[Hypermetabolism in head injury and tetanus: patho-physiological and new therapeutic conceptions].

Longlasting overactivity of the sympathetic nervous system can be considered as one of the causes of the hypermetabolism developing in patients with head injury or tetanus. In tetanus elevated plasma levels of catecholamines can be demonstrated as long as disturbances of the motor nervous system are present. In patients with head injury the plasma catecholamines, mainly noradrenaline, start to increase with the onset of the transition stage from midbrain syndrome to the apallic syndrome. The activity of the sympathetic nervous system remains elevated also in the full stage of the apallic syndr

Pediatric polytrauma: orthopaedic care and hospital course.

Seventy-eight children with multiple trauma who had sustained one major musculoskeletal injury and at least one other major injury were studied. Injury severity was classified by the Modified Injury Severity Scale. There were 137 musculoskeletal injuries; 47 complications occurred in 27 patients. Early osteosynthesis tended to shorten the hospital stay, intensive care unit stay, and length of time ventilatory support was needed. Those patients undergoing immediate surgical stabilization of fractures had fewer complications than those undergoing surgical stabilization after 72 h. Those children

Trochlear nerve palsy associated with superficial siderosis of the central nervous system.

A 56-year-old man with superficial siderosis of the central nervous system (SSCN) presented with complaints of trochlear palsy, visual field defects, gait ataxia, and hearing loss. He had no history of trauma and there were no signs of tumors or aneurysms. T2-weighted magnetic resonance imaging demonstrated characteristic hypointensity in the meninges. We believe that SSCN should be added to the differential diagnosis of trochlear nerve palsy.

Penetrating knitting needle through the mediastinum in a child.

Thoracic injuries are second only to central nervous system injuries as the leading cause of traumatic death in patients of all ages. Penetrating chest injury is very uncommon in children, but it comes with significant morbidity and mortality. Presentation of penetrating thoracic injury in the child is unique with inherent opportunities for learning. The purpose of this case report was to disseminate information regarding rare thoracic trauma in the pediatric patient.

Childhood Ataxia with Cerebral Hypomyelination Syndrome: a Variant of Patient with Early Childhood Onset Related to EIF2B3 Mutation. A Case Report.

Childhood ataxia with central nervous system hypomyelination (CACH) syndrome is an autosomal recessive transmitted leukodystrophy characterised by early childhood onset and acute deterioration following febrile illnesses or head trauma. We describe the case of a child with early onset of CACH syndrome. He presented with cerebellar ataxia beginning around two years of age with mild mental retardation. MRI showed diffuse white matter signal changes with thinning of the corpus callosum.

Jules Tinel (1879-1952): Beyond the eponym, the man and his forgotten neurological contributions.

The trauma of World War I had a lasting impact on clinician and physiologist Jules Tinel (1879-1952). His treatment of peripheral nervous system injuries led him, in 1917, to describe the eponymous sign that he linked to activity of the sympathetic nervous system. Among the sequelae of nerve injuries, he was confronted with causalgia that he attributed, here again, to the autonomic nervous system, the main focus of his laboratory research throughout his career. Tinel's sign became so well known that it eclipsed the originality of his seminal descriptions of exertional headache and of hypertens

Effect of PACAP in central and peripheral nerve injuries.

Pituitary adenylate cyclase activating polypeptide (PACAP) is a bioactive peptide with diverse effects in the nervous system. In addition to its more classic role as a neuromodulator, PACAP functions as a neurotrophic factor. Several neurotrophic factors have been shown to play an important role in the endogenous response following both cerebral ischemia and traumatic brain injury and to be effective when given exogenously. A number of studies have shown the neuroprotective effect of PACAP in different models of ischemia, neurodegenerative diseases and retinal degeneration. The aim of this rev

New perspectives on heterogeneity in astrocyte reactivity in neuroinflammation.

The inflammatory response is a fundamental aspect of all insults to the central nervous system (CNS), which includes acute trauma, infections, and chronic neurodegenerative conditions. As methods for investigating astrocytes have progressed, recent findings indicate that astrocytes can react to a diverse spectrum of insults affecting the central nervous system. Astrocytes respond to external and internal stimuli from the nervous system in a process called glial reactivity. Astrocyte reactivity, previously considered uniform and functionally inactive, is currently a very diverse event in differ

[Hypoxic-ischemic encephalopathy as a special form of head injury complication in battered child syndrome].

The battered child syndrome is increasingly more often described in literature. Head injuries experienced by battered children are the main cause of deaths and neurological complications. A special form of damage inflicted in battered children is vessel background brain injury. The objective of the article was to characterize central nervous system injuries and their results in children who were victims of physical violence. Medical records of three patients hospitalized in Department of Neurology of the Polish Mother Health Center Institute in Lodz, in whom the battered child syndrome had bee

The postnatal human filum terminale is a source of autologous multipotent neurospheres capable of generating motor neurons.

BACKGROUND: Neural progenitor cells (NPCs) are undifferentiated and mitotic and can be induced to differentiate into neurons and glia, the building blocks of the nervous system. NPCs have great therapeutic potential for nervous system trauma and degenerative disorders. They have been identified in the mammalian central nervous system, but current sources are difficult to access surgically and come from regions that are critical for normal brain function. OBJECTIVE: To identify and characterize in detail a novel source of human NPCs in the filum terminale (FT), a vestigial structure at the cau

The duration of antibiotic administration in penetrating abdominal trauma.

BACKGROUND: The epidemiology of penetrating abdominal trauma is changing to reflect an increasing incidence of multiple injuries. Not only do multiple injuries increase the risk of infection, a very high risk of serious infection is conferred by immunosuppression from hemorrhage and transfusion and the high likelihood of intestinal injury, especially to the colon. Optimal timing and choice of presumptive antibiotic therapy has been established for penetrating trauma, but duration has not been studied extensively in such seriously injured patients. The purpose of this study was to test the hypo

Hyponatremia Due to VZV-Induced SIADH in an Older Patient: Case Report and Literature Review.

INTRODUCTION: Hyponatremia is a common finding in hospitalized patients, especially the elderly. Symptoms of hyponatremia can vary depending on the concentration of sodium in serum as well as the dynamics of its escalation. Hyponatremia can have many etiologies, including medication, vomiting, or diarrhea, and central nervous system disorders, including tumors, trauma, and infections. CASE REPORT: In this case, we present a 74-year-old patient who was admitted to the Department of Internal Medicine with symptomatic, acute, and severe hyponatremia in the course of the syndrome of inappropriate

PATHOGENESIS OF RABIES.

The authors have investigated the spread of fixed and street strains of rabies virus from the site of injection to the central nervous system and salivary glands in various animal species. The results indicate conclusively that rabies virus is ordinarily transmitted from the site of exposure to the central nervous system via the peripheral nerves but that other than nerve transmission may occur in young animals, in highly susceptible species or in animals whose resistance has been altered by trauma or shock. Air-borne infection is occasionally possible. Blood-borne infection in nature is belie

Generalized tonic-clonic status epilepticus: causes, treatment, complications and predictors of case fatality.

We retrospectively reviewed the clinical course of 66 patients treated for generalized tonic-clonic status epilepticus at the Ege University neurological intensive care unit from 1988 to 1997. Seventy-two per cent of the study group had a pre-existing seizure disorder, and antiepileptic drug withdrawal was the most prominent cause of status epilepticus. The other causes included drug toxicity, central nervous system infection, cerebrovascular disease, tumour and trauma. Seventy-three per cent of all patients responded to the first-line therapy (diazepam and/or phenytoin), and the remainder wer

[Endogenous factors of neurodestruction (the pharmacologic aspects)].

The data on the biochemical mechanisms of the development of neurodestruction are presented. The emphasis is given to glutamate- and aspartate-induced lesions of the brain in hypoxia, hypoglycemia, trauma. The common mechanisms related to secondary hyperfunction of glutamate- and aspartatergic systems underlie the development of chronic neurodegenerative processes in Alzheimer's disease, Huntington's disease, senile dementia, alcoholism, schizophrenia, etc. The basic pharmacological approaches to prevention of destructive processes in the central nervous system are considered.

[Congenital indifference and congenital insensitivity to pain].

Congenital indifference to pain is often mistaken for congenital insensitivity. It is characterized by the occurrence since childhood of lesions, mainly cutaneous and osteoarticular secondary to strictly painless traumas. However, despite the lack of pain, the patient is able to discriminate a painful stimulus. Autopsy shows no abnormality of the nervous system. A dysfunction of the central endomorphinic systems has been suggested. Congenital analgesia is associated with anhidrosis in Swanson's syndrome (in which Lissauer the tractus is absent in the spinal cord) and with dysautonomia in Riley

Differential T cell response in central and peripheral nerve injury: connection with immune privilege.

The central nervous system (CNS), unlike the peripheral nervous system (PNS), is an immune-privileged site in which local immune responses are restricted. Whereas immune privilege in the intact CNS has been studied intensively, little is known about its effects after trauma. In this study, we examined the influence of CNS immune privilege on T cell response to central nerve injury. Immunocytochemistry revealed a significantly greater accumulation of endogenous T cells in the injured rat sciatic nerve than in the injured rat optic nerve (representing PNS and CNS white matter trauma, respectivel

Incidental bilateral xanthogranuloma of the lateral ventricles at autopsy--a case report.

We report the case of a woman found dead in a public garden. She had multiple contusions, bruises and lacerations of the face due to blunt force trauma. Microscopic examination of the brain was consistent with a death due to severe craniocerebral injury. The neuropathologist also found yellow nodular lesions located in each lateral ventricle and composed by cholesterol clefts, foamy macrophages, and giant cells. A diagnosis of xanthogranuloma of the choroid plexus was made. According to the literature, xanthogranulomas are uncommon benign lesions of the central nervous system. Mostly are found
