A new self-expanding stent approved for the treatment of wide-necked intracranial aneurysms can be difficult to deploy in tortuous vascular anatomy. Some of the difficulty may be attributed to deformation of the delivery microcatheter. We describe a technique for transferring the stent into a braided microcatheter.

The internal and external vertebral venous plexuses (VVP) extend the length of the vertebral column. Authoritative sources state that these veins are devoid of valves, permitting bidirectional blood flow and facilitating the hematogenous spread of malignant tumors that have venous connections with these plexuses. The aim of this investigation was to identify morphologic features that might influence blood flow in the VVP. The VVP of 12 adult cadavers (seven female, mean age 79.5 years) were examined by macro- and micro-dissection and representative veins removed for histology and immunohistoch

OBJECTIVE: Therapeutic embolization of the common carotid artery (CCA), internal carotid artery (ICA), and vertebral artery (VA) is necessary in the treatment of a subset of chronic arteriovenous fistulas (AVFs), hemorrhages, highly vascularized neoplasms before resection, and giant aneurysms. There are currently no reports of the use of the AMPLATZER Vascular Plug II (AVP II) device to occlude the CCA, ICA, or VA. The objective of this article is to present the Duke Cerebrovascular Center experience using the AVP II device in neurointerventional applications.
METHODS: This case series is a r

To evaluate dysphagia, pharyngeal high-resolution impedance manometry (P-HRM-I) is used in conjunction with videofluoroscopic examination of … edullary syndrome. Two patients with severe dysphagia due to lateral medullary infarction caused by vertebral artery dissection were evaluated. In both patients, the initial P-HRM-I showed profound impairment of the UES opening and bolus passage, preventing oral intake. Balloon dilation of the UES, laryngeal elevation exercises, tongue strengthening exercises, and direct swa …

Hypoglossal schwannomas are extremely rare benign tumors arising from Schwann cells of the 12th cranial nerve. Surgical management of hypoglossal schwannomas requires detailed knowledge of the anatomy around the lower cerebellopontine angle (CPA), the craniovertebral junction (CVJ), and high cervical regions. If a tumor extends through the hypoglossal canal to the extracranial space, the canal is exposed by drilling the posterior half of the occipital condyle (OC), maintaining craniovertebral stability. Based on our experience and a literature review, we proposed the following modified grading

We describe the case of a 63-year-old woman with CNS Rosai-Dorfman disease, presenting with diffuse dural infiltration, mimicking idiopathic hypertrophic pachymeningitis, and right vertebral artery dissection. Her symptoms included a progressive 11-month history of vertigo, gait ataxia, and right thalamic stroke. A diagnosis of CNS Rosai-Dorfman disease was made following open dural biopsy, and later confirmed on autopsy studies. The autopsy demonstrated widespread dural infiltration by inflammatory cells, principally large histiocytes, many of which exhibited emperipolesis, a characteristic f…

STUDY DESIGN: Case report and review of the literature.
OBJECTIVE: To report a patient presenting with Brown-Sequard syndrome due to cervical intradural disc herniation after spinal manipulation therapy.
SUMMARY OF BACKGROUND DATA: Spinal manipulation therapy (SMT) is often used by people with neck pain or discomfort as an alternative therapy due to its claimed less invasiveness and comparable efficacy. However, excessive manipulations are reported to cause rare but serious complications such as tetraplegia, vertebral artery dissection, epidural hematoma, and phrenic nerve injury.

The Anterior Inferior Cerebellar Artery-Posterior Inferior Cerebellar Artery (AICA-PICA) common trunk is a rare variant of cerebral posterior circulation in which a single vessel originating from either the basilar or vertebral arteries supplies both cerebellum and brainstem territories. We present the first case of an unruptured right AICA-PICA aneurysm treated with flow diversion using a Shield-enhanced pipeline endovascular device (PED, VANTAGE Embolization Device with Shield Technology, Medtronic, Canada). We expand on this anatomic variant and review the relevant literature. A 39-year-old

INTRODUCTION: Induced hypertension, administered peri-operatively during thoraco-abdominal aortic intervention, is one of the most effective methods to maintain spinal cord perfusion pressure. Posterior reversible encephalopathy syndrome or reversible cerebral vasoconstriction syndrome is a rare encephalopathy, possibly caused by excessive hypertension, usually encountered in the obstetric or cerebrovascular department.
REPORT: A 61 year old man underwent open surgery for repair of an extent II dissecting thoraco-abdominal aneurysm. Several attempts at spinal drainage tube insertion one day p

Introduction Lesions in the cerebellomedullary angle, anterolateral aspect of lower brainstem, and anterior foramen magnum can be accessed through far lateral approach and extreme lateral infrajugular transcondylar exposure. Detailed anatomical knowledge of this region is of paramount importance to avoid untoward complications. Objectives Comprehensive anatomy of this microsurgical corridor is sparse. Our aim was to define the anatomy of this region with relevance to the surgical approach, which would enable the surgeon to navigate this region with ease during surgery. Materials and Methods

OBJECTIVE: The effectiveness of revascularization for complex aneurysms is well-established. This study aimed to describe the technical characteristics and clinical efficacy of intracranial-to-intracranial (IC-IC) bypass for the treatment of complex intracranial aneurysms.
METHODS: We retrospectively reviewed all patients with aneurysms who underwent a preplanned combination of surgical or endovascular treatment and IC-IC bypass at our institution between January 2006 and September 2023. IC-IC bypass techniques included four strategies: type A (end-to-end reanastomosis), type B (end-to-side r

OBJECTIVE: Morphologically challenging cerebral aneurysms cannot be treated through standard endovascular procedures. We report two cases of ruptured aneurysms treated using coils and n-butyl cyanoacrylate (NBCA).
CASE PRESENTATIONS: Case 1 was an 80-year-old woman diagnosed with a subarachnoid hemorrhage (SAH). An angiogram revealed a large and wide-necked basilar artery bifurcation aneurysm. Bilateral superior cerebellar and posterior cerebral arteries (PCAs) originated from the aneurysmal wall. A 3-mm-diameter bleb was detected on the aneurysmal fundus. The bleb enlarged 1 month following

Introduction Blunt cerebrovascular injury (BCVI) can lead to thromboembolic events. The necessity of short-interval noninvasive vascular imaging after the initial diagnosis is controversial. This retrospective cohort assesses the utility of short-interval computed tomography angiography (CTA) after an initial diagnosis of BCVI. Methods We retrospectively reviewed patients with BCVI managed at our institution from 2016 to 2019 who underwent a short-interval (one to three weeks) repeat CTA after initial diagnosis. We excluded patients with age <18 years, penetrating injury, and previous neck ir

Aneurysms of the extracranial vertebral artery are uncommon, with most cases attributed to penetrating head and neck trauma. We report a 29-year-old man with a symptomatic proximal extracranial vertebral artery aneurysm of unclear etiology. This patient's aneurysm was definitively treated after a successful balloon occlusion test of his affected vertebral artery. An endovascular approach was used combining coil embolization of the distal vertebral artery and a covered stent graft in the subclavian. Although aneurysms of this size and location are traditionally repaired with open aneurysmectomy
