OBJECTIVE: The results of mitral valve repair for anterior leaflet prolapse have been less gratifying than those reported for posterior leaflet prolapse. We compared the long-term durability of 2 different surgical techniques: the edge-to-edge repair, which is used for the treatment of anterior leaflet prolapse, and quadrangular resection, which has been adopted for correction of posterior leaflet prolapse.
METHODS: From 1991 through April 2004, 133 patients with anterior leaflet prolapse and 605 with posterior leaflet prolapse caused by degenerative mitral disease underwent valve repair. The

Valvular heart disease is among the most common cardiovascular conditions, usually presenting as aortic stenosis or mitral regurgitation. Clinically significant structural and functional anomalies can arise through congenital malformation, genetic variants, cardiovascular dysfunction, infectious disease sequelae, or chronic inflammation that can progress over time, necessitating surgical replacement or repair. The most common types of valve disease requiring surgical intervention are mitral valve prolapse, often with myxomatous changes due to congenital malformation or infectious disease, and

The work discusses the results of the examination of 108 patients with the syndrome of mitral valve prolapse encountered in the presence of various diseases (cardiomyopathy, acquired heart diseases, ischemic heart disease, pericarditis, etc.). The methods for the diagnosis of this syndrome are discussed: electro- phono-, echocardiography, radiography of the heart, and ventriculography. The clinical manifestations of the syndrome are analyzed.

Osteogenesis imperfecta is a disorder of the connective tissue that affects several structures including heart valves. However, cardiac surgery is associated with high mortality and morbidity rates. In a 48-year-old man with osteogenesis imperfecta and mitral valve prolapse, we performed the first successful mitral valve repair by right anterior mini-thoracotomy. At the 1-year follow-up, he was asymptomatic and echocardiography confirmed the initial success.

UNLABELLED: We analysed the clinical data of 206 patients (pts) with sick sinus syndrome (group S). All the population was divided into two group; pts with normal (104 pts, group S1) and abnormal (102 pts, group S2) "intrinsic" properties of sinus node (SN). Intrinsic properties was defined as normal if intrinsic heart rate was normal and corrected sinus node recovery time after pharmacological heart denervation (propranolol 0.2 mg/kg body weight, atropine 0.04 mg/kg body weight) was no longer than 422 ms. In whole group S, in 131 pts we found organic heart disease (coronary heart disease, n =

BACKGROUND: Repair of anterior mitral leaflet (AML) prolapse is still a technical challenge for cardiac surgeons. It is an important issue to find a way to repair the AML prolapse with a reliable and reproducible technique.
METHODS: Between January 2002 and June 2009, the operation of chordal transfer based on the "edge-to-edge" technique was performed in 21 patients with serious mitral valve regurgitation because of prolapse of the anterior leaflet. After the operation, echocardiography was performed in each patient before discharge and at the time of follow-up.
RESULTS: All patients surviv

A 61-year-old man with diabetes mellitus and chronic bronchitis was brought to the hospital after falling from a roof accidentally. He received blunt trauma to the left chest wall including left rib fractures, pneumothorax, hemothorax and a fracture of left scapula. After endotracheal intubation and chest drainage, he was transferred to the intensive care unit. Sixteen days later, congestive heart failure appeared. Echocardiography showed a severe aortic valve regurgitation with a prolapse of non-coronary cusp. Eight months after the injury, his heart failure deteriorated. He underwent aortic

Of 113 patients who were investigated by echocardiography with regard to mitral prolapse, simultaneous tricuspidal prolapse was detected in 13.3% (15 patients), isolated tricuspidal prolapse (TP) was detected in one patient. Incomplete right bundle branch (RBB) was detected in 11 TP patients, complete RBB in one that might be associated with the involvement in the degenerative process of the proximal part of the right bundle branch in the zone adjoining the tricuspid valve. The characteristic feature of MP and TP was the presence of two systolic clicks.

The authors present investigations of the prolapse of the mitral valves in dogs. In 18 cases there were dogs with a prolapse, in 6 dogs there was a prolapse of the tricuspid valves, and in 3 cases a prolapse of both valves was observed. In all cases prolapse occurred together with endocardiosis of the valves or as a consequence of it.

Background: Mitral valve prolapse is a common valvular heart disorder, usually associated with a benign prognosis in the absence of significant mitral regurgitation. However, a subset of patients is at increased risk for complex ventricular arrhythmias and sudden cardiac death. Identifying these high-risk individuals remains a major clinical challenge. Case Summary: We present the case of a 71-year-old female patient with recurrent syncopal episodes, a strong family history of sudden cardiac death, and complex ventricular ectopy. Multimodality imaging revealed bileaflet mitral valve prolapse,

The aortic valve prolapse which can be diagnosed by echocardiography is usually found in patients with the floppy valve syndrome. The clinical symptoms are identical with those of the mitral valve prolapse. The frequency of aortic valve prolapse in 67 cases of mitral valve prolapse was 8.9%, the frequency of aortic insufficiency 5.9%. A main etiological factor of aortic valve prolapse is a myxomatous degeneration of valvular tissue which can affect one or more cardiac valves.

The hearts of three children who died with Duchenne's progressive muscular dystrophy and features of mitral valve prolapse were examined to find if the valve disorder arose from cardiomyopathy due to the primary disease or from dystrophic changes in th mitral valve itself. Gross, histologic and ultrastructural features of mitral valve annulus, anterior and posterior leaflets, chordae tendineae, right and left ventricles, and anterior and posterior papillary muscles were compared with those of similar tissues from normal children of matched age and sex. A 56-year-old woman was admitted to our department because of congestive heart failure. Chest X-ray showed dextrocardia with situs inversus. The echocardiography revealed severe mitral regurgitation due to prolapse of posterior leaflet and secondary Venography demonstrated the bilateral superior vena cava, huge coronary sinus and hemiazygos continuation with hypoplastic inferior vena cava. Mitral valve regurgitation was repaired by Carpentier's and Burr's technique, and tricuspid valve regurgitation was repaired by Kay's technique. Her postoperative course was uneventful. Sepsis and tricuspid valve regurgitation developed in a 29-year-old man with Crohn's disease after implantation of an intravenous port device for parenteral nutrition. A thrombus, caused by Staphylococcus epidermidis, had grown along the catheter and prolapsed through the valve without affecting it. Complete removal in an open heart procedure led to quick recovery.

We successfully treated a patient with accelerated aortic regurgitation due to localized aortic dissection with mitral regurgitation causing congestive heart failure. A 58-year-old female, who had suffered from aortic regurgitation for more than 10 years, had acute heart failure due to acceleration of aortic regurgitation. The surgical findings showed prolapse of the aortic valve due to localized dissection in the sinus of Valsalva. We performed a Bentall operation and a mitral valve replacement, with a favorable outcome. The postoperative course was uneventful.

A 64-year-old woman with no significant past medical history presented with heart failure symptoms secondary to severe mitral regurgitation. She was referred for mitral valve repair with the intention of valve preservation. Initial transthoracic echocardiography (TTE) suggested that the regurgitation was due to a ruptured chordae resulting in prolapse of the A2 segment. Intraoperative inspection following atriotomy revealed an aberrant chordae tendineae inserting anomalously into the left atrial wall, which was contributing to the A2 prolapse. The aberrant chordae was excised and replaced with

A 69-year-old male presented with obstructive hypertrophic cardiomyopathy, mitral valve regurgitation, and myxomatous mitral valve prolapse. A spontaneous chordal rupture and acute severe mitral regurgitation resulted in abrupt clinical deterioration despite complete relief of severe left ventricular outflow tract obstruction and systolic anterior motion of the anterior mitral leaflet. The patient underwent extensive cardiac surgery due to intractable heart failure. Surgical procedures included a mitral valve replacement, a septal myectomy, and the Maze procedure.

Mitral valve prolapse frequently resembles coronary heart disease. Retrospective clinical, ECG and angiographic analysis of 100 consecutive patients with mitral prolapse and normal coronary arteries, but complaining of anginal pain, shows how difficult it is to establish the correct diagnosis. When resting, 44% of patients have nonspecific ECG disorders of repolarization phase. During periods of chest pain 3 patients experienced transient ST segment changes very similar to acute myocardial ischemia. The exercise test was positive in 39% of cases, and in 2 patients during exercise a sudden drop

This video demonstrates aortic valve repair during ascending aneurysm replacement for a 71-year-old man with congestive heart failure, grade 3 aortic insufficiency, and a 5.4-cm ascending aneurysm. On testing, the noncoronary leaflet is prolapsing, the annulus is 27 mm, and the leaflets size to a 21-mm ring. The ring is sutured beneath the aortic valve annulus with nine horizontal mattress sutures. The noncoronary leaflet is plicated, correcting the prolapse. A 28-mm Dacron tube graft (Maquet Hemashield, Baden-Württemberg, Germany) is sutured to the supracoronary and distal aorta. After repair
