Design, Implementation, and Evolution of the Medicaid Outcomes Distributed Research Network (MODRN).

BACKGROUND: In the US, Medicaid covers over 80 million Americans. Comparing access, quality, and costs across Medicaid programs can provide policymakers with much-needed information. As each Medicaid agency collects its member data, multiple barriers prevent sharing Medicaid data between states. To address this gap, the Medicaid Outcomes Distributed Research Network (MODRN) developed a research network of states to conduct rapid multi-state analyses without sharing individual-level data across states. OBJECTIVE: To describe goals, design, implementation, and evolution of MODRN to inform other

Cancer specificity of promoters of the genes controlling cell proliferation.

Violation of proliferation control is a common feature of cancer cells. We put forward the hypothesis that promoters of genes involved in the control of cell proliferation should possess intrinsic cancer specific activity. We cloned promoter regions of CDC6, POLD1, CKS1B, MCM2, and PLK1 genes into pGL3 reporter vector and studied their ability to drive heterologous gene expression in transfected cancer cells of different origin and in normal human fibroblasts. Each promoter was cloned in short (335-800 bp) and long (up to 2.3 kb) variants to cover probable location of core and whole promoter r

Frequencies of morphological characteristics in two contemporary forensic collections: implications for identification.

Positive identification relies on comparison of antemortem and postmortem data. Some identifications are based on morphological features such as fracture, pathological condition, and surgical hardware, despite little literature indicating the frequencies of such traits. This study examines whether such features are sufficiently rare as to be deemed individualizing. Data were collected on two modern North American skeletal collections (N=482 individuals). Presence/absence of features was scored by skeletal element and side. Results indicate that frequencies vary by geographic region (higher fre

Influence of Vamp Opening Configuration on Foot-Footwear Biomechanics: A Finite Element Analysis of Women's Low-Heeled Court Shoes.

Footwear geometry plays an important role in determining mechanical performance, plantar load distribution, comfort, and foot stability during gait. Understanding how constructive design parameters influence foot biomechanics is essential for developing footwear that improves comfort while reducing excessive mechanical loading. Although various finite element studies have investigated insole and outsole design, midsole materials, and plantar pressure redistribution, comparatively little attention has been paid to the influence of upper construction parameters, particularly vamp opening configu

Dos and don'ts in designing a computerized oral and lip squamous cell cancer registry.

BACKGROUND: In the last ten years, many countries have started to develop constructive systems for registering common diseases and cancers. In this research, we intended to determine and identify the minimum data set (MDS) required for the design of the oral and lip squamous cell cancer registration system in Iran. METHODS AND MATERIAL: At first, primary information elements related to disease registries were extracted using scientific papers published in reliable databases. After reviewing the books, related main guidelines, and 42 valid articles, the initial draft of a researcher-made quest

Novel use of a hash-based tokenization for sharing sickle cell disease data without sharing protected health information.

Efforts to improve care for people living with sickle cell disease (SCD) have led to the development of several registries; however, many are dependent on time-limited funding and lack coordination. Consequently, existing data sets are fragmented and do not provide the comprehensive, longitudinal insights achievable through well-integrated registries. Although relevant data exist within electronic medical records across institutions, aggregation is limited by poor interoperability, inconsistent use of common data elements, and poor translation of natural language into codified data. These barr

Bioinformatics pipeline to guide post-GWAS studies in Alzheimer's: A new catalogue of disease candidate short structural variants.

BACKGROUND: Short structural variants (SSVs), including insertions/deletions (indels), are common in the human genome and impact disease risk. The role of SSVs in late-onset Alzheimer's disease (LOAD) has been understudied. In this study, we developed a bioinformatics pipeline of SSVs within LOAD-genome-wide association study (GWAS) regions to prioritize regulatory SSVs based on the strength of their predicted effect on transcription factor (TF) binding sites. METHODS: The pipeline utilized publicly available functional genomics data sources including candidate cis-regulatory elements (cCREs)

NIH Music-Based Intervention Toolkit: Music-Based Interventions for Brain Disorders of Aging.

Music-based interventions (MBIs) show promise for managing symptoms of various brain disorders. To fully realize the potential of MBIs and dispel the outdated misconception that MBIs are rooted in soft science, the NIH is promoting rigorously designed, well-powered MBI clinical trials. The pressing need of guidelines for scientifically rigorous studies with enhanced data collection brought together the Renée Fleming Foundation, the Foundation for the NIH, the Trans-NIH Music and Health Working Group, and an interdisciplinary scientific expert panel to create the NIH MBI Toolkit for research on

Processing number and length in the parietal cortex: Sharing resources, not a common code.

A current intense discussion in numerical cognition concerns the relationship between the processing of numerosity and other non-numerical quantities. In particular, it is a matter of debate whether number and other quantities (e.g., size, length) are represented separately in the brain or whether they share a common generalized magnitude representation. We acquired high-resolution functional MRI data while adult subjects engaged in a magnitude comparison task involving either numerosity (i.e., which of the two sets has more elements?) or line length (i.e., which of the two lines is longer?).

Evaluation of essential trace and toxic elements in biological samples of normal and night blindness children of age groups 3-7 and 8-12 years.

The most common cause of blindness in developing countries is vitamin A deficiency. The World Health Organization estimates 13.8 million children to have some degree of visual loss related to vitamin A deficiency. The causes of night blindness in children are multifactorial, and particular consideration has been given to childhood nutritional deficiency, which is the most common problem found in underdeveloped countries. Such deficiency can result in physiological and pathological processes that in turn influence biological samples composition. Vitamin and mineral deficiency prevents more than

Structural variants linked to Alzheimer's Disease and other common age-related clinical and neuropathologic traits.

Advances have led to a greater understanding of the genetics of Alzheimer's Disease (AD). However, the gap between the predicted and observed genetic heritability estimates when using single nucleotide polymorphisms (SNPs) and small indel data remains. Large genomic rearrangements, known as structural variants (SVs), have the potential to account for this missing genetic heritability. By leveraging data from two ongoing cohort studies of aging and dementia, the Religious Orders Study and Rush Memory and Aging Project (ROS/MAP), we performed genome-wide association analysis testing around 20,00

Clinical challenges in managing bronchiectasis.

Bronchiectasis is a common disease in the Asia-Pacific and affected patients suffer from chronic sputum production and recurrent exacerbations. Bronchiectasis is largely idiopathic although there is diverse aetiology. The pathogenesis of bronchiectasis comprises infective, inflammatory and emzymetic elements. These interact to perpetuate continued airway damage in bronchiectasis leading to progressive airway and lung damages. Treatment of bronchiectasis is unsatisfactory and there are only very few trials. Existing data suggest some efficacy of inhaled corticosteroid therapy, which has been sh

How recovery definitions vary by service use pathway: Findings from a national survey of adults.

OBJECTIVE: How people define recovery may affect their recovery goals, service use, and ultimately their outcomes. We examined recovery definitions among adults in recovery from an alcohol use disorder (AUD) who had different service use histories. METHOD: We analyzed online survey data from 1,492 adults with resolved lifetime AUD in "treated recovery" (any use of specialty services, such as inpatient or outpatient rehabilitation; n = 375), "assisted recovery" (any use of lay services, such as mutual-help groups, and no use of specialty services; n = 174), or "independent recovery" (no use of

Bioinorganic chemistry of open-angle glaucoma: A review.

At present, physical methods of chemical analysis are constantly improving providing large amount of data on elemental composition of organs and tissues. However, only few works describe the correlation (or the potential connection) between the general or local bioelemental imbalances and specific biochemical reactions that are involved in pathogenesis of certain diseases. This review describes primary open-angle glaucoma (POAG) - one of the most common ophthalmic diseases - in terms of elemental chemistry. The authors look into the impact that various subgroups of elements have on passive and

The Epstein-Barr virus nuclear protein 1 promoter active in type I latency is autoregulated.

The only member of the Epstein-Barr virus family of nuclear proteins (EBNAs) expressed during type I and type II latent infections is EBNA-1. This is in contrast to type III latency, during which all six nuclear proteins are expressed from a common transcription unit. The exclusive expression of EBNA-1 during type I and II latency is mediated through a recently identified promoter, Fp. The objective of this study was to characterize Fp in the Burkitt lymphoma cell background, where it is known to be differentially utilized. Using a short-term transfection assay and reporter gene plasmids conta

Effect of different subtalar joint arthroereisis implants on pediatric flexible flatfoot: a finite element analysis.

BACKGROUND: Flexible flat foot is a relatively common disease treated by foot and ankle surgery, and subtalar arthroereisis (STA) has gradually become favored by foot and ankle surgeons due to its advantages. However, which sinus tarsi implant is optimal remains controversial. The purpose of this study was to compare the biomechanical outcomes of two types sinus tarsi implants of different sizes via finite element analysis. METHODS: A finite element model of pediatric flexible flatfoot deformity was established in which virtual surgery of STA was simulated. Boundary and loading conditions wer

Association between dietary iron intake and pelvic inflammatory disease in women from the US: Findings from the 2013 to 2020 NHANES dataset.

Pelvic inflammatory disease (PID), a prevalent gynecological ailment compromising women's health, demonstrates established risk mitigation from dietary trace minerals. However, iron's role in PID pathogenesis remains unestablished. The National Health and Nutrition Examination Survey collected data on dietary iron intake and PID through dietary intake questionnaires and reproductive health questionnaires. We utilized data from 2013 to 2020 and employed multiple logistic regression to explore this relationship. This was further enhanced by smooth curve fitting and threshold analysis to determin
