Application of Commonly Used Physical Tests in a Virtual Environment in Patients With Concussion to Patients With Various Types and Severities of Acquired Brain Injury: Prospective Cohort Method Comparison Study.

BACKGROUND: People who sustain a concussion and live in remote areas can experience challenges in accessing specialized assessments. In these cases, virtual approaches to assessment are of value. There is limited information on important psychometric properties of physical assessment measures used to evaluate people postconcussion virtually. OBJECTIVE: The aims of this method-comparison psychometric study were to determine (1) inter- and intrarater reliability of a battery of concussion physical tests administered virtually in people with brain injury and (2) sensitivity and specificity of th

HIV incidence and risk factors for seroconversion among female sex workers and single mothers in a 10-year prospective cohort.

OBJECTIVE: To compare HIV incidence among female sex workers (FSW) and single mothers, and to determine the factors associated with seroconversion among both populations. DESIGN: Prospective cohort conducted in Lusaka and Ndola, Zambia between 2012 and 2022. METHODS: Study staff recruited FSW from common sex work locales and recruited single mothers from postnatal infant vaccination clinics. Enrolled participants were HIV-negative, aged 18-45, and identified as either a FSW or single mother. We measured HIV incidence and assessed associated factors using Poisson regression with adjusted rate

Season of sampling and the interpretation of serum homocysteine results: a within-person analysis of 268,877 health-screening examinations.

OBJECTIVES: Serum total homocysteine (tHcy) is interpreted against fixed reference intervals and a decision limit. Whether the month of blood collection is a material source of variation, and whether it reflects the seasonal cycle of 25-hydroxyvitamin D [25(OH)D], is unresolved because earlier studies were cross-sectional or small. METHODS: We analysed 268,877 health-screening examinations from 69,131 adults between January 2014 and December 2025 at a single Korean centre. Because no participant was examined twice in a calendar year, within-person seasonal contrasts span years; all models the

Informing Modern Models of Care: A Randomized, Sequential Trial of In-Person, Telehealth, and Telephone Appointments for Patients with Inflammatory Bowel Disease.

BACKGROUND: A significant shift toward virtual care has occurred for many patients with inflammatory bowel disease (IBD). To date, there are no prospective studies assessing patients visits across different styles of appointments. METHODS: We randomized IBD patients' appointments to in-person, video-assisted virtual, or telephone and subsequent appointments to the alternate style of visit in a single-center study in Victoria, Canada. Participants completed surveys after each appointment. Demographic data were collected and average scores were analyzed for potential associations. RESULTS: For

The dynamic nature of injunctive drinking norms and within-person associations with college student alcohol use.

Objective: Perceptions of friends' approval of drinking behaviors (i.e., injunctive drinking norms) play a central role in shaping college students' alcohol use behaviors. However, we know little about the extent that students' perceptions of friends' approval fluctuate over time and whether there are within-person associations between these injunctive norms and alcohol use. To fill this knowledge gap, we estimated within-person variability in perceptions of friends' approval of alcohol use across a 12-month period and examined within-person associations between perceptions of friends' approva

Remote Therapeutic Monitoring Enhances Adherence to a Home Exercise Program in Chronic Stroke.

BACKGROUND: Poor adherence to home exercise programs is a persistent challenge in physical rehabilitation and beyond. Data suggest that follow-up telehealth visits featuring behavior change techniques can improve adherence. However, the optimal frequency of behavioral support for effective self-management of technology-enabled home exercise programs remains unclear. This paper investigates how adherence to home exercise varies with the number of follow-up visits holding the type of exercise constant and measuring adherence objectively. METHODS: A secondary analysis of adherence was conducted

A heuristic model for cephalometric diagnosis of sagittal dysplasia.

OBJECTIVE: To identify the best-suited cephalometric parameter for assessing the sagittal skeletal discrepancy in the Indian population. DESIGN: An in vitro, observational, single-blinded, retrospective study. SETTING: Department of Orthodontics and Dentofacial Orthopaedics. METHODS: A total of 94 lateral cephalograms were used in this study. The study involved one key person and two examiners. The key person collected the radiographs, coded, analysed and classified them into three groups (skeletal classes I, II and III). Subsequently, the coded radiographs were independently analysed by th

Optimal hand surgery fellowship interview format.

CONTEXT: The ideal format for residency and fellowship interviews has been consistently debated. Secondary to the COVID-19 pandemic, many institutions, including all hand surgery fellowship programs, transitioned interviews to an all-virtual format. In the past year, with ease of travel restrictions, some programs have transitioned back to in-person interviews, while others remain solely virtual. Hand surgery fellowship programs are continually assessing what are the best means for conducting these interviews, with little perspective regarding the applicant's preferences. OBJECTIVES: The purp

Lessons learned from the frontline implementing person-centered practice: a practical pathway that addresses known challenges.

Person-centred care (PCC) is widely endorsed as essential to high-quality healthcare yet consistent frontline delivery remains elusive. Existing frameworks largely describe what person-centred cultures should look like but provide limited operational guidance on how to enact person-centredness within real interactions under real conditions. This practice-informed perspective paper argues that broader frameworks need to be supported by practical methodologies that directly impact provider behaviour. Drawing on two decades of experience developing and implementing HealthChange® Methodology (HCM)

Associations between postpartum pain type, pain intensity and opioid use in patients with and without opioid use disorder: a cross-sectional study.

BACKGROUND: Pain is a multidimensional construct. The purpose of this cross-sectional, single-centre study was to evaluate the relationship between postpartum pain type with pain intensity and opioid use in people with and without opioid use disorder (OUD). METHODS: Postpartum pain type was coded from McGill Pain Questionnaire and Patient-Reported Outcome Measurement Information System (PROMIS) inventories in people with or without OUD after childbirth in a 4-month period. The co-primary outcomes were pain intensity (0-10 scale) and total inpatient oxycodone (mg). Multivariable linear mixed-e

Temporal stability of serum concentrations of cytokines and soluble receptors measured across two years in low-risk HIV-seronegative men.

BACKGROUND: Prospective cohort studies often quantify serum immune biomarkers at a single time point to determine risk of cancer and other chronic diseases that develop years later. Estimates of the within-person temporal stability of serum markers partly assess the utility of single biomarker measurements and may have important implications for the design of prospective studies of chronic disease risk. METHODS: Using archived sera collected from 200 HIV-seronegative men at three visits spaced over approximately 2 years, concentrations of 14 biomarkers (ApoA1, sCD14, sgp130, sIL-6R, sIL-2Rα,

When things don't add up: the role of perceived fungibility in repeated-play decisions.

Previous research on repeated-play decisions has focused on choices with fungible outcomes. We investigated the perceived fungibility of outcomes over repeated plays of risky prospects in several situations, as well as the relationship between perceived fungibility and preferences for taking risks in those situations. Perceived fungibility varied substantially across participants and situations, with outcomes experienced by different people (e.g., medical outcomes for different patients) receiving lower scores than outcomes experienced by a single person. Higher perceived fungibility was assoc…

Complex visual hallucination and mirror sign in posterior cortical atrophy.

OBJECTIVE: In posterior cortical atrophy (PCA), visual hallucinations are rare symptoms and mirror sign has not been described. METHOD: Single case report. RESULTS: We reported a 60-year-old woman with PCA who reported complex visual hallucinations, such as a man walking in her room, and mirror sign, which was the perception of a stranger staring at her when she looked into a mirror. She could not recognize images of herself in the mirror correctly, although she could recognize that a person standing next to her and the images of that person reflected in the mirror were the same person. Can hospice care be provided to people who live alone?

A growing number of people live alone; some in need of hospice care. Current approaches to providing care include the "patchwork" approach, institutional contracts, hospice day care, a demonstration program for people living alone, and supported housing. A brief discussion of each is presented, with a closing challenge to nursing to provide leadership and bold thinking to meet the changing need of these patients.

[The dimensions of isolation in the elderly].

For the aged, social isolation and loneliness are among the most frequent causes of hospitalization or placement in homes. A survey of 730 people over 65 shows that cohabitation becomes rarer with advancing age, a phenomenon that has grown increasingly important during the twentieth century. Social isolation, characterized by extremely infrequent contacts with other people, affects approximately one out of ten elderly people. Most at risk are those who retired from lower positions in employment, divorced people and widowers. The main causes are the absence of children and relatives in the imme

[Contact behavior of elderly females living alone from the social psychology viewpoint--a preliminary study. V. Conclusions and consequences for practical care].

The main results of a preliminary study on contact behaviour of elderly women living alone are summarized. In the first part of the study we could make measurable the contact behaviour--especially concerning to children and relatives. In the same time we pointed out the main contents of interpersonal relations. Basing on the results of the statistical intercorrelation we formulated consequences to individual advice and care of the studied population.

Clinician contributions to central nervous system-active polypharmacy among older adults with dementia in the United States.

BACKGROUND: Exposure to central nervous system (CNS)-active polypharmacy-overlapping exposure to three or more CNS-active medications-is potentially harmful yet common among persons living with dementia (PLWD). The extent to which these medications are prescribed to community-dwelling PLWD by individual clinicians versus distributed across multiple prescribers is unclear. METHODS: We identified community-dwelling Medicare beneficiaries with a dementia diagnosis and Medicare Parts A, B, and D coverage for at least one month in 2019. Using fill date and days' supply for prescriptions filled bet
