Outcome of Total Hip Replacement with Ceramic on Ceramic Bearings, Case series from Aga Khan University Hospital, Karachi, Pakistan.

Total Hip replacement offers complete resolution of symptoms and restoration of the hip function. The long-term success of hip replacement primarily depends on wear and osteolysis resulting from the bearing surfaces used in hip replacement surgery. Different bearing surfaces have been used in hip replacements, with variable success rates. Traditional combinations include metal on polyethylene, ceramic on polyethylene, and metal on metal articulations. Ceramic on ceramic Bearing couple is a relatively newer combination recommended for younger patients, requiring Total hip Arthroplasty. Ceramics

The reasons for ceramic-on-ceramic revisions between the third- and fourth-generation bearings in total hip arthroplasty from multicentric registry data.

This study aimed to evaluate (1) the overall reasons for first revision in CoC THAs; (2) whether the reasons for revision differ between third-generation and fourth-generation CoC THAs; and (3) the specific factors associated with bearing-related problems as the reason for revision. We retrospectively reviewed 2045 patients (2194 hips) who underwent first revision THA between 2004 and 2013, among which 146 hips with CoC bearings underwent revision. There were 92 hips with third-generation ceramic bearings and 54 hips with fourth-generation ceramic bearings. The major reasons for CoC THA revisi

Large head ceramic-on-ceramic bearing in primary total hip arthroplasty: average 3-year follow-up of a multicentre study.

BACKGROUND: Surgeons are increasingly using larger femoral head sizes in total hip arthroplasty (THA) to improve stability and reduce the rate of dislocation, 1 of the leading causes of revision surgery. Large ceramic head sizes up to 48 mm can now be used with monoblock acetabular components. National registries have shown promising results at short-term with large diameter ceramic-on-ceramic THA, with very low revision rates. This study reports on the average 3-year follow up of a press-fit monoblock large diameter acetabular shell with a pre-assembled ceramic liner, with emphasis on the radi

Ceramic-Ceramic Hip Arthroplasty for Osteonecrosis: Average 5-year Follow-up in Patients Less Than 50 Years of Age.

INTRODUCTION: The optimal bearing surface for the young, active patient undergoing total hip arthroplasty (THA) is unknown. We sought to examine the clinical results of ceramic-on-ceramic (CoC) THA in patients under 50 years of age with a diagnosis of osteonecrosis. MATERIALS AND METHODS: Fifty-three hips underwent cementless THA with a CoC articulation. Clinical and functional outcomes, including preoperative and postoperative WOMAC and UCLA scores, were recorded. Serial radiographs were studied for component loosening or evidence of osteolysis. Revision surgery for any reason was the primar

Effect of a crown ferrule on the fracture strength of endodontically treated canines restored with fiber posts and metal-ceramic or all-ceramic crowns.

The aim of this study was to comparatively evaluate the fracture strength of endodontically treated canines restored with glass-fiber posts (GFPs) and either metal-ceramic (MC) or all-ceramic (AC) crowns in the presence or absence of 2 mm of ferrule height. Fifty human maxillary canines were endodontically treated and randomly divided into five groups of 10 specimens each. The first group remained intact (control), while the remainder were restored with GFPs and composite cores with either MC or AC crowns. Each of the AC and MC groups was equally divided between teeth with or without ferrule.

Uncemented third-generation ceramic-on-ceramic total hip arthroplasty using metal acetabular shell with direct taper locking liner.

BACKGROUND: The ceramic-on-ceramic (CoC)-bearing couple in total hip arthroplasty (THA) was developed to reduce the wear debris and osteolysis. Although the mechanical strength of third-generation ceramic has improved over previous generations, the risk of osteolysis and ceramic fracture is still an important concern. METHODS: We studied 124 uncemented THAs with third-generation CoC-bearing couple implanted between 2000 and 2004. The ceramic liner and head were secured with the direct taper locking mechanism. One hundred and eleven hips were followed-up for minimum of 8 years, with an average

Wear of 36-mm BIOLOX(R) delta ceramic-on-ceramic bearing in total hip replacements under edge loading conditions.

Ceramic-on-ceramic bearings have become of great interest due to the substantial improvements in the manufacturing techniques and material properties and due to polyethylene wear debris-induced osteolysis and the issues with metal wear debris and ion release by metal-on-metal bearings. Edge loading conditions due to translational malpositioning (microseparation conditions) have been shown to replicate clinically relevant wear mechanisms and increase the wear of ceramic-on-ceramic bearings; thus, it was necessary to test new bearing materials and designs under these adverse conditions. The aim

Fourth-generation ceramic-on-ceramic total hip arthroplasty in patients of 55 years or younger: short-term results and complications analysis.

BACKGROUND: The incidence of total hip replacement in the younger and more active patients is ever increasing. The ceramic-on-ceramic (COC) bearing was developed to reduce wear debris-induced osteolysis and loosening and to improve the longevity of hip arthroplasties. Few studies have reported the clinical results and complications of the new zirconia-toughened ceramic total hip arthroplasty (THA). METHODS: A consecutive series of 132 young patients (177 hips) that underwent primary cementless THAs between January 2010 and December 2012 were included in this study. These arthroplasties all ha

Serum ion levels after ceramic-on-ceramic and metal-on-metal total hip arthroplasty: 8-year minimum follow-up.

Alternative bearing surfaces for total hip arthroplasty, such as metal-on-metal and ceramic-on-ceramic, offer the potential to reduce mechanical wear and osteolysis. In the short and medium term, the second generation of metal-on-metal bearings demonstrated high systemic metal ion levels, whereas ceramic-on-ceramic bearings showed the lowest ones. We aimed to verify whether the long-term ion release in metal-on-metal subjects was still relevant at a median 10-year follow-up, and whether a fretting process at the modular junctions occurred in ceramic-on-ceramic patients and induced an ion disse

How have alternative bearings (such as metal-on-metal, highly cross-linked polyethylene, and ceramic-on-ceramic) affected the prevention and treatment of osteolysis?

Osteolysis is a multifactorial process dependent on surgical technique, implant design, patient factors, and material composition. Alternative bearing surfaces, such as highly cross-linked polyethylene, ceramic-on-ceramic, and metal-on-metal articular surfaces, have been introduced in an attempt to reduce wear and osteolysis following total hip arthroplasty. Intermediate-term follow-up data available suggest that the prevalence and severity of osteolysis may be reduced with these materials compared with conventional metal-on-polyethylene bearing surface couples. However, long-term data are pre

[Exposure to ceramic fibers in the work environment. II. Occupational exposure to dust in plants producing ceramic fiber; fibrogenic effect of the fibers].

The aim of the work was to assess the exposure to dust containing ceramic fibres among workers employed in the plant producing aluminosilicate fibres, and to investigate fibrogenic properties of produced ceramic aluminosilicate fibres (L-1). The studies carried out in the work environment revealed that workers were exposed to dust containing respirable fibres from materials produced (fibres, mats). Mean concentrations of these fibres ranged from 0.07 to 0.37 f/cm3 and they were considerably lower than MAC value (1 f/cm3) proposed for respirable ceramic fibres. Mean total dust concentrations ra

Two-Stage Method of Manufacturing Passive Conducting Structures by Laser Decomposition of AlN Ceramics.

This paper presents a two-stage method for fabricating passive conductive structures of arbitrary geometry on aluminum nitride (AlN) ceramic substrates via laser decomposition using nanosecond pulses of Nd:YAG fiber laser (λ = 1062 nm). We developed a two-stage process which allows one to create elements of low resistance and produce permanent joints to the obtained structures. Studies of laser decomposition of AlN under variable process conditions have shown that effective decomposition of the AlN ceramic surface layer should be carried out in an atmosphere of argon with laser beam fluence we
