Modified technique for frontal sinus obliteration using calvarial bone and Tisseel glue.

BACKGROUND: Frontal sinus obliteration is indicated in trauma and frontal sinus disease. We present our experience with a modified technique of frontal sinus obliteration. METHODS: We reviewed the records of 17 patients who underwent frontal sinus obliteration with a combination of Tisseel fibrin glue (Baxter Healthcare Corporation, Deerfield, IL) and calvarial bone and describe our technique. RESULTS: One female and 16 male patients (mean age, 36.6) underwent frontal sinus obliteration from 2001 to 2005. Traumatic fracture was the most common indication. The patients were followed up for a

Floor-reaching middle turbinate variants: Dual pathophysiology and descriptive terminology.

PURPOSE: The Bolger classification defines three concha bullosa types but does not account for extreme middle turbinate variants extending to the nasal floor. Only one prior report provided quantitative measurements (45mm). We present four patients (32-52mm vertical height) and distinguish primary pneumatization from secondary disease-filled expansion as the underlying mechanisms. METHODS: We retrospectively reviewed patients undergoing endoscopic surgery from January 2021 to December 2024 at a tertiary rhinology center. Four patients with middle turbinate extension to within ≤3mm of the nasa

Migratory Preseptal Cellulitis Secondary to Rhinosinusitis Complicated by Left Frontal Subdural Empyema in a Child: A Case Report.

Preseptal cellulitis is a common pediatric infection involving the eyelids and surrounding periocular tissues. It is typically benign and rarely progresses to intracranial complications, unlike orbital cellulitis. However, when secondary to rhinosinusitis, the infection may behave unpredictably and extend beyond expected anatomical boundaries. We report a rare case of migratory preseptal cellulitis in a 12-year-old boy, initially presenting with unilateral eyelid swelling, erythema, and fever. Despite prompt initiation of empirical intravenous antibiotics, the condition failed to improve and i

Treatment of Displaced Posterior Table Frontal Sinus Fractures: To Cranialize or Not to Cranialize? A Systematic Review and Meta-Analysis.

Frontal sinus fractures account for 5% to 11% of craniofacial traumas in the United States, and up to 40% of them involve the posterior table. Moderate to severe displacement or comminution of posterior table fractures has traditionally been addressed with cranialization. However, noncranialization surgical strategies, endoscopic surgical repair or observation have been proposed as alternative treatment options. A systematic review and meta-analysis were performed to evaluate complication rates and outcomes for cranialization versus noncranialization strategies or endoscopic or observation man

Multiparametric statistical correlations between paranasal sinus anatomic variations and chronic rhinosinusitis.

Improvements in functional endoscopic sinus surgery (FESS) and computed tomography (CT) have concurrently increased interest in the anatomy of the paranasal region. Common anatomical variations are not rare in patients with chronic paranasal sinusitis. The aim of this retrospective study was to analyze the incidence of anatomic variations of the lateral nasal wall in a series of 200 patients with persistent symptoms of rhinosinusitis, after failure of medical therapies, and their correlation with paranasal sinus disease. A detailed analysis of CT scans showed that 140 of 200 (70%) patients had

[Does CT-navigation improve the outcome of functional endonasal sinus surgery?].

INTRODUCTION: Computer-assisted surgery (CAS) has found widespread use in functional endonasal sinus surgery (FESS) over the past few years. The present study investigates if CAS leads to a better outcome in FESS. MATERIAL AND METHODS: All patients who underwent endonasal sphenoethmoidectomy were enrolled in a prospective, non-randomized study. The procedures were done without CAS (group A) in 2003 and in 2004 with CAS (group B), using a Stryker navigation unit. 62 patients (113 sphenoidectomies) were included in group A and 61 patients (109 sphenoidectomies) in group B. The underlying diseas

Trends of Odontogenic Sinusitis Incidence During the COVID-19 Pandemic.

OBJECTIVES: The COVID-19 pandemic affected the epidemiology of several diseases. This study aims to compare the incidence of surgically treated odontogenic sinusitis (ODS) before and during the COVID-19 pandemic and identify unique features. METHODS: A retrospective chart review of patients who underwent at least maxillary antrostomy at a tertiary referral center was performed. The patients were divided into two cohorts: "pre-COVID" (March 2018 to February 2020) and "COVID" (March 2020 to February 2022). Data on demographics, comorbidities, and treatment interventions were collected and analy

Orbital myxoma: A case report.

BACKGROUND: Myxomas are rare, locally infiltrative, benign neoplasms of mesenchymal origin. Although benign, these tumors are locally aggressive with a high rate of recurrence following conservative resection. Their relative infrequency, variable location, and insidious growth present a diagnostic challenge to clinicians. Cases of myxomas have been described throughout the body, but intraosseous myxomas of the orbit are exceedingly rare. CASE DESCRIPTION: We report a case of a 53-year-old male with a history of chronic sinusitis and symptoms of hyposmia and bifrontal headaches refractory medi

Canine sino-nasal aspergillosis: parallels with human disease.

Canine sino-nasal aspergillosis (SNA) is characterized by the formation of a superficial mucosal fungal plaque within the nasal cavity and/or frontal sinus of systemically healthy dogs. The most common causative agent is Aspergillus fumigatus. The fungus does not invade beneath the level of mucosal epithelium but incites a severe chronic inflammatory response that leads to local destruction of nasal bone. These clinicopathological features are equivalent to those of human chronic erosive non-invasive fungal sinusitis. The clinical diagnosis of canine SNA relies on multiple modalities but local

Brain Abscesses of Ear, Nose, and Throat Origin: Comparison between Otogenic and Sinogenic Etiologies.

This retrospective study analyzed 29 cerebral abscesses of ear, nose, and throat (ENT) origin. The mean follow-up of patients was 37 months. ENT etiologies included 45% otitis media (n = 13), 48% sinusitis (n = 14), and 7% ethmoidal sinus tumors (n = 2). Thirty-eight percent (n = 5) of otogenic abscesses occurred within 15 days after a mastoidectomy. Sinogenic abscesses were never due to surgery but were associated in 31% of cases (n = 5) with anterior skull base defects. The main locations of otogenic abscesses were the temporal lobe (54%; n = 7) and the cerebellum (23%; n = 3), whereas sinog

A Computed Tomographic Assessment of Osteitis of Sinus Bony Structures in Horses With Sinonasal Disorders.

Background: Computed tomographic (CT) imaging has shown some horses with sinonasal diseases to have changes in their sinus bony structures. Scintigraphic and clinical evidence of sinus osteitis have also been reported. However, no study has objectively examined for the presence and degree of osteitis in equine sinonasal disease. Objectives: To assess for the presence and extent of osteitis of sinus-related bony structures by examination of CT images of horses with clinically and sinoscopically confirmed unilateral sinonasal disease. Study Design: Retrospective examination of CT images of horse

Multiple Extraaxial Cavernous Hemangiomas: Rare Entity.

BACKGROUND: Cavernous hemangiomas arising in the extraaxial space are rarely encountered, often mimicking other more common pathologies. Furthermore, multiple coexisting lesions and posterior fossa involvement are scarcely reported. Herein, we present the case of a patient with concurrent frontal bone and posterior fossa extraaxial cavernous hemangiomas. We also review the challenges associated with the diagnosis and management of these entities. CASE DESCRIPTION: An otherwise healthy 41-year-old gentleman presented with a 7-month history of a progressive right forehead mass. Imaging demonstr

[Computerized tomography, radionuclide gamma-topography and angiography in the diagnosis of brain abscesses of rhinosinusogenic origin].

X-ray and other methods were used to examine 21 patients with brain abscesses of rhinosinusogenic origin. The diagnostic methods including computer tomography, brain gamma-topography and cerebral angiography yielded objective information about the localization, size, distribution and multiplicity of brain abscesses. They were displayed as doughnuts or rings on computer tomograms and as pies on gamma-topograms. The noninvasive methods such as computer tomography or gamma-topography proved very helpful for assessing the efficacy of surgical intervention at an early postoperative stage.

Thin rubber sheeting in frontal sinus surgery: animal and clinical studies.

Closure of the nasofrontal communication is the most common cause of failure of nonobliterative external of nonobliterative external frontal sinus operations for chronic sinus disease. To determine whether silicone rubber sheeting or tubing might prevent closure, we designed animal experiments and conducted a clinical study. We enlarged the nasofrontal duct in dogs, inserted silicone rubber tubing or sheeting and assessed ductal patency grossly and microscopically. The tubes prevented epithelization and failed to prevent local osteoblastic reaction, fibroplasia, and scar formation. In contrast
