skin incision, meticulous muscle dissection was performed

Percutaneous subclavian artery stent-graft placement following failed ultrasound guided subclavian venous access.

BACKGROUND: Ultrasound guidance for central and peripheral venous access has been proven to improve success rates and reduce complications of venous cannulation. Appropriately trained and experienced operators add significantly to diminished patient morbidity related to venous access procedures. We discuss a patient who required an arterial stent-graft to prevent arterial hemorrhage following inadvertent cannulation of the proximal, ventral, right subclavian artery related to unsuccessful ultrasound guided access of the subclavian vein. CASE PRESENTATION: During pre-operative preparation for

Delayed Diagnosis of Postpartum Subarachnoid Hemorrhage Caused by Vertebral Artery Aneurysm: A Case Initially Misdiagnosed as Post-Dural Puncture Headache.

BACKGROUND: Postpartum subarachnoid hemorrhage caused by aneurysm rupture is a rare but potentially life-threatening complication that can mimic a benign postpartum headache, especially after neuraxial anesthesia. CASE REPORT: We describe a 40-year-old woman who developed a severe headache, predominantly located in the occipital and posterior neck region, following cesarean section under spinal anesthesia. Initially presumed to be post-dural puncture headache, her condition deteriorated by postoperative day 5, with coma and seizures. Cranial computed tomography revealed diffuse subarachnoid a

Selective postmortem arterial angiography of head and neck using digital x-ray or computed tomography: A practical and rapid method to identify vascular injuries.

Identification of vascular injuries is crucial for complete postmortem evaluation and understanding of trauma deaths by the Medical Examiner. Some vascular injuries are difficult to evaluate due to challenging anatomic locations, especially in the head and neck. Documenting injuries of the facial and vertebral arteries is challenging and necessitates time-consuming dissections that can create artifacts and disfigurement. In busy medical examiner offices with a significant number of traumatic injuries, finding a creative solution to employ reliable postmortem angiography is desirable. At the Of

First case of indigenous visceral leishmaniasis from central India.

Because of the presumed non-atherosclerotic pathogenesis, the potential link between spontaneous cervical artery dissection (sCAD) and common risk factors for atherosclerosis has never been investigated systematically. Therefore, this prospective, multicentre, case-control study compared the frequency of tobacco use, hypertension, diabetes mellitus, and hypercholesterolaemia among a group of consecutive patients with sCAD (n = 153), a group of patients with ischaemic stroke, not related to CAD (non-CAD), and a group of controls. As opposed to the other variables, a trend towards a significant

Extracranial Revascularization Therapy: Angioplasty and Stenting.

Beginning with simple balloon angioplasty, minimally invasive revascularization techniques have progressed to the use of metallic stents for improved immediate and long-term results. Stent-supported angioplasty now offers a therapeutic option for those individuals ineligible for surgical revascularization of stenotic atherosclerotic lesions and who have failed maximal medical therapy. However, the clinical equivalence, or possibly even superiority, of angioplasty of the extracranial carotid and vertebral arteries in atheromatous occlusive disease over surgical revascularization has yet to be d

Associations between prescribed amphetamines and spontaneous cervical artery dissection: a matched case-control study.

AIMS: To assess whether prescribed amphetamines are associated with spontaneous cervical artery dissection (sCeAD), an important cause of stroke in young adults. METHODS: We conducted a retrospective matched case-control study within a large academic health system in Chicago, Illinois. Adults aged 18-85 years with incident sCeAD were matched 1:4 to controls without dissection on age, sex, and race/ethnicity. The primary exposure was documented amphetamine prescription. Conditional logistic regression was used to estimate adjusted odds ratios (ORs) controlling for relevant vascular and clinica

Vasa Vasorum and the Growing of Thrombosed Giant Aneurysm of the Vertebral Artery: A Case Report.

BACKGROUND: Giant aneurysms arising from the vertebral artery (VA) are often associated with thrombosis and present as mass lesions with slow progression of symptoms and signs. A consensus is still to be established on the underlying growth mechanisms and surgical management of thrombosed giant aneurysms of the VA. CASE DESCRIPTION: Here, we report the case of a 54-year-old man who sought neurosurgical care after 3 months of progressive cervical axial pain. He underwent cervical magnetic resonance imaging and cerebral angiography, which revealed a giant VA aneurysm with thrombosed component.

Surgical treatment of intracranial VA dissecting aneurysm.

OBJECT: Intracranial VA dissections are reported to cause headache, brain stem infarction, and SAH with an associated high morbidity and mortality. We aimed to clarify both the clinical characteristics and effectual treatment of intracranial VA dissections, and to present a retrospective analysis of our experience in the treatment. MATERIAL AND METHOD: Between 1995 and 2007 we experienced 62 VA dissections in our institution. Fourteen of 62 (23%) cases of VA dissections were associated with aneurysm, and received surgical treatment. Five of fourteen cases presented with SAH, 8/14 cases with i

Surgical Treatment of Complex Cerebral Aneurysms Using Interposition Short Vein Graft.

BACKGROUND AND AIMS: Cerebral revascularization strategies may become necessary in select patients who present with challenging cerebral aneurysms. In this study, we present the techniques of a moderate-flow extra-intracranial bypass using a short interposition vein graft and concurrent aneurysm management. METHODS: The short interposition vein graft was used for the reconstruction of complex cerebral aneurysms in nine patients. In eight of them, the superficial temporal artery (STA) main trunk was used as a donor site for the anastomosis of a short interposition vein graft, and an extracrani

Prevalence and manifestations of diagnosed fibromuscular dysplasia by sex and race: Analysis of >4500 FMD cases in the United States.

OBJECTIVE: Fibromuscular Dysplasia (FMD) is a rare non-atherosclerotic non-inflammatory vascular disease associated with arterial aneurysms and dissections. The epidemiology of FMD is not well understood. We sought to characterize the epidemiology of FMD utilizing a large aggregated electronic medical record database. METHODS: We used a commercial database (Explorys, IBM Watson), which aggregates data from electronic health records from 26 major integrated healthcare systems in the United States. Fibromuscular dysplasia cases were identified using a unique systemized nomenclature of medical t

Far Lateral Craniotomy for Obliteration of High-Risk Craniocervical Junction Arteriovenous Fistula.

BACKGROUND AND INTRODUCTION: Dural arteriovenous fistulas (dAVFs) are a rare pathology with a clinical presentation related to their anatomical location. Craniocervical junction (CCJ) dAVFs are challenging to treat given the delicate structures that surround the CCJ. Endovascular treatment has evolved significantly in the past decade, but open microsurgery remains an invaluable tool for this pathology. OBJECTIVE: To demonstrate the step-by-step elements of the far lateral approach for microsurgical ligation of CCJ dAVF. SURGICAL TECHNIQUE: A retroauricular incision is created, extending down

Microsurgical anatomy of the venous drainage of the mesencephalodiencephalic junction.

OBJECTIVE: The veins draining the posterior wall of the third ventricle and its adjacent structures and the posterior part of the midbrain have been the most neglected of the intracranial vascular structures in both the anatomic and neurosurgical literature. During our dissections of the pineal region and the quadrigeminal cistern, we did not always encounter topographic anatomy as described in previous articles. The purpose of this study is to describe the topographic anatomy and normal variations of the specific veins that drain the collicular plate and the pineal body and their adjacent str

Hypoglossal carotid entrapment syndrome.

Inflammation of the upper respiratory tract if frequently contiguous with the ICA and the HN in the neck. If severe, the inflammation of itself may occlude or directly extent into the ICA wall. The resulting scar produces a fixed relationship between artery and nerve that is vulnerable to lymph node enlargement, by head position, or blood pressure elevation. Trauma in the absence of scar may result in intimal injury of the ICA. Hypoglossal carotid entrapment may give rise to arterial stricture or diaphragm formation, microembolism, dissecting aneurysm, and arterial occlusion. Hypoglossal palsy

Ultrastructural connective tissue abnormalities in patients with spontaneous cervicocerebral artery dissections.

The cause of spontaneous cervicocerebral artery dissection is unknown. An underlying arteriopathy due to a connective tissue disorder has often been presumed. We studied 25 patients with proven nontraumatic dissections. The ultrastructural morphology of dermal connective tissue components was assessed by transmission electron microscopy of skin biopsies. Ultrastructural abnormalities were seen in 17 (68%) patients, resembling in some cases the aberrations found in Ehlers-Danlos syndrome type II or III. These observations indicate a correlation of cervical artery dissections with connective tis
