Epidural plasmacytoma often reflects systemic myelomatous involvement and usually has a poor prognosis. The isolated spinal plasmacytoma, however, is a lesion with the potential for long term remission or even cure in some cases. We report six patients with isolated plasmacytoma of the low thoracic or lumbar region, all of whom presented with pain and minimal neurological deficits. The sonographic appearance of the fetal spine was studied using a dynamic section (real time) machine in 7000 pregnancies. The interpretation of the appearance was verified by the use of aborted fetuses and models in a water bath, and by clinical outcome. Eosinophil granuloma of the spine is an uncommon lesion. Six patients with seven vertebral lesions were seen over the past 15 years. Clinical data and suggested pathomechanics relating to each patient are given. The latter correlates well with subsequent progress in each case. All patients were treated symptomatically with initial bed rest, with or without traction. The results of an X-ray investigation of 480 patients suffering from thalidomide embryopathy demonstrate that, beside dysmelia, spinal alterations are an important factor in malformations of the postural and locomotion apparatus. The pattern of malformations consists-according to the frequency of accumulated incidences - of spondylolysis, scoliosis, malformations of the intervertebral disc and dysgenesia of the sacrum. As in the case of extremities this pattern of malformations can only be established by radiological morphology. It necessarily entails alterations of varying functional value. T

Examination of the cervical spine is discussed briefly in relation to history, neurological examination, vertebrobasilar insufficiency and x-rays. The general principles of examining cervical movements are presented. Details of the examination are discussed in more detail under the following headings: the upper cervical spine and its examination for cervical headache; the middle cervical spine with its examination being related to spondylosis/arthrosis; the middle and lower cervical spine and its examination related to wry neck, nerve root irritation or compression, and whiplash injury.

Fourteen cases of nontuberculous infection of the spine were presented, and compared with similar published series (172 cases). The ESR was very high in all cases except one. It returned to normal after recovery. The long delay (5-28 weeks) in diagnosis emphasizes the need for increased awareness of the condition, and the importance of repeated investigations, if these are initially normal. If diagnosed and treated in time the disease runs a relatively benign course. Open exploration is not necessary in all cases, and whether this speeds recovery is open to question.
