X-Stop: foot drop.

BACKGROUND CONTEXT: The X-Stop (Kyphon, Sunnyvale, CA), a device implanted between adjacent lumbar spinous processes at one or two levels particularly in the geriatric population, theoretically decompresses the spinal canal by reversing the lordosis. PURPOSE: The X-Stop relieves the symptoms of lumbar stenosis. STUDY DESIGN/SETTING: A case report from the United States. PATIENT SAMPLE: A case report focusing on how X-Stop placement resulted in a bilateral foot drop in an 84-year-old patient, which was relieved 9 months later with laminectomy. OUTCOME MEASURES: The patient's outcome was bas…

[Inserts for foot deformities].

Inserts are orthopedic aids in the treatment of foot disorders that result from changes of the static or dynamic situation. Provision of appropriate orthopedic devices can relieve the pain caused by forefoot deformities either in lieu of surgical intervention or in rare cases also following surgical treatment to improve the symptoms of residual pain.Available materials provide support, padding, and cushioning. Inserts are custom-made to measure and/or based on a plaster impression. Determining the indication, prescribing the inlay, and checking the orthosis are the tasks of the physician. One …

Foot and ankle biomechanics.

With the advent of imaging advancements there has been renewed interest in the foot and ankle. However, many of the basic functions and biomechanical considerations of the bones, joints, and specialized tissues of the normal and dysfunctional foot and ankle remain unfamiliar to many radiologists. This article focuses on the basic biomechanics, normal alignment, and common alignment disorders of the foot and ankle that are relevant to radiologists.

Common pediatric foot dermatoses.

The pediatric foot dermatoses known as tinea pedis, shoe dermatitis, and juvenile plantar dermatosis are seen commonly in clinical practice. A knowledge of the clinical presentations and causes of these disorders can help pediatric and family nurse practitioners identify, diagnose, and treat childhood rashes of the feet. The conventional view in pediatrics is that small children rarely have tinea pedis. However, a 1992 study disputes this claim. Any child who has an atypical or persistent foot rash will need a referral or consultation.

Common painful foot syndromes.

The prevalence of foot problems in the general population is 10%, and in the elderly it ranges from 53% to 95%. Proximal plantar fasciitis is the most common cause of painful feet in clinical practice, and is twice as common among women as among men. Metatarsalgia is probably the most common cause of foot pain among middle-aged women.

[Hollow foot in adults].

Contrary to the general impression, the hollow foot is much more common than the flat foot. We distinguish three categories of hollow foot: varus hollow (clinical form I), valgus hollow (clinical form II), and pseudo-hollow (clinical form III). These three clinical forms have a common deviation dynamics, the consequence of shortening of the sural-Achilles-calcaneoplantar system. Metatarsalgia often precedes deformation of the front of the foot. Talalgia is linked to excess traction or to a conflict between the heel and the back of the shoe. Common therapy consists of plantar orthoses, eventual…

Unilateral cleft foot deformity.

A 17-year-old girl with unilateral atypical cleft foot deformity is reported. In this case the deformity was associated with deafness, mandibular retrognathia and congenital bilateral temporomandibular joint luxation. Family history of cleft foot deformity was not found. A syndactylization procedure with shortening of the big toe was done. The temporomandibular joint luxation was also corrected.

Acute foot compartment syndromes.

Twelve cases of compartment syndrome of the foot in 10 patients were retrospectively reviewed. All were high-energy injuries sustained in a fall from a height (six), crush (three), or motor vehicle accident (three). Bone injuries of the foot included five calcaneal fractures, three multiple metatarsal and/or phalangeal fractures, and two Lisfranc fracture-dislocations with multiple metatarsal neck fractures. The most consistent physical finding was tense swelling of the foot. The diagnosis was confirmed with compartmental pressure measurements in all cases. Decompressive fasciotomies were adeq…

Pseudoaneurysm after foot surgery.

Pseudoaneurysm (PA) is recognized as a rare complication after pediatric foot surgery. We identified the incidence, pertinent clinical features, and response to surgical intervention in PA as a complication of foot surgery. Four PAs were identified after 2,756 foot operations, an overall incidence of 0.14%. These patients typically had symptoms between 2 and 3 months after index operation with an enlarging, pulsatile, compressible mass in the plantar medial aspect of the foot. Arteriography was helpful in planning surgical intervention. Operative treatment consisted of ligation and excision of…
