Limbic encephalitis associated with AMPA receptor and CRMP5 antibodies: A case report and literature review.

AIMS: AMPA receptor (AMPAR) and CRMP5 antibodies are relatively uncommon in limbic encephalitis, and patients with both antibodies are rare. We recently treated such a patient, but the patient died after active treatment. To further understand this disease, we conducted a case report and literature review. DISCUSSIONS: To date, five encephalitis patients, including our patient, have been found to be positive for AMPAR and CRMP5 antibodies. The male-to-female ratio of the reported cases is 4:1, and the age range is 26 and 62 years old. All five patients presented with various neuropsychiatric …

A pre-senile case of limbic encephalitis and cerebellar degeneration, with subacute onset of progressive dementia.

In case a pre-senile patient presented subacutely progressive dementia, secondary dementia, such as paraneoplastic neurological syndrome (PNS), hypothyroidism, confusion, early phase of primary degenerative dementia and prion diseases are to be considered. It is a case of pathologically confirmed, and clinico-pathologically assessed limbic encephalitis with cerebellar degeneration. The patient was a 63-year old male, with a well followed up medical history of gastric cancer 8 years earlier. Four weeks after he presented himself at our hospital his memory and disorientation progressively declin…

Recurrent polychondritis complicated with demyelinating encephalopathy and limbic encephalitis: a case report and literature review.

Recurrent polychondritis (RP) is a rare systemic autoimmune disorder that rarely involves the central nervous system (CNS). This report desc … sis of RP was confirmed based on the Damiani-Levine criteria, alongside the diagnoses of autoimmune limbic encephalitis (LE) and secondary demyelinating encephalopathy (DE). Following treatment with methylprednisolone pulse therapy followed by cyclophosphamide, the patient's symptoms improved significantly. At the three-month follow-up, limb muscle strength had recov …

Synchronous Late-Onset of Limbic Encephalitis and Tumefactive Demyelinating Lesion After the End of Immunotherapy.

Immunoreactive encephalitis is a very rare side effect of immunotherapy, which can lead to severe morbidity. Following azetolizumab, it typically occurs within 0-12 months, with a median of 15 days. In this case, immunoreactive encephalitis occurred 3.5 months after treatment with azetolizumab ended and 15.5 months after it began. Tumefactive demyelinating lesions represent a focal cerebral lesion with inflammatory demyelination. They are usually associated with multiple sclerosis, but can also occur drug-induced or in the context of autoantibody-mediated diseases. In MRI, tumefactive demyelin

Mesiotemporal Volumetry, Cortical Thickness, and Neuropsychological Deficits in the Long-term Course of Limbic Encephalitis.

BACKGROUND AND OBJECTIVES: Limbic encephalitis (LE) is an autoimmune disease often associated with temporal lobe epilepsy and subacute memory deficits. It is categorized into serologic subgroups, which differ in clinical progress, therapy response, and prognosis. Using longitudinal MRI analysis, we hypothesized that mesiotemporal and cortical atrophy rates would reveal serotype-specific patterns and reflect disease severity. METHODS: In this longitudinal case-control study, all individuals with antibody-positive (glutamic acid decarboxylase 65 [GAD], leucine-rich glioma-inactivated protein 1 …

Seronegative limbic encephalitis manifesting as subacute amnestic syndrome: a case report and review of the literature.

BACKGROUND: Limbic encephalitis (LE), a variant of autoimmune encephalitis, is inflammation of the limbic system of the brain. The disorder presents with subacute impairment of short-term memory, psychiatric manifestations, confusion and seizures. "Seronegative LE" is a challenging diagnosis in the absence of well-characterized autoantibodies. CASE PRESENTATION: A 33-year-old Kuwaiti woman with no relevant past history presented with subacute progressive amnesia of 6-month duration. Magnetic resonance imaging (MRI) showed bilateral hyperintensity of the limbic structures. An extensive workup …

Structural network topology in limbic encephalitis is associated with amygdala enlargement, memory performance and serostatus.

Limbic encephalitis (LE) forms a spectrum of autoimmune diseases involving temporal lobe epilepsy and memory impairment. Imaging features of LE are known to depend on the associated antibody and to occur on the brain network level. However, first studies investigating brain networks in LE have either focused on one distinct antibody subgroup or on distinct anatomical regions. In this study, brain graphs of 17 LE patients with autoantibodies against glutamic acid decarboxylase 65 (GAD-LE), four LE patients with autoantibodies against leucine-rich glioma-inactivated 1, five LE patients with auto…

[Non-small cell carcinoma presenting as limbic encephalitis. Place of endobronchial ultrasound in the diagnosis].

INTRODUCTION: Limbic encephalitis is a rare neurological paraneoplastic syndrome, characterized by anterograde amnesia, epilepsy and confusion. Diagnosis of the underlying cancer is essential for treatment. CASE REPORT: A 55-year-old heavy smoker was admitted on account of general physical deterioration and neurological symptoms. The diagnosis of limbic encephalitis was based on rapidly progressive symptoms, inflammatory cerebrospinal fluid, increased signal intensity in the temporal lobes on magnetic resonance imaging and the presence of anti-neuronal anti-Ma2 antibodies. The initial work-up…

[Acute encephalitis/encephalopathy associated with transient involuntary movement and limbic dysfunction in the recovery phase].

We report 3 patients aged 3 - 4 years who presented with impaired consciousness and status epilepticus or a cluster of seizures, followed by transient involuntary movements and limbic dysfunction in the recovery phase. In all the patients, the involuntary movements were observed mainly on the left side and consisted of dystonia, athetosis, chorea, facial myoclonus, and oral dyskinesia. The patients also showed stereotypic movement, oral tendency, visual agnosia, and emotional disturbance, which suggested limbic dysfunction resembling Klüver-Bucy syndrome. Single-photon emission computed tomogr

Atypical manifestation of human herpes virus-6 encephalitis with extra-limbic lesions: a case report.

Human herpesvirus-6 (HHV-6) encephalitis represents a serious opportunistic infection in immunocompromised patients that is associated with substantial mortality and long-term neurological sequelae. The disease most often involves the limbic system, with typical neuroimaging findings restricted to the hippocampal and amygdaloid structures. Here, we describe a rare case of HHV-6 encephalitis presenting with atypical imaging features, which demonstrated near-complete resolution following ganciclovir therapy. This case underscores that HHV-6 encephalitis is not invariably confined to the limbic s

Effective Treatment of Anti-GAD65 Limbic Encephalitis-Associated Epilepsy Despite Delayed Immunotherapy: A Brief Report.

A 44-year-old woman was referred to our epilepsy center for evaluation of refractory seizures persisting for 36 years. Diagnostic workup revealed anti-glutamic acid decarboxylase 65 limbic encephalitis (anti-GAD65 LE), supported by markedly elevated serum anti-GAD65 antibody titers and a compatible clinical presentation. Following initiation of a 6-month course of intravenous immunoglobulin (IVIG), seizure frequency decreased substantially, from 12 to 16 focal seizures per day to approximately one brief focal seizure per month. This was accompanied by improvements in cognition, psychiatric sym…

Limbic Encephalitis in Myelin Oligodendrocyte Glycoprotein Antibody-associated Disease: A Case Report and Literature Review.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a clinically diverse, demyelinating, and neuroinflammatory disorder of the central nervous system. We herein describe a case of MOGAD manifesting as limbic encephalitis (LE), characterized by a unilateral medial temporal lesion on magnetic resonance imaging of the brain. Electroencephalography (EEG) revealed sharp waves and occasional slow-wave activity in the bilateral parietal and occipitotemporal regions. The presence of imaging abnormalities characteristic of MOGAD and EEG abnormalities not typical of other autoimmu…

Case Report: Anti-SOX1 antibody-associated limbic encephalitis with hippocampal sclerosis: the first autopsy case.

We report the first autopsy case of anti-SOX1 antibody-associated encephalitis accompanied by hippocampal sclerosis (HS). The patient was a 69-year-old man with a long-standing history of schizophrenia who presented with generalized seizures, fever, and altered mental status. Despite treatment for pneumonia and hyponatremia, his condition rapidly deteriorated, resulting in recurrent seizures and impaired consciousness. Imaging and cerebrospinal fluid analysis confirmed paraneoplastic limbic encephalitis associated with small-cell lung carcinoma (SCLC), and serum analysis revealed strongly posi…

Psychiatric symptoms in anti glutamic acid decarboxylase associated limbic encephalitis in adults: a systematic review.

Autoimmune Limbic Encephalitis (LE) is a relatively new category of immune-mediated diseases with a wide range of neuropsychiatric symptoms. LE associated with Glutamic Acid Decarboxylase (GAD) antibodies is difficult to diagnose due to its possible atypical presentation with neuropsychiatric and behavioral features. We performed a systematic review of literature and retrieved 21 cases of anti GAD-associated LE with neuropsychiatric signs. Median age at onset was 27 years with a female predominance (81.0 %) and median diagnostic delay of 6 months. Clinical presentation included typical LE symp…

Anti-myelin oligodendrocyte glycoprotein antibody-positive acute disseminated encephalomyelitis mimicking limbic encephalitis: A case report.

Anti-myelin oligodendrocyte glycoprotein antibodies (MOG-ab) have been detected in various disorders of the central nervous system including acute disseminated encephalomyelitis (ADEM), neuromyelitis optica spectrum disorders (NMOSD), optic neuritis, myelitis, and cortical encephalitis. We report an atypical case of MOG-ab-associated encephalomyelitis with part of the clinical manifestations resembling limbic encephalitis. Multifocal, hyperintense, bilateral lesions predominantly affecting the white matter on brain magnetic resonance imaging and marked response to steroid therapy were compatib…

Small-Cell Lung Cancer with Positive Anti-NMDAR and Anti-AMPAR Antibodies Paraneoplastic Limbic Encephalitis.

We report the case of a 66-year-old woman, with paraneoplastic limbic encephalitis, treated 6 months earlier for bladder neoplasia. The patient presented to the emergency room with rapidly increasing symptoms, noninfectious cerebral spinal fluid associated with positive anti-NMDAR (as well as in serum) and positive AMPAR antibodies in the serum. Four months later, the patient was diagnosed with a small-cell lung cancer for which chemotherapy and radiotherapy was commenced. Simultaneously, endoscopic surgical treatment was undertaken for an in situ relapse of the bladder neoplasm. After the com…

Multicentric Castleman's disease with voltage-gated potassium channel antibody-positive limbic encephalitis: a case report.

BACKGROUND: Castleman's disease is a rare lymphoproliferative disorder which occurs in localized and multicentric forms and can mimic lymphoma. Despite its well-known association with certain autoimmune diseases, including paraneoplastic pemphigus and myasthenia gravis, Castleman's disease has not previously been associated with limbic encephalitis. CASE PRESENTATION: We report the case of a 47-year old Caucasian man who presented with subacute onset of constitutional symptoms, diffuse lymphadenopathy, and stereotyped spells involving olfactory aura, nausea, disorientation, and unresponsivene…
