Comparative Analysis Between Cortical Bone Trajectory (CBT) Screw Fixation and Traditional Pedicle Screw Fixation in Lumbar Spine Surgery: A Systematic Review and Meta-Analysis.

Cortical bone trajectory (CBT) screw fixation has been proposed as an alternative method to pedicle screw (PS) fixation for lumbar instrumented fusion. However, the benefits of this technique remain controversial. We searched PubMed, Scopus, Web of Science Advance, ScienceDirect, Embase (Ovid), and Cochrane databases from inception to July 22, 2023. Articles were included if they were related to adults undergoing lumbar spine surgery and showed comparative outcomes with cortical bone and traditional PS trajectory. Twenty-three studies with 1,929 patients (833 in the CBT group and 1,096 in the

The effect of the cortical bone trajectory screw fixation and traditional pedicle screw fixation on surgical site wound infection in posterior lumbar fusion wound: A meta-analysis.

Источник пометил публикацию как отозванную

A meta-analysis investigation was performed to measure the influence of cortical bone trajectory screw fixation (CBTSF) and traditional pedicle screw fixation (TPSF) on surgical site wound infection (SSWI) in posterior lumbar fusion (PLF). A comprehensive literature inspection till February 2023 was applied and 1657 interrelated investigations were reviewed. The 13 chosen investigations enclosed 1195 individuals with PLF in the chosen investigations' starting point, 578 of them were using CBTSF, and 617 were using TPSF. Odds ratio (OR) in addition to 95% confidence intervals (CIs) were utilise

No single best technique: trade-offs in femoral interference screw fixation for bone-patellar tendon-bone anterior cruciate ligament reconstruction.

This review presents how surgeons should choose femoral interference screw in anterior cruciate ligament (ACL) reconstruction - insertion direction (inside-out vs outside-in), tunnel shape (round vs rectangular), screw position (anterior vs posterior), and screw material (metal, bioabsorbable, PEEK, magnesium) - to balance fixation strength, anatomic fidelity, and revision feasibility. Inside-out maximizes aperture compression and preserves the lateral femoral cortex but requires deep flexion and precise trajectory (risk of iatrogenic cartilage contact/posterior wall breach). Outside-in improv

Comparison of arthroscopic anterior cruciate ligament reconstruction by bone-patellar tendon-bone graft with or without using interferential screw in general population.

Rupture of the Anterior Cruciate Ligament (ACL) is common, resulting reduced quality of life, increasing the meniscal injury risk, knee instability and early degenerative joint disease. Bone-Patellar Tendon-Bone (BPTB) became the gold standard surgery where conservative management failed. Adding interferential screw provides rigid fixation which is important for early accelerated rehabilitation program in athletes but we have carried out this prospective interventional study in Bangabandhu Sheikh Mujib Medical University (BSMMU) and our private settings from January 2007 to December 2011 to as

Hollow-bone-graft dynamic hip screw can fix and promote bone union after femoral neck fracture: an experimental research.

BACKGROUND: Delayed bone union, nonunion or osteonecrosis often occur after femoral neck fractures in young adults. Secondary bone healing requires strong internal fixation, intramedullary pressure reduction and early functional exercise. OBJECTIVE: To compare bone healing of femoral neck fractures treated with hollow-bone-graft dynamic hip screws (Hb-DHS) and standard dynamic hip screws (DHS) in an animal model. DESIGN: Testing of specifically designed fixation devices in a pig animal model. INTERVENTIONS/METHODS: We designed Hb-DHS and DHS devices appropriate to the femoral neck and head

Bioabsorbable screw divergence angle, not tunnel preparation method influences soft tissue tendon graft-bone tunnel fixation in healthy bone.

Bone tunnel fixation of a soft tissue tendon graft is the weak link immediately following ACL reconstruction. This biomechanical study evaluated the influence of extraction drilled or step dilated bone tunnels and bioabsorbable screw divergence on soft tissue tendon graft fixation. From an initial group of 50 available specimens, similar apparent bone mineral density porcine tibiae (1.2 +/- 0.24 g/cm2) were divided into two groups of ten specimens each. Group 1 (extraction drilled) received 9 mm diameter tunnels. Group 2 (step dilated) received 7 mm diameter tunnels that were dilated to 9 mm.

Screw-bone interface loads from rod bending: Development of an instrumented spinal fusion simulator.

During spinal fusion procedures, spinal rods are bent to match the current and desired curvature of a spinal segment. Following bending, the rods are attached to the heads of pedicle screws that have been driven into the vertebral pedicles. A mismatch between the curvature of the spine and the curvature of the rod can lead to increased loads at the screw-bone interfaces. These increased loads have been associated with screw loosening and pull-out. We have developed a novel and inexpensive spinal fusion simulation device to explore how variables such as surgical techniques, tools, and hardware

Trapeziometacarpal Joint Arthrodesis Using a Locking Plate and Headless Compression Screw With an Autogenous Bone Graft.

PURPOSE: Trapeziometacarpal (TMC) joint arthrodesis is an established surgical option to manage TMC joint arthritis. However, nonunion remains a complication after arthrodesis. In this study, we aimed to evaluate the outcomes of using a locking plate and headless compression screw with autogenous bone grafting. METHODS: We identified 21 thumbs that underwent TMC arthrodesis using this procedure. Radiographs and computed tomography scans were obtained to define bone union. Furthermore, we asked patients whether they had returned to their original work at every follow-up visit. To assess object

Treatment of scaphoid fractures and pseudarthroses with the human allogeneic cortical bone screw. A multicentric retrospective study.

BACKGROUND: Allograft bone screws are rarely described for the fixation of the scaphoid. When fresh fractures are treated, metal screws are mainly used; when pseudarthrosis is the indication, plates in combination with vascularized or non-vascularized bone grafts are mainly used. The necessity of metallic screw removal is under debate, but it is mandatory for plates because of movement restrictions due to the plate. The use of biomaterials in scaphoid fracture fixation was described as leading to union rates of between 64 and 100%. Brcic showed the incorporation of an allogeneic cortical bone …

Comparison of the fixation ability between lag screw and bone plate for oblique metacarpal shaft fracture.

BACKGROUND: For oblique metacarpal shaft fracture, if anatomical reduction is achieved through conservative cast immobilization rather than stable fixation, bone malrotation can easily occur, resulting in severe loss in hand prehensile function. However, whether bone plate fixation or only lag screw fixation is more preferable remains unclear. Few studies have evaluated whether screw fixation can provide biomechanical fixation strength similar to bone plate fixation. OBJECTIVE: We assessed the difference in fixation strength between fixtation with two lag screws and bone plate for oblique met

The clinical outcomes of scaphoid nonunion treated with a precisely processed autologous bone screw: a case series.

INTRODUCTION: Metal implants and bioabsorbable implants are frequently used in orthopaedic surgery, but they have some disadvantages. The usefulness of autologous bone has been described, and a method to precisely process autologous bone into implants such as screws and apply the implants clinically has been desired. We created a new system for manufacturing autologous bone screws during surgery and report five cases of scaphoid nonunion treated with precise autologous bone screws made from the tibial cortex using the new system. PATIENTS AND METHODS: From 2012 through 2017, seven patients we…

Effect of the positional relationship between the interference screw and the tendon graft in the bone tunnel in ligament reconstruction.

PURPOSE: To reveal the effects of the positional and length relationships between the interference screws (ISs) and the tendon graft in the bone tunnel on the fixation strength in ligament reconstruction. METHODS: We compared three IS positions on the anterior (the Anterior group) or posterior (the Posterior group) or side (the Side group) of the tendon graft in relation to the pullout direction. The tendon graft was pulled at 0°, 30°, 60°, and 90° to the bone tunnel, and the maximum pullout load at each angle was compared among the groups. We also investigated the relationship between the le

Micro Screw Fixation for Small Proximal Pole Scaphoid Fractures with Distal Radius Bone Graft.

Background  Achieving adequate fixation and healing of small proximal pole acute scaphoid fractures can be surgically challenging due to both fragment size and tenuous vascularity. Purpose  The purpose of this study was to demonstrate that this injury can be managed successfully with osteosynthesis using a "micro" small diameter compression screw with distal radius bone graft with leading and trailing screw threads less than 2.8 mm. Patients and Methods  Patients with proximal pole scaphoid fragments comprising less than 20% of the entire scaphoid were included. Fixation was accomplished from

Non-vascularized bone graft with Herbert-type screw fixation for proximal pole scaphoid nonunion.

BACKGROUND: Since proximal pole fractures of the scaphoid are frequently overlooked, the poor vascularity in the proximal pole fragment often leads to nonunion. Vascularized bone grafts have been recently tested in cases with scaphoid proximal pole nonunion, but the indication for this treatment has not been well established. Alternatively, we have been treating such patients with a non-vascularized iliac bone graft and Herbert-type screw fixation with considerable success. The purpose of this investigation is to evaluate these cases retrospectively and clarify the surgical efficacy of our pro
