Treatment of scaphoid nonunion by volar inlay distal radius bone graft.

Volar inlay corticocancellous bone grafting is an accepted method of treatment for symptomatic nonunion of the carpal scaphoid. Performance of this procedure with a radial graft without styloidectomy yields results comparable to those obtained with iliac grafting. This permits the operation to be performed through a single incision. The elimination of the need for iliac dissection avoids morbidity associated with the iliac exposure.

Scaphoid specific volar locking plate and non-vascularised iliac crest bone graft in scaphoid non-union. A comparative cohort study.

Scaphoid non-union develops in 10% of scaphoid fractures. There is sparse literature on fixation methods other than screws. We compared union rates following fixation of scaphoid non-union using screw fixation and a novel method of plate fixation. Retrospective study. Union rates were assessed at 3 months post-operatively by a musculoskeletal radiologist. 15 patients underwent screw fixation and 15 underwent plate fixation. 86% union rate with screw fixation and 72% plate fixation united. There was no significant difference. Screw fixation and plate fixation in our institution both provide uni

Local distal radius bone graft versus iliac crest bone graft for scaphoid nonunion: a comparative study.

The aim of the study is to find whether there is any superiority of the iliac crest bone graft over the distal radius bone graft, in surgery of nonunion of scaphoid fractures. This is a prospective randomized controlled trial comparing 50 patients treated with internal fixation and distal radius bone graft (group 1) and 50 patients in whom iliac crest bone graft was used instead (group 2). The patients donor site pain in the postoperative period was assessed using visual analogue scale. At each follow-up, each patient was evaluated using Quick DASH score and Mayo's scoring system. A new variant of scaphoid reconstruction: Treatment of scaphoid non-union with avascular bone interponate and high compression screw (Synthes(®)).

Scaphoid fractures as frequently overseen injuries often result in scaphoid non-unions, that need to be treated to prevent carpal collapse a … I-V complete in 100%, moderate pain n=2 (16.67%). Three patients with persisting fracture gap had a scaphoid bone fractured in the proximal third; one patient even with a very small proximal fragment. One persisting non-union was localized in the middle third (period between injury and operation = 5 years). In conclusion, our patients showed better healing rate …

Nonunion scaphoid bone shape prediction using iterative kernel principal polynomial shape analysis.

BACKGROUND: The scaphoid is an important mechanical stabilizer for both the proximal and distal carpal columns. The precise estimation of the complete scaphoid bone based on partial bone geometric information is a crucial factor in the effective management of scaphoid nonunion. Statistical shape model (SSM) could be utilized to predict the complete scaphoid shape based on the defective scaphoid. However, traditional principal component analysis (PCA) based SSM is limited by its linearity and the inability to adjust the number of modes used for prediction. PURPOSE: This study proposes an itera…

Free Vascularized Medial Femoral Condyle Bone Graft After Failed Scaphoid Nonunion Surgery.

BACKGROUND: Outcomes following revision surgery for scaphoid nonunions with osteonecrosis are guarded. We hypothesized that use of free vascularized medial femoral condyle (MFC) bone grafts can lead to healing of the nonunion, increase vascularity of the proximal pole, and restore scaphoid architecture, resulting in acceptable functional outcomes. METHODS: We performed a retrospective review of 49 patients who were treated with an MFC bone graft, between May 2005 and September 2016, after prior failure of operative treatment for scaphoid nonunion. The mean time from the injury to the revision

Analysis of wrist motion following vascularized bone graft to the proximal scaphoid.

PURPOSE: To determine whether there is any motion loss associated with the 1,2 intracompartmental supraretinacular artery (ICSRA) bone graft to the dorsal scaphoid. The null hypothesis is that placement of a vascularized bone graft in the dorsal scaphoid does not lead to a significant change in range of motion. METHODS: Seven fresh-frozen cadaveric upper extremities were examined. Simulated 1,2 ICSRA bone grafts were harvested and placed into a dorsal trough made in the proximal scaphoid. Wrist motion measurements were performed before and after 1,2 ICSRA bone graft implantation. RESULTS: Th

Morphological and morphometric features of scaphoid bone in north eastern population, India.

A study was performed to analyse the morphometry and morphological variants of adult scaphoid bone in Sikkimese population of North Eastern India. The study included 100 dry human scaphoid bones. The bones which had previous signs of fracture were excluded. The morphometric parameters were measured with vernier caliper of 0.02 mm accuracy; the circumferences were measured by placing a thread around them and measuring its length. A magnifying lens was used to observe the number of foramina. From our observations, 22 (44%) of the left scaphoid were having conical shape and 28 (56%) were pyramida…

Early magnetic resonance imaging compared with bone scintigraphy in suspected scaphoid fractures.

We evaluated 100 consecutive patients with a suspected scaphoid fracture but without evidence of a fracture on plain radiographs using MRI within 24 hours of injury, and bone scintigraphy three to five days after injury. The reference standard for a true radiologically-occult scaphoid fracture was either a diagnosis of fracture on both MRI and bone scintigraphy, or, in the case of discrepancy, clinical and/or radiological evidence of a fracture. MRI revealed 16 scaphoid and 24 other fractures. Bone scintigraphy showed 28 scaphoid and 40 other fractures. According to the reference standard ther

A prospective comparison for suspected scaphoid fractures: bone scintigraphy versus clinical outcome.

UNLABELLED: Early diagnosis and treatment of scaphoid fractures limits the number of delayed and non-unions. Bone scintigraphy proved to be a sensitive diagnostic tool for the detection of occult scaphoid fractures. However, the results have to be interpreted with care. OBJECTIVE: To prospectively correlate the results of bone scintigraphy with clinical outcome. METHODS: In a prospective study, we analysed 50 consecutive patients with signs of a scaphoid fracture at physical examination but no evidence of a scaphoid fracture on scaphoid radiographs. All patients had a protocolised follow up

MR imaging of avascular scaphoid nonunion before and after vascularized bone grafting.

OBJECTIVE: To investigate the magnetic resonance (MR) imaging appearances of chronic nonunion of the scaphoid with proximal pole avascular necrosis before and after insertion of a vascularized bone graft, using computed tomography (CT) as the imaging gold standard. DESIGN AND PATIENTS: A retrospective study was performed involving MR imaging (n=26), CT scans (n=37) and radiographs (n=52) of 13 men (mean age 29 years, age range 20-38 years) with avascular scaphoid nonunion. Avascular necrosis of the scaphoid proximal pole was confirmed intraoperatively (n=13). MR images were acquired preoperat

Anterior bone grafting and conventional lag screw fixation to treat scaphoid nonunions.

The results of 20 established nonunions of the scaphoid treated with resection of the pseudoarthrosis, anterior cortic-cancellous iliac bone grafting, and conventional lag screw fixation with the ASIF 2.7 mm cortical screw are presented. Union rate was 95% and the average time off work was 8.9 weeks. Review of the relevant literature uniformly shows that the most common reasons for failure are improper internal fixation techniques and/or the absence of bone grafting. Successful treatment of scaphoid nonunions with screw fixation and cast-free after-treatment does not depend on the implant used

Late treatment of scaphoid fractures by bone grafting and compression staple osteosynthesis.

Scaphoid nonunions and delayed unions still offer a difficult challenge. In 15 of 16 cases it was possible to achieve a secure and lasting union by using a compression staple and a bone graft. The subjective and objective clinical evaluation also showed similar favorable results. This modification of the Russe operation is recommended as a relatively simple procedure with good results and minimal need for immobilization.

Bone scintigraphy in the management of X-ray-negative potential scaphoid fractures.

One hundred and eleven patients with signs and symptoms of carpal scaphoid injury, but with no fracture visible on X-ray, underwent bone scintigraphy of the wrists. The first 42 patients were re-X-rayed 10 days after injury: bone scanning had identified all fractures confirmed on this X-ray; there were no false negative bone scans. Sixty-seven patients (60%) did not have increased focal uptake over the scaphoid or distal radius, and were mobilised immediately. None of these had a fracture at follow up. Twenty-nine patients with increased uptake over the scaphoid area (26%) remained in plaster

Anatomical and radiological description of ligament insertions on the radial aspect of the scaphoid bone.

Anatomical studies on the radial side of the scaphoid mention inter-ligamentous connections, but without detailed description of their relations to one another. The purpose of this study was to provide an anatomical and radiological description of the ligamentous structure on the radial side of the scaphoid. High-field 3-Tesla 3D MRI scans of 7 cadaveric formaldehyde-fixed wrists were performed to assess the presence and location of each ligament. Dissection was performed in 10 wrists under microscopy on the radial side to assess the dimensions, anatomical variations and angles between ligamen

Pedicled vascularized bone grafts compared with xenografts in the treatment of scaphoid nonunion.

INTRODUCTION: Fractures of the scaphoid account for 60-70% of all wrist bone fractures. The results of treatment in terms of bone healing vary depending on the type and location of the fracture, the time elapsed since the injury, the type of surgical treatment. Nonunion occurs in 5-15% of the cases on average. The purpose of this paper is to compare the surgical techniques and results of treating scaphoid nonunion (SNU) with osteoplastic xenografts of bovine origin or a vascularized autograft of the distal part of the dorsal radius. METHODS: We compare two groups of patients with symptomatic
