Significance of block distal to the His bundle induced by atrial pacing in patients with chronic bifascicular block.

Twenty-one of 496 (4%) patients with chronic bifascicular block, studied and followed prospectively, had block distal to the His bundle (BDH) induced by atrial pacing during initial electrophysiologic studies. In six, BDH was noted during pacing-induced atrioventricular (AV) nodal Wenckebach periods (at paced rates of 150--190 beats/min), with BDH in the short HH cycles after the AV nodal blocked P (lond cycle). The AH interval was normal in all six patients and HV was normal in four. None of the six patients has developed AV block during a mean follow-up of 5.33 +/- 0.48 years. In 15 patients

[Dromotropic effects of drug combinations. Initial results bearing on a combination of deslanoside and ajmaline].

The electrophysiological effects of the cardiotropic drugs have been studied in man by the agency of endocavitary electrocardiography. The effects of drug combinations, which are often prescribed therapeutically, have been studied less often. The authors report the results of a preliminary study of the combination of deslanoside with ajmaline in 26 patients; its effects were compared with those using each drug separately. This combination seems to have true dromotropic effects; although deslanoside alone, in the doses used, does not modify conduction below the bundle of His, it can still act s

Dual atrioventricular nodal pathways. A common electrophysiological response.

Evidence of dual atrioventricular nodal pathwats (a sudden jump in H1-H2 at critical A1-A2 coupling intervals) was shown in 41 out of 397 patients studied with atrial extrastimulus techniques. In 27 of these 41, dual pathways were demonstrable during sinus rhythm, or at a cycle length close to sinus rhythm (CL1). In the remaining 14, dual pathways were only demonstrated at a shorter cycle length (CL2). All patients with dual pathways at cycle length who were also tested at cycle length (11 patients) had dual pathways demonstrable at both cycle lengths. In these 11 patients both fast and slow p

[Atrial sensing and atrioventricular synchrony in single lead VDD pacemakers. Can the appearance of atrial undersensing be predicted?].

Single-lead VDD-pacing is an alternative to DDD-systems in patients with AV-block and normal sinus node function. Atrial sensing plays a central role in these pacemakers. AV-synchrony, incidence of atrial arrhythmias and the occurrence of sinus node disease were investigated in 108 patients with VDD-pacemakers followed over a mean period of 24.8 months after implantation. Determinants influencing the occurrence of atrial undersensing were especially focused on. Mean atrial potential and sensing threshold were reduced within the first 2 weeks after implantation (p < 0.01). Intermittent atrial u

His bundle recordings in a case of complete atrioventricular block combined with pre-excitation syndrome.

In a patient with complete A-V block suffering from attacks of dizziness an intermittent A-V conduction with a short P-R interval and a delta wave of the conducted ventricular complex were observed. After accelerating the sinus rate by atropine and by exercise, one-to-one conduction was established with QRS complexes of WPW type A configuration. His bundle recordings revealed a complete block within the normal conduction system at the level of the A-V node. A slow junctional rhythm with a normal H-V interval was activating the ventricle. During atrial pacing a one-to-one conduction through an

Incidence and risk factors for permanent pacemaker implantation following aortic root surgery.

BACKGROUND: Permanent pacemaker implantation (PPI) remains a serious complication following aortic root replacement (ARR) and valve-sparing root replacement (VSRR). This is due to the proximity of the atrioventricular node and the bundle of His to the aortic valve annulus and the left ventricle. The data regarding PPI after aortic root surgery are sparse. We describe the incidence, indication and timing of PPI following conventional ARR and VSRR. METHODS: 727 consecutive patients undergoing elective and urgent aortic root surgery (549 ARR, 178 VSRR) between 2006-2023 were studied. Patients re…

Minimum and Average Heart Rates in Pediatric Fontan Patients with Favorable Postoperative Outcomes: A Holter Monitoring Study.

Fontan palliation is associated with an increased risk for arrhythmias and sinus node dysfunction. Although annual Holter monitoring is recommended for routine surveillance, there is no normative Holter data for this population, with only one study describing such data in healthy children. To describe Holter data for patients with Fontan physiology, compare this data to healthy children, and establish a normative data set for those undergoing routine ambulatory ECG screening. This retrospective, single-center study analyzed 642 Holter monitors from 133 Fontan patients aged 5-21 years, collecte

Reciprocated tachycardias in cardiac laminopathy: a clinical case report.

BACKGROUND: Cardiac laminopathies, associated with mutations in the LMNA gene, are a rare inherited disorder characterized by a broad range of clinical manifestations. There are currently no data on the association between supraventricular re-entrant tachycardias and LMNA-related cardiomyopathy. CASE SUMMARY: A 26-year-old male presented with either wide-QRS tachycardia with a left bundle branch block (LBBB) pattern or narrow QRS tachycardia, as well as a history of palpitations since age 15. Echocardiography showed no overt structural heart disease. Electrophysiological studies confirmed the

Risk stratification and management of arrhythmias in patients with Wolff-Parkinson-White syndrome.

Wolff-Parkinson-White (WPW) syndrome is a cardiac disorder characterized by an accessory pathway known as the bundle of Kent, which bypasses the atrioventricular node and predisposes patients to tachyarrhythmias, including supraventricular tachycardia and atrial fibrillation. The prevalence of WPW syndrome ranges from 0.1% to 0.5% globally, with a higher incidence in males and diagnosis typically occurring between ages 10 and 30. Although many individuals remain asymptomatic, WPW presents a risk of life-threatening arrhythmias, including sudden cardiac death, particularly when atrial fibrillat…

Alcohol intake and bradyarrhythmia risk: a cohort study of 407 948 individuals.

AIMS: There is a paucity of epidemiological evidence on alcohol and the risk of bradyarrhythmias. We thus characterized associations of total and beverage-specific alcohol consumption with incident bradyarrhythmias using data from the UK Biobank. METHODS AND RESULTS: Alcohol consumption reported at baseline was calculated as UK standard drinks (8 g alcohol)/week. Bradyarrhythmia events were defined as sinus node dysfunction (SND), high-level atrioventricular block (AVB), and permanent pacemaker implantations. Outcomes were assessed through hospitalization and death records, and dose-response

Hemodynamic Impact of Atrial Pacing in Patients with Fontan Physiology and Junctional Rhythm: A Cardiac Catheterization Study.

Sinus node dysfunction is a common problem and adversely impacts patients who have undergone the Fontan operation. In Fontan patients with an adequate junctional escape rhythm, the benefit of atrial pacing to restore atrioventricular synchrony remains unclear. Data were collected retrospectively on all Fontan patients with junctional rhythm who underwent atrial pacing during cardiac catheterization. Hemodynamics were obtained at baseline and after atrial pacing for 5-10 min. Seven patients, mean age 10 years (3-21) were studied. The type of Fontan was extracardiac in 6 and lateral tunnel in on

Left Bundle Branch Pacing: Current Knowledge and Future Prospects.

Cardiac pacing is an effective therapy for treating patients with bradycardia due to sinus node dysfunction or atrioventricular block. However, traditional right ventricular apical pacing (RVAP) causes electric and mechanical dyssynchrony, which is associated with increased risk for atrial arrhythmias and heart failure. Therefore, there is a need to develop a physiological pacing approach that activates the normal cardiac conduction and provides synchronized contraction of ventricles. Although His bundle pacing (HBP) has been widely used as a physiological pacing modality, it is limited by cha

Novel atrioventricular sequential pacing approach using a transvenous atrial pacemaker and a leadless pacemaker: a case report.

BACKGROUND: The use of transvenous pacing leads is associated with the risk of developing tricuspid valve (TV) dysfunction. This develops through several mechanisms including the failure of leaflet coaptation or direct damage to the TV or to its sub-valvular apparatus and can result in significant tricuspid regurgitation (TR). Multiple approaches to pacemaker implantation after transvenous lead extraction (TLE) or surgical TV repair have been described. Placement of pacing leads across the TV is generally avoided in such circumstances. CASE SUMMARY: A 66-year-old woman presented with a year-l

Left bundle branch pacing for symptomatic bradycardia: Implant success rate, safety, and pacing characteristics.

BACKGROUND: In patients with or without left bundle branch block, left bundle branch pacing (LBBP) can produce near normalization of QRS duration (QRSd). This has recently emerged as an alternative technique to His bundle pacing. OBJECTIVES: The purpose of this study was to characterize a novel approach for LBBP in patients with bradycardia indications for pacing and to assess implant success rate and midterm safety. METHODS: Patients with bradycardia indications for pacing underwent LBBP by a trans-ventricular-septal method in the basal ventricular septum. Procedural success, pacing paramet

Management of cardiac hemochromatosis.

Iron-overload syndromes may be hereditary or acquired. Patients may be asymptomatic early in the disease. Once heart failure develops, there is rapid deterioration. Cardiac hemochromatosis is characterized by a dilated cardiomyopathy with dilated ventricles, reduced ejection fraction, and reduced fractional shortening. Deposition of iron may occur in the entire cardiac conduction system, especially the atrioventricular node. Cardiac hemochromatosis should be considered in any patient with unexplained heart failure. Screening for systemic iron overload with serum ferritin and transferin saturat…
