Distance mapping in three-dimensional virtual surgical planning in hand, wrist and forearm surgery: a tool to avoid mistakes.

PURPOSE: Three-dimensional planning in corrective surgeries in the hand and wrist has become popular throughout the last 20 years. Imaging technologies and software have improved since their first description in the late 1980s. New imaging technologies, such as distance mapping (DM), improve the safety of virtual surgical planning (VSP) and help to avoid mistakes. We describe the effective use of DM in two representative and frequently performed surgical interventions (radius malunion and scaphoid pseudoarthrosis). METHODS: We simulated surgical intervention in both cases using DM. Joint spac

The Effect of Hamatum Curvature Angle on Carpal Tunnel Volumetry: A Mathematical Simulation Model.

In carpal tunnel volume measurements, the angle of the hamatum curvature is not considered a variable, and its effect on carpal tunnel volume has not been investigated. We hypothesize that a change in the anatomical angle of the hamatum curvature changes the carpal tunnel volume. To prove our hypothesis, we used a mathematical simulation model considering the carpal tunnel as a truncated cone. We reviewed the wrist CT scans of 91 adults (>18 years of age), including 51 men and 40 women. We measured the angle of the hamatum curvature in the CT scans. We measured cross-sectional areas at the out

[Étienne Destot (1864-1918): the first radioanatomist in Lyon, France].

Destot was born in Dijon, France, in 1864. He began his education in Burgundy, then he started his preclinical curriculum in Lyon, France, from 1884. He had to leave Lyon, and spent some times in Algeria to treat a tuberculosis. He came back in Lyon as a resident in 1886. Destot worked as an assistant in the laboratory of anatomy of Leo Testut in 1880. His thesis, in 1892, analyzed mortality in the departments of surgery of the Lyon hospitals. The polemical results he presented compromised his surgical career. He went on as prosector by Leo Testut, and then became electrician-physician in 1895

Orientations of wrist axes of rotation influence torque required to hold the hand against gravity: a simulation study of the nonimpaired and surgically salvaged wrist.

The wrist is a complex kinematic link connecting the forearm and hand. The kinematic design of the wrist is permanently altered during surgical salvage procedures, such as proximal row carpectomy (PRC) and scaphoid-excision four-corner fusion (SE4CF), which have the unintended consequence of long-term functional impairments to both the wrist and hand. We developed simulations of the nonimpaired, PRC, and SE4CF wrists to evaluate if surgically altered wrist kinematics contribute to functional impairments. Specifically, as a step toward understanding the connection between kinematics and functio

Clinical effectiveness of early surgical fixation versus cast immobilization for adults with a scaphoid waist fracture: five-year follow-up of the Scaphoid Waist Internal Fixation for Fractures Trial.

AIMS: In the Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT), surgical fixation was compared with cast immobilization using the Patient-Rated Wrist Evaluation (PRWE) score at one year as the primary endpoint. The aim of this longer-term study was to assess the clinical effectiveness of the two treatment pathways at a five-year follow-up. METHODS: Patients who had not withdrawn from the trial were invited for a clinical assessment at five years after randomization and asked to complete a patient-reported questionnaire in the clinic, or by post or telephone. The PRWE was collected

The Diagnostic Value of the Scapholunate C-Sign: A New Tool for Detecting Through-and-Through Scapholunate Interosseous Ligament Injuries.

PURPOSE: Currently, there is a lack of a validated clinical sign to accurately detect through-and-through or complete injuries to both the dorsal and volar scapholunate interosseous ligament. The aim of this study was to evaluate the validity of a clinical sign for detecting this condition. METHODS: This study included 28 patients with suspected high-grade tears of the scapholunate interosseous ligament who underwent the C-sign maneuver. Each patient received a comprehensive evaluation, including a history, physical examination, magnetic resonance imaging interpretations by a musculoskeletal

Robotic evaluation of a 3D-printed scaffold for reconstruction of scapholunate interosseous ligament rupture: a biomechanical cadaveric study.

BACKGROUND: Rupture of the scapholunate (SL) interosseous ligament (SLIL) is a challenging injury to treat surgically due to the small and complex nature of the SL linkage. This study was a preliminary robotic assessment of the immediate biomechanical effects of a novel 3D-printed scaffold used to reconstruct the ruptured SLIL. METHODS: Nine minimally loaded cadaveric wrists underwent robotically manipulated flexion-extension and radial-ulnar deviation under conditions of intact, transected, and reconstructed SLIL. Simulated radiographic measures (i.e., SL angle and SL gap) and three-dimensio

Scaphoid fracture epidemiology.

BACKGROUND: The current available literature related to scaphoid fracture epidemiology is inconsistent. The aim of this study was to describe the epidemiology of true scaphoid fractures in a defined adult population. METHODS: Using a prospective database, we identified all patients who sustained a radiographically confirmed acute fracture of the scaphoid over a 1-year period. Age, gender, mechanism of injury, the Herbert fracture classification, and associated injuries were recorded and analyzed. RESULTS: There were 151 scaphoid fractures diagnosed giving an annual incidence of 29 per 100,00

Predictors of fracture following suspected injury to the scaphoid.

A prospective study was performed to develop a clinical prediction rule that incorporated demographic and clinical factors predictive of a fracture of the scaphoid. Of 260 consecutive patients with a clinically suspected or radiologically confirmed scaphoid fracture, 223 returned for evaluation two weeks after injury and formed the basis of our analysis. Patients were evaluated within 72 hours of injury and at approximately two and six weeks after injury using clinical assessment and standard radiographs. Demographic data and the results of seven specific tests in the clinical examination were

Dorsal percutaneous cannulated screw fixation for delayed union and nonunion of the scaphoid.

BACKGROUND: Percutaneous fixation of fractures of the scaphoid is well documented in the acute setting by both dorsal and volar methods. What is not commonly discussed is the use of this method for delayed unions and nonunions of the scaphoid. The authors present their case series of patients who underwent dorsal percutaneous fixation for delayed union or nonunion of the scaphoid. METHODS: This study retrospectively reviewed eight consecutive patients (six male patients and two female patients) with a delayed union (8 to 12 weeks) or nonunion (≥13 weeks) of the scaphoid waist treated with dor

Radiographic Landmarks for Ideal Port Placement in Wrist Arthroscopy.

Background  The placement of wrist arthroscopy portals is traditionally performed using distances from anatomic landmarks. We sought to evaluate the safety of traditional portal placement and determine if radiographic landmarks could provide an additional method of identifying tendon intervals. Methods  Six cadaveric specimens were used to evaluate the accuracy of portal placement based on anatomic and radiographic landmarks. Fluoroscopic images were used to document the location of previously described surface landmarks. Soft tissue was dissected away to identify the relationship between the

How was the Turin Shroud Man crucified?

As the literature is not exhaustive with reference to the way the Turin Shroud (TS) Man was crucified, and it is not easy to draw significant information from only a "photograph" of a man on a linen sheet, this study tries to add some detail on this issue based on both image processing of high resolution photos of the TS and on experimental tests on arms and legs of human cadavers. With regard to the TS Man hands, a first hypothesis states that the left hand of the TS Man was nailed twice at two different anatomical sites: the midcarpal joint medially to the pisiform between the lunate/pyramid

Detection of occult wrist fractures by quantitative radioscintigraphy: a prospective study on selected patients.

OBJECTIVE: To determine the value of quantitative radioscintigraphy (QRS) in the diagnosis of wrist trauma occult fractures. PURPOSE: Because of the risk of non-union, the diagnosis of wrist fractures, including scaphoid fractures, is essential but remains difficult despite many imaging modalities. The aim of the study was to assess the benefits of QRS in the diagnosis of occult post-trauma wrist fractures. METHODS: This prospective study included all patients presenting at the orthopaedic department at Brest University Hospital for wrist pain after trauma with initial normal plain radiograp

Rehabilitation Protocol for Lunate Fracture in a Clinical Case Report.

A carpal injury called a lunate fracture can cause severe carpal instability if treatment is not received. After the scaphoid, triquetrum, and trapezium, the lunate is the fourth most frequently fractured carpal bone. Due to lunate fracture, the functional prognosis is uncertain, and conservative treatment frequently results in surgery. Lunate fracture may be caused by anatomical features such as ulnar and radial variation, although necrosis is not commonly one of them. Vascularization in the lunate is unstable and dependent on the capsular arterioles. High shear stresses are frequently applie

Imaging techniques for distal radius fractures and related injuries.

In patients with distal radius fractures, routine roentgenographic examination in at least two or three planes should be performed first and is usually sufficient for correct diagnosis and adequate treatment. A careful examination of the soft tissues of the forearm and wrist on conventional roentgenograms may be key for correct diagnoses. Routine roentgenographic examination of the wrist, preferable in four views, should be performed in every patient with a distal radius fracture acutely as well as at follow-up. The technique is easy to perform, universally available, and inexpensive. Advanced
