MRI findings of temporomandibular joints with disk perforation.

OBJECTIVE: To investigate magnetic resonance imaging (MRI) findings of temporomandibular joints (TMJs) with disk perforation. STUDY DESIGN: Thirty-one TMJs (31 patients) with disk perforation, 37 TMJs (34 patients) with anterior disk displacement and no disk perforation, and 22 asymptomatic TMJs of 11 volunteers were examined by MRI. The presence or absence of disk perforation was confirmed by TMJ arthrography. The MR images were evaluated for the presence or absence of disk deformity, disk displacement, condylar bone changes and joint effusion and for visualization of the temporal posterior …

The lexicon of polyethylene wear in artificial joints.

The analysis of wear on polyethylene components that have been retrieved after use in patients has provided invaluable understanding of how wear occurs in vivo, and how it may be minimized through improved materials and implant design. The great number of such studies that have been published over the past three decades has lead to an extensive vocabulary to describe the tribology of prosthetic joints. However, these also have led to some confusion, due to the occasional misuse of terms from classical tribology, along with the use of multiple terms to describe the same wear phenomenon, and vic…

Stability of Lisfranc joints in ballet pointe position.

BACKGROUND: Ballerinas develop stress fractures at the second metatarsal base associated with dancing en pointe. The purpose of this study w … d cuneiforms was determined radiographically. RESULTS: No significant displacement of the Lisfranc joints occurred when the shod foot with intact ligaments was loaded in the foot flat or en pointe positions. Serial sectioning of the ligaments from dorsal to plantar in the shod foot en pointe demonstrated no change in alignment after the dorsal and inter …

Radiographic evaluation of the acromioclavicular and sternoclavicular joints.

Plain radiography is useful for the initial assessment of suspected disorders of the sternoclavicular and acromioclavicular joints. Other modalities are often required to further assess more complex pathologies involving these joints, however. Ultrasound has been described as a screening tool to assess possible sternoclavicular joint dislocation; however, it is usually used only if CT and MRI are not readily available. It has also been used to confirm intraoperative relocation. Ultrasound has a limited role in the evaluation of the AC joint, where it is most useful to exclude the presence of j…

Ligamentous injuries about the ankle and subtalar joints.

Ligamentous injuries at the ankle and subtalar joint range from simple sprains to severe talar dislocations. While lateral ankle sprains are among the most frequently encountered injuries and do not pose a greater diagnostic problem, the surgeon must be suspicious not to overlook associated ligamentous injuries at the subtalar and midtarsal level that may result in chronic painful conditions. Syndesmotic instabilities with or without ankle fractures must be assessed carefully and treated properly, since these are prearthrotic conditions. In the treatment of chronic ankle or subtalar instabilit

The radioulnar joints and forearm axis: surgeons' perspective.

Forearm injuries are common and can vary in complexity. The fractures are easily diagnosed, but the associated soft tissue injuries may not … findings from these studies may lead to earlier repair of soft tissue injuries about the radioulnar joints and the interosseous membrane. Further follow-up of these procedures will be needed to validate their indications.

Ultrasound and operative evaluation of arthritic shoulder joints.

OBJECTIVE: To assess the diagnostic value of ultrasonography (US) in the evaluation of arthritic shoulder joints. METHODS: Twenty shoulders of 20 inpatients with arthritis were evaluated by US one day before the shoulder operation. Changes in the subacromial-subdeltoid bursa, biceps tendon and tendon sheath, rotatof cuff, and glenohumeral joint were recorded and compared with findings at operation. RESULTS: In the detection of effusion/hypertrophy in the subacromial-subdeltoid bursa, US had a sensitivity of 93% and a specificity of 83%. For a biceps tendon rupture US had a sensitivity of 70%…

[Involvement of the temporomandibular joints in Bechterew's disease].

Temporomandibular joints are involved in Bechterew's diseases in 59.6% of cases, more often in the peripheral form of the disease. Involvement at the debut of the disease was observed in 33.3%, including monoarthritis in 28.6%. The incidence of these lesions correlated with involvement of the hip, knee, and ankle joints. Unstable symmetrical arthritides with early rigidity predominate in the clinical picture. Extraarticular symptoms, such as defiguration, tenosynovitis, are typical. X-ray examination reveals pseudodilatation of the joint fissure, periarticular osteosclerosis, ossification of t…

Arthroscopic synovectomy. When, which diseases and which joints.

The success of arthroscopic synovectomy depends both on surgical skill and patient selection. Close cooperation between rheumatologist and rheumasurgeon is advocated. Alternative local joint treatments are discussed, with particular respect to radiation synovectomy. Patients with chronic inflammatory joint disease comprise the major group of patients. However, other diseases like haemophilia, pigmented villonodular synovitis, synovial chondromatosis, posttraumatic synovitis and septic arthritis are also discussed. A discussion of open versus arthroscopic synovectomy for each joint is presented

Mycobacterial and fungal infections of bone and joints.

The number of cases of tuberculous bone or joint infection reported annually in the United States has been rising, but it decreased slightly in 1993. Management of skeletal tuberculosis is a complex and evolving issue that requires knowledge of the treatment of drug-resistant organisms. Nontuberculous mycobacteria also may cause skeletal infections, which are often located in tenosynovium or osteoarticular components of the hand or wrist but may occur at other skeletal sites, particularly when there is underlying immunosuppression. Identification of the organism and determination of its drug s

Mycobacterial and fungal infections of bone and joints.

The incidence of tuberculosis of the bone or joint is increasing. The number of Mycobacterium tuberculosis isolates that are resistant to mu … ilum is an emerging pathogen in immunosuppressed patients with a proclivity for infecting bones and joints. The availability of the triazoles has substantially altered the therapeutic approach to fungal infections of bone or joint. Itraconazole can now be considered as therapy of blastomycosis, coccidioidosis, histoplasmosis, or sporotrichosis. Fluconazo …

Intra-articular inclusions of the cervical synovial joints.

Two hundred and ten cervical synovial joints from 15 adult vertebral columns were studied by dissection to determine the form, dispositions and prevalence of intra-articular inclusions. Three types of inclusions were found. Fat pads occurred regularly in the atlanto-occipital joints but rarely in the zygapophysial joints. Capsular rims occurred occasionally at all levels. Fibro-adipose meniscoid occurred regularly in the atlanto-axial and zygapophysial joints. The function of fibroadipose meniscoids seems to be to protect the articular cartilages in gliding joints that subluxate during normal …

Pressure-volume ratio in human cadaver hip joints.

In postmortem examinations 26 hip joints of 23 adult individuals were filled with Ringer's solution in order to furnish data on the relationship between intra-articular pressure, capsular content, and joint position. A tube system was connected to an 18-gauge needle and to a pressure transducer. After instillation of 2.5, 5.0, 7.5, and 10.0 ml of Ringer's solution, intra-articular pressure was recorded in six different joint positions (extension, 45 degrees and 90 degrees flexion, 40 degrees internal and 40 degrees external rotation, and 45 degrees abduction in extension). Intra-articular pres…

[Subchondral osteonecroses of large joints in rheumatoid arthritis].

The frequency and preferred localisation of overt subchondral osteonecrosis in large joints have been studied in a roentgenological and clinical study. Among 619 patients (454 female, 165 male) a severe subchondral osteonecrosis was found 44 times on one or more (maximum 3) large joints of 30 patients. The hip and shoulder joints were most frequently attacked, while patients with an advanced stage or longer duration of the disease and those in the middle age groups showed this most often. Osteonecrosis did not necessarily correlate with the activity of the disease process and with the presence…
