Hanging consists of the external pressure of the neck by a ligature through suspension of the body. It may cause death by asphyxiation, vascular change, cerebral ischaemia, anoxia or bone trauma. Skeletal trauma by hanging occurs predominantly at the axis, but may likewise take place at the cranial base, other upper cervical vertebrae, and hyoid bone. Six individuals from the Lisbon Identified Skeletal Collection, whose cause of death was by hanging, were macroscopically analyzed for perimortem trauma at the cranium and cervical area. Only one female individual of 53 years exhibited perimortem

A rare anomaly of the C-2 vertebra (axis) was found in a 54-year-old man with myelopathy. In addition to spina bifida of C-2 and abnormal articulations between C-2 and C-3, the lamina of the axis was separated from the rest of the posterior arch on each side, and the free dysplastic laminae and spinous process invaginated deeply into the spinal canal. The condition differed from the infrequent but previously described cervical spondylolysis in that the pars interarticularis and the pedicle were not dehiscent. Shaded three-dimensional computed tomographic display proved most useful in demonstra

The craniocervical junction is the transition between the skull and the vertebral column that provides mobility while maintaining sufficient stability (i.e., protection of the brainstem and the spinal cord). The key elements involved are the occiput, the first cervical vertebra (CV1, atlas) and the second cervical vertebra (CV2, axis). The two vertebrae forming the atlas-axis complex are distinct in their morphology and differences in form have been linked to differences in ecological function in mammals. Here, we quantified the morphological diversity of the cranium, CV1 and CV2 in a sample o

PURPOSE: Distal junctional kyphosis (DJK) is a significant complication following posterior cervical or cervicothoracic fusion. Despite the increasing number of these procedures, the factors associated with DJK remain underexplored, particularly compared to proximal junctional kyphosis. This systematic review and meta-analysis aimed to identify key factors associated with DJK and provide actionable recommendations for its prevention.
METHODS: A comprehensive search of PubMed, Scopus, Embase, and Web of Science was conducted to identify case-control and cohort studies reporting on factors asso

Nine of twelve family members from three generations were affected by an inherited form of cervical vertebral dysplasia. All of the affected people had an abnormality of the first cervical vertebra. Some also had defects of the axis and caudad to it. The mode of transmission of the disorder is autosomal dominant, with apparently complete penetrance and variable expressivity. Two patients had symptoms. One had a passively correctable tilt of the head, with an associated audible clunk and hypoplasia of the left superior facet of the second cervical vertebra. This patient had no local symptoms, n

We describe a patient with traumatic right-sided invagination of two consecutive laminae into the spinal canal. The injury resembled a greenstick fracture and resulted in an acute Brown-Sequard syndrome. There was also an undisplaced hangman's fracture of the axis vertebra. These injuries were caused by an acute hyperextension and axial compression of the cervical spine. Open reduction and internal fixation of the laminar fractures without fusion was followed by full neurological recovery within six weeks.

BackgroundAtlanto-occipital and atlantoaxial assimilation are rare congenital abnormalities characterized by abnormal fusion of the atlas vertebra with the occipital bone or the axis vertebra, respectively, within the craniovertebral junction. These anomalies can significantly alter the biomechanics and structural integrity of the cervical spine, leading to diverse clinical presentations ranging from restricted neck mobility to neurological deficits. Accordingly, this study aims to investigate the prevalence, anatomical abnormalities, and clinical significance of atlanto-occipital and atlantoa

Craniocervical injuries (CCJs) account for 10% to 30% of all cervical spine trauma. An increasing number of patients are surviving these injuries due to advancements in automobile technology, resuscitation techniques, and diagnostic modalities. The leading injury mechanisms are motor vehicle crashes, falls from height, and sports-related events. Current treatment with urgent rigid posterior fixation of the occiput to the cervical spine has resulted in a substantial reduction in management delays expedites treatment of CCJ injuries. Within CCJ injuries, there is a spectrum of instability, rangi

Synovial chondromatosis is an uncommon disorder characterized by the formation of multiple cartilaginous nodules within the synovium, most commonly affecting large joints. Its involvement with the spine is rare; only six cases have been reported. The authors describe two patients with synovial chondromatosis involving the cervical spine. In the first case, synovial chondromatosis arose from the left C1-2 facet joint. This patient underwent a two-stage procedure including a posterior approach for tumor resection and occipitocervical fusion as well as a transmandibular circumglossal approach to

STUDY DESIGN: A series of 10 patients surgically treated with prevascular or retrovascular extraoral retropharyngeal approach to the upper cervical spine is examined.
OBJECTIVES: In orthopedic surgery, retropharyngeal approach may replace the transoral surgery, obviating the risks of infection and the uncomfortable postoperative course of cases in which median labiomandibular glossotomy was used to accomplish complex bone reconstruction.
SUMMARY OF THE BACKGROUND DATA: The transoral approach is reported in literature as the classical anterior access to the upper cervical spine that provides

This study on the morphological features of bones of the helmeted guinea fowl (Numidia meleagridis) specifically the axial skeleton was conducted for documentation and valuable information on differences when compared with other species of the same order. Ten (10) Helmeted guinea fowls (5 females and 5 males) with an average weight of 3.0 kg were euthanized via jugular vein severance with bone preparation achieved via the use of insect larvae. The skull presented an elongated structure made up of paired and unpaired flat, short and long bones with distinctive features. The frontal bone project

Acute injuries of the cervicocranium (from the occiput to the second cervical intervertebral disk) may be radiographically obscure due to minimal displacement of fracture fragments, minor alterations of normal anatomic relationships (occipitoatlantal subluxation), or superimposition of normal skeletal structures. With the nasooropharynx adequately distended with air, the normal cervicocranial prevertebral soft-tissue contour is congruent with the anterior cortical margin of the cervicocranium; namely, concave above, convex anterior to, and concave below the anterior tubercle of C1. Alterations

Surgical fusion of the upper cervical spine was used to treat atlanto-axial subluxation in 7 patients with rheumatoid arthritis. Postsurgical follow-up was 3 to 16 yr. Despite progressive and often severe radiological changes in the cervical spine, no patient has had a clinical recurrence of neck symptoms requiring further surgery. These results suggest that surgical fusion is successful in the longterm, despite the often dramatic radiological changes which occur in the cervical spine with the passage of time.

Case Report. Spine surgery in patients with Parkinson's disease (PD) involves increased risk. We describe a case of cervical myelopathy in a patient with PD, multiple fractures involving the atlas and axis vertebrae, and spasmodic torticollis. The patient was successfully treated with an upper cervical decompression and occipital-cervical (OC) fusion surgery. Strategies for torticollis reduction and successful surgical outcome are discussed. Risks and benefits must be carefully weighed when considering occipital cervical fusion in PD patients. Conclusion. Intraoperative manual reduction of lat

BACKGROUND: Although instrumented stabilization of pediatric atlanto-occipital dislocation (AOD) has been described in the literature, there is little evidence regarding instrumentation techniques in pediatric patients presenting with both AOD and a cervical fracture. We present a case of a 2-year-old male involved in a motor vehicle collision with an unstable C2 fracture and AOD, treated with an occiput-C4 posterior arthrodesis using a rod, crosslink, and cable construct.
CASE DESCRIPTION: This patient suffered a type III C2 fracture and AOD with 4 mm craniocaudal and 3 mm anterior displacem

STUDY DESIGN: A retrospective technical report.
OBJECTIVE: To assess the effect of bilateral C1 laminar hooks combined with C2 pedicle screw fixation for the treatment of C1-C2 instability.
SUMMARY OF BACKGROUND DATA: Various posterior atlantoaxial fixations for C1-C2 instability have been developed. However, due to anatomic anomalies of the vertebral artery, the smallness of the pedicle, trajectories of broken screws, or a lack of surgical experience, a simple atlantoaxial fixation technique with good safety and effectiveness is urgently needed.
MATERIALS AND METHODS: From January 2007 to
