Hypercalcemia, hypercalciuria and nephrocalcinosis in a breast-fed term newborn: a rare presentation.

Although hypercalcemia and hypercalciuria are known to occur in breast-fed pre-term infants, to the best of our knowledge, it has never been reported in a term baby previously. We report a term male baby who was followed-up during pregnancy for having bright kidneys, but a follow-up renal ultrasound (US) after birth had revealed normal scan. Laboratory investigations revealed normal serum calcium (Ca), phosphorous (PO₄) and alkaline phosphatase (ALP). The baby was being fed by breast milk. Follow-up US two months later showed early nephrocalcinosis along with hypercalcemia and hypercalciuria;

Relactation Success: A Randomized Controlled Trial of Health Promotion Model-Based Education and Follow-Up.

BACKGROUND: Early breastfeeding cessation due to physiological and psychosocial factors limits long-term maternal and infant health benefits. Relactation, the reinitiation of breastfeeding, is a promising strategy to restore lactation and improve well-being. This study aimed to evaluate the effect of education and follow-up based on Pender's Health Promotion Model (HPM) on relactation success among mothers who had discontinued breastfeeding or experienced low milk supply. METHODS: This randomized controlled trial was conducted between July and December 2023 in a hospital and five family healt

Frey's syndrome in an infant misdiagnosed as food allergy.

BACKGROUND: Frey's syndrome (auriculotemporal syndrome) is characterized by gustatory flushing and sweating in the distribution of the auriculotemporal nerve. Although classically described after parotid surgery in adults, congenital and idiopathic forms are increasingly recognized in infants. Because symptoms occur during feeding, the condition can closely mimic IgE-mediated food allergy, leading to unnecessary dietary restrictions and anxiety. CASE PRESENTATION: We report an 8-month-old boy referred for evaluation of suspected multiple food allergies after several emergency department visit

Developing a Framework for Culturally Sensitive Breastfeeding Interventions: A Community Needs Assessment of Breastfeeding Experiences and Practices in a Black Immigrant Community.

BACKGROUND/OBJECTIVES: Despite high breastfeeding initiation rates nationwide, disparities in breastfeeding continuation among Black mothers remain a public health issue. The BreastFeed Iowa Black Immigrant Project aimed to improve breastfeeding rates among low-income Black immigrant mother-infant dyads in Johnson County, Iowa by exploring factors influencing breastfeeding retention and developing a framework for culturally sensitive breastfeeding interventions. METHODS: Using a descriptive cross-sectional study design, we employed a convergent, parallel mixed-methods approach to explore fact

"I can't provide what my child needs": Early feeding experiences of caregivers of children with craniofacial microsomia.

PURPOSE: Craniofacial microsomia (CFM) is a congenital condition that can be associated with feeding challenges in infants. As part of the larger 'Craniofacial microsomia: Accelerating Research and Education (CARE)' program, this study described caregivers' early feeding experiences. DESIGN AND MATERIALS: US-based caregivers of 34 children with CFM participated in remote narrative interviews. Two authors completed inductive thematic analysis in an iterative process until consensus was reached. RESULTS: Caregivers' narratives outlined the inherent challenges of feeding an infant with special

Sardinian Infants of Diabetic Mothers: A Metabolomics Observational Study.

Gestational diabetes mellitus (GDM) is a condition characterized by glucose intolerance, with hyperglycemia of varying severity with onset during pregnancy. An uncontrolled GDM can lead to an increased risk of morbidity in the fetus and newborn, and an increased risk of obesity or developing type 2 diabetes, hypertension or neurocognitive developmental impairment in adulthood. In this study, we used nuclear magnetic resonance (NMR) spectroscopy and gas chromatography-mass spectrometry (GS-MS) to analyze the urinary metabolomic profile of newborns of diabetic mothers (NDMs) with the aim of iden

Emergencies in paediatric hepatology.

The aetiology of several liver diseases in children is age specific and many of these conditions have significant and potentially long-term clinical repercussions if not diagnosed early and managed in a timely fashion. We address 5 clinical scenarios that cover most of the diagnostic and therapeutic emergencies in children: infants with liver disease; acute liver failure; management of bleeding varices; liver-based metabolic disorders; and liver tumours and trauma. A wide spectrum of conditions that cause liver disease in infants may present as conjugated jaundice, which could be the only symp

Preventing mother to child transmission of HIV: lessons learned from China.

BACKGROUND: The program for the prevention of mother-to-child transmission (PMTCT) of human immunodeficiency virus (HIV) was launched in 2003 in China, but few studies have been conducted to describe the panorama of PMTCT. We investigated the rate and associated factors of mother-to-child transmission (MTCT) in China from 2004 to 2018. METHODS: HIV-infected pregnant women from two areas in China between 2004 and 2018 were enrolled. Antiretrovirals (ARVs) were provided to the mothers and their babies, and the children were followed and tested for HIV. RESULTS: In total, 857 mothers and their

Selective serotonin reuptake inhibitors' passage into human milk of lactating women.

Objective: Selective serotonin reuptake inhibitors are commonly used for the treatment of pregnancy-related and postnatal depression. However, only a few studies have evaluated the passage of these drugs into human milk, often with conflicting results. Here, we sought to evaluate the passage of selective serotonin reuptake inhibitors into human milk in the first days after delivery and their potential association with neonatal outcomes. Study design: The passage of selective serotonin reuptake inhibitors into human milk was expressed both as percentage of milk-to-plasma ratio of drug concentra

Health Effects of Carotenoids during Pregnancy and Lactation.

Adequate nutrition is particularly important during pregnancy since it is needed not only for maintaining the health of the mother, but also … pends on its diet. Both mix feeding and artificial feeding may cause depletion of carotenoids since infant formulas contain only trace amounts of these compounds. Carotenoids, particularly lutein and zeaxanthin play a significant role in the development of vision and nervous system (among others, they are important for the development of retina as well as energy …

Born at 27 weeks of gestation with classical PKU: challenges of dietetic management in a very preterm infant.

Few cases of premature infants with classical phenylketonuria (PKU) have been reported. Treatment of these patients is challenging due to the lack of a phenylalanine (Phe)-free amino acid (AA) solution for parenteral nutrition. A boy born at 27 weeks of gestation with a weight of 1000 g was diagnosed with classical PKU on day 7 because of highly elevated Phe level at newborn screening (2800 µmol/L). Phe intake was suspended for 5 days and during this time intravenous glucose and lipids as well as small amounts of Phe-free formula through nasogastric tube were given. Because of insufficient wei

Closing the quality gap: promoting evidence-based breastfeeding care in the hospital.

Evidence shows that hospital-based practices affect breastfeeding duration and exclusivity throughout the first year of life. However, a 2007 CDC survey of US maternity facilities documented poor adherence with evidence-based practice. Of a possible score of 100 points, the average hospital scored only 63 with great regional disparities. Inappropriate provision and promotion of infant formula were common, despite evidence that such practices reduce breastfeeding success. Twenty-four percent of facilities reported regularly giving non-breast milk supplements to more than half of all healthy, fu…

The measurement of the QT and QTc on the neonatal and infant electrocardiogram: a comprehensive reliability assessment.

BACKGROUND: An electrocardiogram has been proposed to screen for prolonged QT interval that may predispose infants to sudden death in the first year of life. Understanding the reliability of QT interval measurement will inform the design of a screening program. METHODS: Three pediatric cardiologists measured the QT/RR intervals on 60 infant electrocardiograms (median age 46 days), from leads II, V5 and V6 on three separate occasions, 7 days apart, according to a standard protocol. The QTc was corrected by Bazett's (QTcB), Fridericia's (QT(CFrid)), and Hodges' (QTcH) formulae. Intraobserver an

The dilemma of postnatal mother-to-child transmission of HIV: to breastfeed or not?

The promotion of nearly universal breastfeeding has played an important role in improving child health by providing optimum nutrition and protection against common childhood infections, and by promoting child spacing. Unfortunately, it has become clear that breastfeeding is responsible also for much of the increasing burden of worldwide pediatric human immunodeficiency virus (HIV) infection, especially in the developing nations (12-14% additional risk of HIV infection transmitted by breastfeeding; 35% total proportion of all HIV-infected children in an area infected through breastfeeding). Sev

Decreased Oral Tolerance to Egg Allergen Ovalbumin in Mice Fed an Amino Acid-Based Diet via Triggering Epithelial-Derived IL-33 Release and Suppressing p-Treg Production.

Oral tolerance (OT) to dietary antigens is crucial to preventing food allergy (FA). The generation and maintenance of the OT require constant stimulation with dietary antigens. Infants fed long-term with amino acid-based formulas (AAFs) lack this stimulation, leaving the impact on subsequent OT induction unknown. Herein, we investigated the effects of long-term AAF consumption on OT development by subjecting neonatal mice to an amino acid-based diet (AAD) before introducing the egg allergen ovalbumin. The results showed that the AAD significantly impaired OT generation. Upon an oral allergen c
