Treatment of social anxiety disorder and attenuated psychotic symptoms with cannabidiol.

Anxiety disorders in young people are frequently comorbid with other mental disorders and respond unsatisfactorily to first-line treatment in many cases. Here, we report the case of a 20-year-old man with severe social anxiety disorder, major depressive disorder, insomnia and attenuated psychotic symptoms despite ongoing treatment with cognitive behavioural therapy and mirtazapine who was treated with adjunctive cannabidiol (CBD) in doses between 200 and 800 mg/day for 6 months. During treatment with CBD, he experienced subjective benefits to his anxiety, depression and positive symptoms durin

Treatment of a Patient with Psychotic Disorder Undergoing Haemodialysis: a Case Report.

We report a case of a 35-year old male patient that was in treatment for chronic kidney failure for 20 years, with a personal history of unsuccessful kidney transplantation and undergoing continuous haemodialysis. He had to be treated psychiatrically for the first time because of exacerbation of psychotic symptoms. He was hospitalized twice in succession, because of the severity of the psychotic symptoms that included imperative auditory hallucinations that also led to a serious suicidal attempt. The prescribed psychiatric therapy had to be adjusted in type, dosage and especially the time of a

[Amisulpride: a treatment option for psychotic disorders soon to be available in the Netherlands].

BACKGROUND: The antipsychotic drug amisulpride has not yet been officially approved for use in the Netherlands, although it is already available in other European countries , including Belgium, the UK and Germany. However, a fast-track procedure has recently been initiated so that amisulpride will soon become available in the Netherlands as well. AIM: To summarise the efficacy and side effects of amisulpride. METHOD: We discuss the evidence presented in the scientific literature. RESULTS: The scientific literature assures us that amisulpride is an effective antipsychotic drug with an accept

Disruption of cortical association networks in schizophrenia and psychotic bipolar disorder.

IMPORTANCE: Psychotic disorders (including schizophrenia, schizoaffective disorder, and psychotic bipolar disorder) are devastating illnesses characterized by breakdown in the integration of information processing. Recent advances in neuroimaging allow for the estimation of brain networks on the basis of intrinsic functional connectivity, but the specific network abnormalities in psychotic disorders are poorly understood. OBJECTIVE: To compare intrinsic functional connectivity across the cerebral cortex in patients with schizophrenia spectrum disorders or psychotic bipolar disorder and health

[Alcohol-induced psychotic disorder--a diagnostic entity of its own?].

Alcohol-induced psychotic disorders are relatively rare schizophrenia-like disorders characterized by key symptoms, such as auditory hallucinations, anxiety and delusions while disorders of consciousness and orientation are lacking. The diagnostic entity of this disorder has been questioned. Recent findings indicate a high risk for rehospitalization and relapse but compared to other drug-induced disorders a low risk of schizophrenia-like course of the illness (5 % compared to 40-50 % with other drugs). Pathophysiological, diagnostic and therapeutic aspects of alcohol-induced psychotic disorder

Variations in incidence and age of onset of acute and transient psychotic disorders.

PURPOSE: To determine incidence and age of onset of the ICD-10 category of 'acute and transient psychotic disorders' (ATPDs) characterised by subtypes with polymorphic, schizophrenic and predominantly delusional symptoms, pointing out differences from schizophrenia (SZ) and bipolar affective disorder (BD). METHODS: We identified all subjects aged 15-64 years who were listed for the first time in the Danish Psychiatric Register with a diagnosis of ATPDs (n = 3,350), SZ (n = 4,576) and BD (n = 3,200) in 1995-2008. Incidence rates and rate ratios (IRR; 95 % confidence interval) by gender and age

Pituitary volume, stress reactivity and genetic risk for psychotic disorder.

BACKGROUND: Hypothalamic-pituitary-adrenal (HPA) axis hyperactivity, associated with increased pituitary volume, may mediate observed alterations in stress reactivity in patients with psychotic disorder. We examined the association between pituitary volume, real-life stress reactivity and genetic liability for psychotic disorder. METHOD: Pituitary volumes were derived from magnetic resonance imaging (MRI) scans of 20 patients with psychotic disorder, 37 non-psychotic siblings of these patients, and 32 controls. The Experience Sampling Method (ESM) was used to measure emotional stress reactivi

[Shared psychotic disorder (Folie à deux) and Huntington's disease].

INTRODUCTION: Folie à deux describes a rare psychiatric disorder of mostly closely related persons. The main feature of this disease is the unconditional adoption of the delusions of the primarily diseased person by the second person. This disturbance most frequently originates from a paranoid schizophrenia. METHOD: Our case is the first published description of a married couple with shared psychotic disorders which were caused by a genetically verified Chorea Huntington of the husband. RESULTS: The symptoms of the wife quickly declined after spatial separation from her primarily diseased hu

Schizophreniform-like psychotic disorder induced by phentermine: a case report.

Phentermine is a central nervous system stimulant that can increase brain dopamine levels and has a structure similar to amphetamine. We report a case of schizophreniform-like psychotic disorder induced by phentermine in a young woman without a personal or family history of psychiatric disorders. The patient had taken phentermine 30 mg daily for more than three years. Psychotic symptoms developed acutely after she increased the phentermine dosage to 150-180 mg/day for one month due to an increase in work load. After discontinuation of phentermine and administration of neuroleptics, the psychot

Effectiveness of group metacognitive training and cognitive-behavioural therapy in a transdiagnostic manner for young patients with psychotic and non-psychotic disorders.

AIM: To investigate group metacognitive training and cognitive-behavioural therapy (MCT/CBT) prospectively in a young population with various psychiatric disorders, including psychotic and mood disorders. METHODS: This was a prospective study to investigate the effectiveness of group MCT/CBT on quality of life, psychotic symptoms, depression, self-esteem, perceived stress, social function and social cognition. The objective measures included the Positive and Negative Syndrome Scale (PANSS), clinical global impression (CGI), personal and social performance scale for social functioning, a compu

Neurocognition and functional outcome in patients with psychotic, non-psychotic bipolar I disorder, and schizophrenia. A five-year follow-up.

BACKGROUND: Bipolar disorder (BD) and schizophrenia (SZ) are characterized by neurocognitive and functional deficits with marked heterogeneity. It has been suggested that BD with a history of psychotic symptoms (BD-P) could constitute a phenotypically homogeneous subtype characterized by greater neurocognitive and functional impairments, or by a distinct trajectory of such deficits. The aim of this study was to compare the neurocognitive and functional course of euthymic BD-P, euthymic BD patients without a history of psychosis (BD-NP), stabilized patients with schizophrenia and healthy subjec

The role of cognitive functioning in the relationship between childhood trauma and a mixed phenotype of affective-anxious-psychotic symptoms in psychotic disorders.

Cognitive impairments in patients with psychotic disorder have been associated with poor functioning and increased symptom severity. Furthermore, childhood trauma (CT) exposure has been associated with worse cognitive functioning as well as co-occurrence of affective-anxious-psychosis symptoms or a 'mixed phenotype of psychopathology' (MP), which in turn is associated with greater symptom severity, and poor functioning. This study aims to evaluate if cognition could be associated with CT/MP. 532 patients with non-affective psychotic patients were assessed on CT, symptom profile, cognition, fun

Contribution of psychological and social factors to psychotic and non-psychotic relapse after childbirth in women with previous histories of affective disorder.

Twenty-six women with a history of bipolar or schizoaffective disorder, 17 women with histories of major depressive disorder and 45 control women without any previous psychiatric history were assessed in the 9th month of pregnancy on selected psychosocial measures. No subject was a 'case' as defined by the Research Diagnostic Criteria (RDC) from this time until the delivery. Within 6 months postpartum, 22 (51%) of the women with histories of mental illness were categorised as having relapsed (RDC case). Twelve women developed a psychosis (mania, hypomania or schizomania) and these illnesses oc

Co-occurrence of psychotic disorders and borderline personality disorder: a systematic review and meta-analysis.

BACKGROUND: The co-occurrence of psychotic disorders and borderline personality disorder (BPD) complicates clinical management, with overlapping symptoms exacerbating morbidity and impairing therapeutic outcomes. This systematic review and meta-analysis aimed to estimate the prevalence of psychotic disorders and BPD co-occurrence, including with first-episode psychosis (FEP) and to describe associated sociodemographic and clinical characteristics. METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, four databases were systematically s

The Prevalence of Attention Deficit Hyperactivity Disorder in Psychotic Disorders: Systematic Review and Meta-analysis.

BACKGROUND: Although attention deficit hyperactivity disorder (ADHD) is known to be common in psychotic disorders, reported prevalence rates vary widely, with limited understanding of how different factors (eg, assessment methods, geographical region) may be associated with this variation. The aim was to conduct a systematic review and meta-analysis to determine the prevalence of ADHD in psychotic disorders and factors associated with the variability in reported rates. STUDY DESIGN: Searches were conducted in MEDLINE, Embase, PsycINFO, CINAHL, and Scopus in May 2023. Studies were eligible if

Clinical predictors of acute risperidone response in schizophrenia, schizoaffective disorder, and psychotic mood disorders.

BACKGROUND: In studies of patients with schizophrenia, the atypical antipsychotic risperidone has been shown to be comparable in efficacy to haloperidol and, at dosages of 4 to 8 mg/day, to have a lower rate of extrapyramidal side effects. However, little is known about the efficacy of risperidone in patients with schizophrenia refractory to treatment with typical antipsychotics, schizoaffective disorder, and psychotic mood disorders. The purpose of this study was to assess the efficacy of risperidone in the treatment of these disorders and to identify clinical factors associated with risperid

Clinical Significance of Psychotic Experiences in Preadolescent Children with Non-Psychotic Mental Disorders-The Danish High Risk and Resilience Study, VIA 11.

BACKGROUND AND HYPOTHESIS: Psychotic experiences (PE) are associated with higher severity of other psychopathology in adults and adolescents with non-psychotic mental disorders. Knowledge on the clinical significance of PE in children with non-psychotic mental disorders, which may help improve early identification of need for treatment, is sparse. STUDY DESIGN: In this cross-sectional study, preadolescent children (n = 157, mean age 12.0, SD 0.2, range 11.1-12.7 years) at familial high-risk of schizophrenia (n = 78) or bipolar disorder (n = 40) and population-based controls (n = 39) with non-

The association between relapse and the outcome of schizophrenia and recurrent psychotic disorders.

BACKGROUND: Having a relapse of schizophrenia or recurrent psychosis is feared by patients, can cause social and personal disruption and has been suggested to cause long-term deterioration, possibly because of a toxic biological process. AIMS: To assess whether relapse affected the social and clinical outcomes of people enrolled in a 24-month randomised controlled trial of antipsychotic medication dose reduction versus maintenance treatment. METHODS: The trial involved participants with a diagnosis of schizophrenia or recurrent, non-affective psychosis. Relapse was defined as admission to ho
