Hormonal evaluation of the infertile man can help to identify a treatable abnormality or to suggest, with a minimum of patient discomfort, that the infertility is not treatable. The need for testosterone replacement can also be determined. Impaired spermatogenesis associated with an elevated serum FSH concentration suggests primary damage to the seminiferous tubules, and a low serum testosterone concentration associated with an elevated serum LH concentration suggests primary damage to the Leydig cells. A low serum testosterone concentration associated with a serum LH concentration that is not

Although recent research has highlighted the distressing impact of infertility for men, fertility issues are still routinely seen as a 'women's issue' - even when male factor infertility is highlighted. This article reports findings from a qualitative questionnaire study focusing on a sample of men with a male factor infertility diagnosis; an under-researched and marginalised group in the context of reproductive medicine. Our analysis suggests that male factor infertility is viewed by men as a failure of masculinity, as stigmatising and silencing, and as an isolating and traumatic experience.

BACKGROUND: The cystic fibrosis transmembrane conductance regulator (CFTR) is a cAMP-activated Cl(-) and HCO(3)(-) conducting channel, mutations of which are known to be associated with male infertility. However, the underlying mechanisms remain elusive.
METHODS: Literature databases were searched for papers on the topics related to CFTR and male fertility and infertility with relevant keywords. Unpublished data from authors' laboratory were also included for analysis.
RESULTS: Clinical evidence shows increased mutation frequency or reduced CFTR expression in men with congenital bilateral ab

INTRODUCTION: Male infertility (MI) has been widely associated with different comorbid conditions. The aim of this review is to summarize the available evidences investigating the link between MI cancer, chronic non-malignant conditions and overall health.
EVIDENCE ACQUISITION: A literature search has been conducted using the MEDLINE/PubMed and Scopus databases for English-language original and review articles and selecting publications from January 2007 to June 2017, although highly regarded older publications were also considered. The following key words and MeSH terms were combined: "male

We investigated the impact of prior anticancer treatments such as chemotherapy and radiotherapy on subsequent infertility treatment in cancer survivors who consulted our male infertility division. Of 1,525 male infertility patients who consulted our division between 2008 and 2018, 56 (3.7%) were cancer survivors. Of these, 32 received anticancer treatment (group A) and 24 were treated with surgery alone or were seen before anticancer treatment (group B). Semen analysis revealed that azoospermia in 26 subjects (81.3%) and 14 (58.3%) in groups A and B respectively. Ejaculatory dysfunction was ob

Male infertility is a disease that deserves greater clinical attention and research. A universally accepted definition that emphasizes the modulatory impact of age, lifestyle and environmental factors and includes comprehensive diagnostic and treatment guidelines is needed to ensure accurate evaluation and effective care. Accordingly, male infertility should be defined as a disease of the male reproductive system, caused primarily by congenital and genetic conditions, anatomical, endocrine, functional or immunological abnormalities of the reproductive system, genital tract infections, cancer a

OBJECTIVE: Semen parameters are directly correlated with the infertility of the male. Incidence rates of male factor infertility, azoospermia and cryptozoospermia differ according to many factors such as geographic region, age, occupation and body weight. The aim of the present study is to determine the incidence of male factor infertility, azoospermia and cryptozoospermia among patients who have been admitted to three separate infertility clinics in Turkey for infertility investigation and analyze the outcomes of these patients.
MATERIAL AND METHODS: A total of 9733 men, who have been admitt

Male infertility is an increasingly serious global health problem. As an organ with immune privilege, the testis possesses a unique immune microenvironment that shapes normal spermatogenesis. Distinct from the testicular microenvironment, the epididymis plays a critical role in both sperm maturation and immune regulation. This review aims to systematically describe the regulatory mechanisms of the testicular and epididymal immune microenvironment and, using various diseases as examples (varicocele, testicular torsion, prostatitis, orchitis, viral infections, environmental toxicant exposure, ag

The semen microbiome, once believed to be sterile, is now recognized as a dynamic ecosystem containing a diverse range of microorganisms with potential implications for male fertility and reproductive health. We aimed to examine the relationship between the semen microbiome, male infertility, and reproductive outcomes, highlighting the transformative role of next generation sequencing techniques and bioinformatics in exploring this intricate interaction, and we present a critical review of the published literature on this issue. Current evidence suggests a complex association between the compo

Male infertility is a global health problem, affecting up to 6% of reproductive age men worldwide. It has an enormous personal and societal burden, along with public health implications beyond the inability to reproduce, including reduced future health and longevity. While the impact of infertility has long been appreciated, the molecular architecture of the disease is largely unknown. Nevertheless, the past decade has witnessed significant advances in our understanding of the molecular basis of male infertility. Here, we describe the contributions of genetic and epigenetic mechanisms to infer

Infertility is defined as failure to achieve pregnancy after 12 months or more of regular, unprotected intercourse. Infertility is presumed after 12 months when the female partner is younger than 35 years, and after 6 months when the female partner is older than 35 years. Couples attempting to conceive should be encouraged to engage in intercourse every 1 to 2 days and can track ovulation to maximize the likelihood of conception. Evaluation of male infertility should include assessment for underlying medical conditions that may affect fertility and at least one semen analysis. Approximately 25

Following a year of regular unprotected intercourse with his partner, and without achieving pregnancy, Mr. L. turned to his general practitioner. A semen analysis was carried out and no spermatozoa was found. After being referred to a male infertility specialist, the patient underwent a second test and a comprehensive assessment of his azoospermia. The azoospermia was confirmed and the genetic investigation revealed aneuploidy..

Couple's infertility is a rising issue worldwide affecting approximately 15% of couples. In 50% of the couples, a male factor infertility can be identified. Moreover, 30% of the men exhibit reduced sperm quality without any identifiable reason, thereby delineating the condition of idiopathic male infertility (IMI). Despite numerous improvements in the diagnosis and treatment of male infertility over the last decades, idiopathic forms are still the most challenging clinical dilemmas. The aim of this article is to describe the comprehensive diagnostic work-up that each idiopathic infertile man s

SEPT12, as a member of the septin gene family, is preferentially expressed in the adult male testis and plays an important role in the spermatogenesis and maturation of sperm. It is also essential for the maintenance of the structural integrity of the sperm tail. SEPT12 is closely related to sperm morphological abnormality, and its mutation leads to decreased fertility or infertility of males. This review presents an overview of the advances in and affords a prospect of the studies on the structure and biological functions of SEPT12 and the impact of its mutation on male fertility.

Infertility is a common problem affecting one in six couples and in 30% of infertile couples, the male factor is a major cause. A large number of genes are involved in spermatogenesis and a significant proportion of male infertility phenotypes are of genetic origin. Studies on infertility have so far primarily focused on chromosomal abnormalities and sequence variants in protein-coding genes and have identified a large number of disease-associated genes. However, it has been shown that a multitude of factors across various omics levels also contribute to infertility phenotypes. The complexity

BACKGROUND: The general practitioner is often the first to assess couples with infertility. In up to half of all infertile couples, a male factor may be found as a contributing cause.
OBJECTIVE: The aim of this article is to provide a broad understanding of the options available for surgical management of male infertility to assist couples in navigating their treatment journey.
DISCUSSION: Treatments may be classified into four categories: surgery for diagnostic purposes, surgery to improve semen parameters, surgery to improve sperm delivery and surgery to retrieve sperm for in vitro fertili
