Asynchronies of diphenhydramine plasma-performance relationships.

The relation between performance on driving-related tasks and plasma levels of diphenhydramine was studied in eight male volunteers over 24 hours following oral administration. DPH plasma concentrations rose to peak levels in 1.5-2.5 hours varying with dose, declining to a nearly constant level by 12-24 hours. For all behavioral measures, the shape of performance curves over time was similar to that of plasma, reaching maximum decrements in 1-4 hours. The relation between plasma levels and performance was asynchronous varying with behavioral measure and dose. The use of plasma DPH values to pr

Eye tracker outcomes in a randomized trial of 40 sessions of hyperbaric oxygen or sham in participants with persistent post concussive symptoms.

PURPOSE: Eye movements may offer a sensitive method to measure response to intervention in mild traumatic brain injury (mTBI). METHODS: The Brain Injury and Mechanisms of Action of Hyperbaric Oxygen for Persistent Post-Concussive Symptoms after Mild Traumatic Brain Injury Study (BIMA) randomized 71 participants to 40 sessions of hyperbaric oxygen or sham. A companion normative study (Normal) enrolled 75 participants. An eye tracking system measured left and right eye movements for saccadic and smooth pursuit. At baseline two smooth pursuit tasks, circular and horizontal ramp, and four saccadi

Quantification of tracking eye movements in patients with neurological disorders.

Tracking eye movements were studied in 20 healthy subjects and in 24 patients with vestibular neuronitis, disorders within the frontal cortex, the cerebellum or the brain stem. The tracking tests were performed at six different target velocities between 10-60 degrees s-1 and different parameters were evaluated. The maximum velocity gain of smooth pursuit as normal in the group with vestibular neuronitis but reduced in the groups with disorders within the frontal cortex, cerebellum and the brain stem. The total amplitude of tracking and amplitude of smooth pursuit were normal in the groups with

Eye movements as indicators of awareness in prolonged disorders of consciousness: a scoping review of behavioral and neural evidence.

BACKGROUND: In prolonged disorders of consciousness (DoC), visual fixation and smooth pursuit are among the earliest signs of re-emerging awareness. Yet, because their physiology relies heavily on brainstem–cerebellar circuitry, it remains debated whether these ocular behaviors invariably reflect consciousness or, after severe brain injury, also arise from reflexive/subcortical mechanisms. METHODS: To address this question, we conducted a PRISMA-ScR scoping review of PubMed/MEDLINE and Scopus to verify the behavioral and neural correlates of fixation and pursuit. Ocular behavior evidence was c

Selective saccadic palsy after cardiac surgery.

We report a patient who showed a selective deficit of voluntary saccades and quick phases of nystagmus after cardiac surgery. Voluntary saccades in the horizontal plane were very slow, while vertical saccades, vestibular and optokinetic nystagmus, were absent. However, smooth pursuit, the vestibulo-ocular reflex, and the ability to hold steady eccentric gaze were preserved.

Eye movements in children with opsoclonus-polymyoclonus.

The anatomical localisation of the abnormality underlying opsoclonus-polymyoclonus-the "Dancing Eye Syndrome"-is uncertain and both the brainstem and cerebellum have been implicated. We used electrooculographic recordings to assess the eye movements in five children with this syndrome. Overshoot dysmetria was the consistent feature for the saccades of all five cases. One patient was also assessed after the main symptoms of ataxia and opsoclonus (saccadic oscillations, without an intersaccadic interval) had resolved and the saccades were found to remain hypermetric. Smooth pursuit and optokinet

Reliability Testing of a Multitask Virtual Reality Sensorimotor Assessment.

We evaluated the test-retest reliability of a comprehensive, multitask virtual reality (VR)-based sensorimotor assessment designed to capture and quantify aspects of body movement during standardized visuomotor, auditory, gait, oculomotor, and balance tasks. Twenty healthy adults completed 3 sessions of the Dynamic Assessment of Sensorimotor Health protocol on nonconsecutive days at a university research center. Dynamic Assessment of Sensorimotor Health includes 7 VR tasks (Trail Making, Sound Localization, Tandem Gait, Reaction Board, Smooth Pursuit, Single-Leg Balance, and Target Matching Ba

Distinguishing schizophrenic patients from healthy controls by quantitative measurement of eye movement parameters.

BACKGROUND: Eye tracking dysfunction is a putative trait marker for susceptibility to schizophrenia; however, it cannot be recommended as an additional tool for the diagnosis of schizophrenia, due to low sensitivity and specificity. METHODS: To assess the diagnostic potentials of combinations of eye movement paradigms, four smooth pursuit experiments (1: constant velocity of 15 degrees/sec; 2 and 3: combination with either visual or auditory distractors; 4: constant velocity of 30 degrees/sec) and two saccadic eye movement experiments (1: reflexive saccades; 2: voluntary saccades) were conduc

Auditory clicks distort perceived velocity but only when the system has to rely on extraretinal signals.

Previous work has found that repetitive auditory stimulation (click trains) increases the subjective velocity of subsequently presented moving stimuli. We ask whether the effect of click trains is stronger for retinal velocity signals (produced when the target moves across the retina) or for extraretinal velocity signals (produced during smooth pursuit eye movements, when target motion across the retina is limited). In Experiment 1, participants viewed leftward or rightward moving single dot targets, travelling at speeds from 7.5 to 17.5 deg/s. They estimated velocity at the end of each trial.

Periodic alternating nystagmus in Meniere's disease: the peripheral type?

Periodic alternating nystagmus (PAN) is a spontaneous horizontal nystagmus that reverses direction periodically. PAN has been reported in acquired and congenital forms. The lesion site of the acquired form of PAN has been attributed to the caudal brainstem or cerebellum. We report an extremely rare case (a 59-year-old woman) with bilateral Meniere's disease, who presented PAN. PAN in this patient was suppressed in the light. This patient had smooth pursuit and normal optokinetic nystagmus, although patients with PAN usually show saccadic pursuit and impaired optokinetic nystagmus. These findin

Impaired suppression of vestibular nystagmus by fixation of visual and acoustic targets in neurological patients.

Suppression of vestibular nystagmus induced by fixation of visual and acoustic targets moving with the head during sinusoidal rotation (0.1 Hz, 75 degrees/second peak velocity) was tested in cerebellar and noncerebellar patients. Visual suppression was impaired greatly in cerebellar patients, without correlation with visual smooth-pursuit defects. Acoustic suppression was equal to or slightly weaker than visual suppression. In noncerebellar patients, a disturbance of visual suppression was found only in the presence of a severe impairment of pursuit eye movements. Acoustic suppression did not

Eye movements of human albinos.

Albino mammals are known to suffer from misrouted optic projections and there is a growing body of evidence suggesting that human albinos have similar aberrant anatomical pathways. The present study examined the possible consequences of such aberrant pathways on the oculomotor performance of five adult human albinos. Optokinetic nystagmus to drifting grating patterns and pursuit eye movements were measured. The subjects' congenital nystagmus was also measured under different conditions of gaze position and ambient room illumination. Two of the subjects showed clear instances of an inversion in

Oculomotor disorders and nystagmus.

The precision of visual perception depends on accurately directing the fovea to objects of interest, a process mediated by six supranuclear oculomotor systems that all converge in the brainstem: saccadic and smooth pursuit, vestibular optokinetic, fixation, and vergence. This chapter provides a clinically oriented overview of brainstem neuroanatomy relevant to eye movement control and links it to characterisitic patterns of gaze disturbance. After outlining the organization and functions of the oculomotor (III), trochlear (IV), and abducens (VI) nuclei and their connections via the medial long

Abnormal Oculomotor Functions in Amyotrophic Lateral Sclerosis.

BACKGROUND AND PURPOSE: Although traditionally regarded as spared, a range of oculomotor dysfunction has been recognized in amyotrophic lateral sclerosis (ALS) patients. ALS is nowadays considered as a neurodegenerative disorder of a third compartment comprising widespread areas of extra-motor brain including cerebellum. Our objective was to perform an observational study to examine for ocular motor dysfunction in patients with ALS and for any differences between bulbar-onset and spinal-onset patients. METHODS: Thirty two ALS patients (bulbar onset: 10, spinal onset: 22) underwent the standar

Bilateral lesions of the medial longitudinal fasciculus in monkeys: effects on the horizontal and vertical components of voluntary and vestibular induced eye movements.

Bilateral transections across the brainstem interrupted the medial longitudinal fasciculus (MLF) in three monkeys trained to make eye movements while subjected to horizontal or vertical angular accelerations. Eye movements measured before and after the lesion revealed deficits in both voluntary and vestibular compensatory eye movements; the deficits differed in the horizontal and vertical directions. Vertical saccades in both directions were normal but eccentric positions of fixation could not be maintained; a drift toward the midline followed by a corrective saccade produced vertical fixation

[Eye tracking disorders in schizophrenic patients and their parents].

UNLABELLED: Several studies have confirmed the existence of genetic factors in schizophrenia. However, the genotype predisposing for the disease is not known yet. Nevertheless, those genetic factors in the families of schizophrenic patients urge us to search for genetic vulnerability markers of schizophrenia. Ocular pursuit disorders, in particular, could be one of those vulnerability markers. Eye movements have been often tested in schizophrenia. Most of the schizophrenic patients have eye-tracking disorders and their biological relatives demonstrate an increased prevalence of eye-tracking im

Dyskinesia and eye tracking dysfunction in young schizophrenic patients - a pilot study.

The study dealt with the occurrence of dyskinetic symptoms and disturbances in eye movements in 15 young schizophrenic patients and the effect on these disturbances of anticholinergic antiparkinsonian medication administered during neuroleptic medication maintained at a stable level. The onset of illness had occurred at a younger age in the dyskinetic patients than in the others, and they had received larger amounts of high-dose neuroleptic medication, which, however, equalled an average daily dose of only 331 mg of chlorpromazine during an average of 2 1/2 years; thus, dyskinetic symptoms can
