Selective chemical vestibulectomy.

In an attempt to destroy selectively the affected peripheral vestibular labyrinth in patients with intractable vertigo as a result of Meniere's disease, a known quantity of streptomycin was introduced within the bony labyrinth following fenestration of the horizontal semicircular canal. Initial laboratory results in animal models proved that near total ablation of the vestibular end-organs (including the hair cell population of the cristae ampularis, utricle, and to some extent the saccule) could be achieved chemically without involvement of the cochlear hair cell population. The exact dose re

Stapes mobilization for otosclerotic deafness; the monitored peribasal technique.

Direct surgical operation on the stapes footplate region in the treatment of hearing loss due to otosclerosis has been revived. In the last 100 cases of a total series of 600, the peribasal stapes mobilization (stapedolysis) technique was completely successful in 62 per cent, partially successful in 29 per cent and a failure in 9 per cent. Stapes mobilization requires precise microsurgical technique monitored by audiometry during the surgical procedure. It appears that mobilization is the preferred approach to the surgical treatment of otosclerosis, but fenestration of the vestibular labyrinth

[Juliusz Lempert (1890-1959): the author of the fenestration technique].

Juliusz Lempert was born in 1890 in Poland. A few years later the poor Jewish family emigrated to the United States. Lempert obtained his MD degree at Long Island Medical College, and soon after that established a small hospital of his own in Manhattan. Later he bought an old five-storey building and converted it into a new otological medical center, which he called Endaural Hospital. His life was rather difficult with the wave of antisemitism in the United States and the adversity which he encountered so many times. However his contribution to the microsurgical treatment of conductive deafnes

Large versus small fenestration technique in stapedectomy. A comparative investigation of House and Fisch prostheses in stapedectomy.

Sixty patients with otosclerosis have been operated on by 2 different techniques using either a small fenestra or a large fenestra. The hearing improvement by using a Fisch prosthesis (small fenestra) was significantly better in the high frequency range compared to results obtained by using a House prosthesis on a vein (large fenestra). Due to significantly better hearing in the higher frequencies the Fisch prosthesis is recommended. In technically difficult cases of stapedectomy, where extraction of the footplate is difficult or impossible the Fisch prosthesis can be valuable.

A new kind of labyrinth-like capillary is responsible for leakage from human choroidal neovascular endothelium, as investigated by high-resolution electron microscopy.

PURPOSE: This study reports the clinicopathologic findings of leaky sites in pathological vessels after submacular removal of choroidal neovascular membranes (CNV). As the site that causes fluid exudation from neovascular vessels is unknown, specific attention was focused on the formation of fenestrations, cellular junctions, and morphologic alteration which can cause endothelial leakage. METHODS: Choroidal neovascular membranes of 15 patients who underwent submacular surgery for CNV were investigated. Five patients received bevacizumab treatment before surgery, and another five received phot

The fenestration operation of Lempert: a historical perspective.

OBJECTIVE: To describe the events surrounding the introduction of the one-stage fenestration operation for otosclerosis and the presentation of the paper given to the American Otological Society in 1938. DATA SOURCES: Published data in scientific journals and news media, archival material from the New York Academy of Medicine and the John Q. Adams Center of The American Academy of Otolaryngology-Head and Neck Surgery, and a personal interview with the late Gordon D. Hoople, M.D. CONCLUSION: This article describes Dr. Gordon Hoople's exclusive account of the events surrounding the presentatio
