Anesthetic Management of a Patient with Advanced Anti-Myelin-Associated Glycoprotein Antibody Neuropathy in the Absence of Measurable Quantitative Neuromuscular Responses: A Case Report.

Background and Clinical Significance: Anti-myelin-associated glycoprotein (MAG) antibody polyneuropathy is a rare, chronic IgM-mediated demyelinating peripheral neuropathy predominantly affecting sensory nerves in older adults, commonly in association with monoclonal gammopathy of undetermined significance. Reports describing anesthetic management in patients with this condition remain extremely limited, and no specific guidelines currently exist regarding neuromuscular blocking agent (NMBA) use or neuromuscular monitoring in this population. Case Presentation: A 79-year-old man with anti-MAG

Bone health in children with neuromuscular disorders.

This paper provides a broad review of bone health issues in children with neuromuscular diseases (NMD). As Duchenne muscular dystrophy is the most common neuromuscular disease of childhood, there is a strong emphasis on this. This paper reviews the incidence of fractures, the degree of the reduced bone mineral density, the risk factors associated with this common problem, and the non-pharmacological and pharmacological treatment options. Recommendations for monitoring and treatment are provided.

A simulation of neuromuscular function and heart rate during induction, maintenance, and reversal of neuromuscular blockade.

We developed a two-compartment model to simulate neuromuscular function and heart rate following the administration of four nondepolarizing neuromuscular blocking agents (atracurium, vecuronium, pancuronium, and d-tubocurarine), three neuromuscular block reversal agents (edrophonium, neostigmine, and pyridostigmine), and two anticholinergic agents (atropine and glycopyrrolate). Twitch depression, train-of-four ratio, and heart rate were modeled during fentanyl, halothane, enflurane, or isoflurane anesthesia, optionally supplemented with nitrous oxide. Simulation results, compared with publishe

Use of Muscle Relaxants in Emergency Medicine: A Review.

Modern anesthesia, intensive care, and emergency medicine rely heavily on neuromuscular blocking agents (NMBAs), first introduced in 1942. These agents not only facilitate endotracheal intubation but also improve surgical conditions by suppressing muscle responses to stimuli. NMBAs function via depolarizing (eg, succinylcholine) or non-depolarizing mechanisms. Non-depolarizing agents are further classified by chemical structure into aminosteroids (eg, rocuronium, vecuronium), benzylisoquinolinium compounds (eg, atracurium, cisatracurium), and asymmetric mixed-onium chlorofumarates (gantacurium

Evaluation of Neuromuscular Blockade: A Comparative Study of TOF-Cuff® on the Lower Leg and TOF-Scan® on the Ulnar Nerve During Mivacurium Anesthesia.

BACKGROUND To evaluate neuromuscular monitoring during anesthesia with mivacurium, this study assessed the correlation between measurements of TOF-Cuff® placed on the lower leg and stimulating the tibial nerve and TOF-Scan® values from the adductor pollicis muscle. Additionally, systolic (SBP) and diastolic (DBO) blood pressure measured in both locations were compared. MATERIAL AND METHODS Twenty-six patients participated in this observational clinical trial. The TOF-Cuff® was placed on the lower leg and the TOF-Scan® was placed on the thumb. Train-of-four (TOF) values were recorded simultaneo

Unexpected delayed reversal of rocuronium-induced neuromuscular blockade by sugammadex: A case report and review of literature.

BACKGROUND: Rocuronium, a nondepolarizing muscle relaxant, is usually administered during general anesthesia to facilitate endotracheal intubation and keep patients immobile during the surgery. Sugammadex, the selective reversal agent of rocuronium, fully reverses the neuromuscular blockade (NMB) at the end of surgery. Most reports show that sugammadex rapidly achieves a ratio of train-of-four (TOF), a quantitative method of neuromuscular monitoring, of 0.9 which ensures adequate recovery for safe extubation. However, very rare patients with neuromuscular diseases may respond poorly to sugamma

Neuromuscular Consequences of Mechanical Ventilation: Prevention and Management.

Mechanical ventilation is a life-saving supportive therapy, yet it can lead to several harmful consequences such as lung injury, hemodynamic compromise and neuromuscular dysfunction when not managed appropriately. Critically ill patients are vulnerable due to a myriad of modifiable risk factors, which can be mitigated through targeted interventions. Systematic monitoring of respiratory and limb muscle function is therefore essential to individualize ventilatory support, titrate sedation, and guide rehabilitation strategies aimed at preserving neuromuscular integrity. With the advent of novel t

Requirements for muscle relaxation in Friedreich's ataxia.

Friedreich's ataxia is an inherited disorder of the nervous system, requiring special care during anaesthesia, because of increased sensitivity to muscle relaxants. We report a case of Friedreich's ataxia in a 31-year-old woman, anaesthetised on two occasions, for tendinoplasty and pes cavus repair. Atracurium was used for neuromuscular blockade and monitored by a train-of-four twitch technique. The patient's response was normal. She returned to adequate spontaneous breathing within 20 min of the last dose of the muscle relaxant without need for anticholinesterase administration. When neuromus

The impact of sugammadex dosing and administration practices on potential cost savings for pharmacy departments.

PURPOSE: Neuromuscular blocking agents (NMBAs) are commonly used during surgery, and restoring neuromuscular function at the end of surgery is vital in preventing complications of residual paralysis. Recent guidelines from the American Society of Anesthesiology recommend using sugammadex over neostigmine; however, sugammadex is significantly more expensive than neostigmine and may increase drug expenditure for pharmacy departments. This review summarizes evidence evaluating sugammadex dose adjustments and the potential of these dose adjustments to lead to cost savings for pharmacy departments.

The effects of acetylcholinesterase inhibitors on morbidity after general anesthesia and surgery.

Non-depolarizing neuromuscular blocking agents are used during general anesthesia to facilitate intubation and optimize surgical conditions. When patients leave the operating room after surgery, postoperative residual neuromuscular block occurs frequently, increasing vulnerability to respiratory complications such as hypoxemia and unplanned postoperative mechanical ventilation. To restore neuromuscular transmission and skeletal muscle strength, anesthesiologists typically administer peripherally acting acetylcholinesterase inhibitors such as neostigmine. However, neostigmine's desirable effect

[Secondary neuromuscular deficiencies in critical care patients].

The authors report three cases of neuromuscular deficits occurring after a variable time of mechanical ventilation in ICU patients suffering from acute respiratory failure requiring sedation and neuromuscular blockade by pancuronium bromide. The clinical features of these deficits were similar : flaccid quadriparesia slowly reversible without sensory loss and difficult weaning from ventilation for two patients. The paraclinical investigations (measurements of creatine kinase, electrophysiological examination) permitted to distinguish two diseases: critical illness polyneuropathy (in patient wi

Myostatin Is a Quantifiable Biomarker for Monitoring Pharmaco-gene Therapy in Duchenne Muscular Dystrophy.

Recently, several promising treatments have emerged for neuromuscular disorders, highlighting the need for robust biomarkers for monitoring therapeutic efficacy and maintenance of the therapeutic effect. Several studies have proposed circulating and tissue biomarkers, but none of them has been validated to monitor acute and long-term drug response. We previously described how the myostatin (MSTN) level is naturally downregulated in several neuromuscular diseases, including Duchenne muscular dystrophy (DMD). Here, we show that the dystrophin-deficient Golden Retriever muscular dystrophy (GRMD)

Anesthesia and myasthenia gravis.

Myasthenia gravis (MG) is a disease affecting the nicotinic acetylcholine receptor of the post-synaptic membrane of the neuromuscular junction, causing muscle fatigue and weakness. The myasthenic patient can be a challenge to anesthesiologists, and the post-surgical risk of respiratory failure has always been a matter of concern. The incidence and prevalence of MG have been increasing for decades and the disease is underdiagnosed. This makes it important for the anesthesiologist to be aware of possible signs of the disease and to be properly updated on the optimal perioperative anesthesiologic

The isolation of thymopoietin (thymin).

The isolation from bovine thymus of two closely related polypeptides, thymopoietin I and II, is described. These are considered to be thymic hormones, which physiologically induce the differentation of prothymocytes to thymocytes within the thymus. Ths isolation of the thymopoietins was monitored not by their differentiative effects, but by a presumably secondary effect on neuromuscular transmission. This was discerned in experimental studies related to the human disease myasthenia gravis in which it was suggested that autoimmune thymitis was regularly present. In an animal model, experimental

Validation of a convolutional neural network that reliably identifies electromyographic compound motor action potentials following train-of-four stimulation: an algorithm development experimental study.

BACKGROUND: International guidelines recommend quantitative neuromuscular monitoring when administering neuromuscular blocking agents. The train-of-four count is important for determining the depth of block and appropriate reversal agents and doses. However, identifying valid compound motor action potentials (cMAPs) during surgery can be challenging because of low-amplitude signals and an inability to observe motor responses. A convolutional neural network (CNN) to classify cMAPs as valid or not might improve the accuracy of such determinations. METHODS: We modified a high-accuracy CNN origin
