Effectiveness of prehospital chest decompression in resolving clinical signs of tension pneumothorax.

BACKGROUND: Thoracic injuries are a leading cause of morbidity and mortality in military trauma. Tension pneumothorax (TPX) is a critical diagnosis that can lead to rapid hemodynamic and respiratory collapse if untreated. While timely intervention is essential, prehospital TPX diagnosis is challenging and may lead to unnecessary interventions. This study aimed to assess military prehospital chest injury management, including indications for chest decompression and clinical improvement post-intervention. STUDY DESIGN AND METHODS: Retrospective analysis of the Israel Defense Forces (IDF) Trauma

Ketamine alleviates fear memory and spatial cognition deficits in a PTSD rat model via the BDNF signaling pathway of the hippocampus and amygdala.

BACKGROUND: Post-traumatic stress disorder (PTSD) is associated with traumatic stress experiences. This condition can be accompanied by learning and cognitive deficits. Studies have demonstrated that ketamine can rapidly and significantly alleviate symptoms in patients with chronic PTSD. Nonetheless, the effects of ketamine on neurocognitive impairment and its mechanism of action in PTSD remain unclear. METHODS: In this study, different concentrations of ketamine (5, 10, 15, and 20 mg/kg, i.p.) were evaluated in rat models of single prolonged stress and electrophonic shock (SPS&S). Expression

CORM-3 exerts a neuroprotective effect in a rodent model of traumatic brain injury via the bidirectional gut-brain interactions.

OBJECTIVE: Traumatic brain injury (TBI) induced the gastrointestinal inflammation that is associated with TBI-related morbidity and mortality. Carbon monoxide-releasing molecule (CORM)-3 is a water-soluble exogenous carbon monoxide that exerts protective effects against inflammation-induced pyroptosis. We investigated the gastrointestinal inflammation in a rodent model of traumatic brain injury (TBI) with subsequent hemorrhagic shock and resuscitation (HSR), as well as effects of CORM-3 using an intestinal injection on both gut and brain. METHODS: Following exposure to TBI plus HSR, rats were

Haematomyelia and myelomalacia following an inadvertent thoracic intraspinal injection in a cat.

CASE SUMMARY: A 4-month-old cat was presented with acute paraplegia after the referring veterinarian performed a subcutaneous injection (cefovecin and dexamethasone) in the caudodorsal thoracic area, during which the cat suddenly became uncooperative. A complete neurological examination performed 1 day after the injection revealed paraplegia without deep pain perception and reduced segmental spinal reflexes in the pelvic limbs. Findings were consistent with either an L4-S3 myelopathy or a T3-L3 myelopathy with subsequent spinal shock. MRI showed swelling of the spinal cord from T1 to L1 with h

Association Between Blood Donor Demographics and Post-injury Multiple Organ Failure after Polytrauma.

OBJECTIVE: To test the hypothesis that blood donor demographics are associated with transfused polytrauma patients' post-injury multiple organ failure (MOF) status. SUMMARY OF BACKGROUND DATA: Traumatic shock and MOF are preventable causes of death and post-traumatic hemorrhage is a frequent indication for transfusion. The role of blood donor demographics on transfusion recipients is not well known. METHODS: A log-linear analysis accounting for the correlated structure of the data based on our prospective MOF database was utilized. Tests for trend and interaction were computed using a likeli

The "Slash Pack" as a Temporary Method for Initial Hemorrhage Control of Penetrating Neck Trauma in the Emergency Department: A Report of Two Cases.

Penetrating trauma to the neck poses a critical challenge in the emergency department. These wounds have the potential to lead to rapid blood loss, hemorrhagic shock, and an increase in mortality. Large academic tertiary centers typically have vast resources and consultants to allow for rapid assessment and treatment of penetrating neck trauma. In community emergency departments, this is not typically the case, as surgical backup is usually not readily available and lacks the additional subspecialty coverage to aid in the management of the trauma patient. This situation often necessitates inno

Chronic traumatic encephalopathy in athletes, players, boxers and military: systematic review.

The objective of the study was to demonstrate whether athletes, players, boxers and military personnel can really be victims of Chronic traumatic encephalopathy (CTE), and to elucidate this pathology. In 53 articles, 14 were selected for qualitative synthesis in the results table that addresses CTE in football, soccer and rugby players, boxers and the military. Neuropathologically, CTE shows cerebral atrophy, a pelvic septum cavity with fenestrations, dense diffuse immunoreactive inclusions and a TDP-43 proteinopathy. Microscopically, there are extensive neurofibrillary tangles and spindle-sha

Resuscitation thoracotomy.

Resuscitation thoracotomy is a rarely performed procedure whose use, in France, remains marginal. It has five specific goals that correspond point-by-point to the causes of traumatic cardiac arrest: decompression of pericardial tamponade, control of cardiac hemorrhage, performance of internal cardiac massage, cross-clamping of the descending thoracic aorta, and control of lung injuries and other intra-thoracic hemorrhage. This approach is part of an overall Damage Control strategy, with a targeted operating time of less than 60minutes. It is indicated for patients with cardiac arrest after pen

Traumatic mitral valve and pericardial injury.

Cardiac injury after blunt trauma is common but underreported. Common cardiac trauma after the blunt chest injury (BCI) is cardiac contusion; it is very rare to have cardiac valve injury. The mitral valve injury during chest trauma occurs when extreme pressure is applied at early systole during the isovolumic contraction between the closure of the mitral valve and the opening of the aortic valve. Traumatic mitral valve injury can involve valve leaflet, chordae tendineae, or papillary muscles. For the diagnosis of mitral valve injury, a high index of suspicion is required, as in polytrauma pati

Vivien Thomas (1910-1985): the backstage pioneer and educator.

Vivien Thomas (1910-1985) was an African-American scientist, pioneer, and renowned educator. Thomas's contributions to cardiovascular surgery were unequivocal, and yet it was only after his death that he gained more widespread recognition. Thomas's more notable work involves aiding in the discovery of the cause of traumatic shock, designing and guiding the first operation to treat Tetralogy of Fallot, carrying out the first atrial septectomy, and helping develop the electrical defibrillator. Thomas struggled amidst the adversities of racism and the Great Depression (1929-1941), armed merely wi

[Therapeutic approach to the management of fat embolism syndrome].

So far, no clinical or experimental study has demonstrated that any drug has a beneficial effect (heparin, cortisone, dextran, etc.) on the course of fat embolism syndrome (FES). Thus, prevention, early diagnosis, and adequate symptomatic treatment are of paramount importance. Besides surgical measures, such as reduction of intraosseous pressure and bone-marrow release during hip or knee replacement, proper treatment of shock in traumatized patients, recognition of risk factors and maintainance of intraoperative cardiorespiratory stability are cornerstones in the prevention of fat embolism syn

[Age-dependent injury tolerance exemplified by fatal pedestrian accidents].

The evaluation of forensic autopsy material from pedestrian accident casualties comprising more than 500 cases confirms that the tolerance of injuries greatly depends on age. Slightly to moderately severe injured persons with ISS values under 30 have a very much lower risk of death from comparable traumatizations in the third and fourth decade of life as compared to children and above all compared to elderly persons. In the case of 20 to 30 year olds, only just under 10% with such accident injuries die as compared to the percentage of deaths in infants is almost 30% and more than 50% in casual

A novel role for ADAMTS13 in hyperfibrinolysis after trauma-induced shock☆.

BACKGROUND: Trauma-induced coagulopathy with hyperfibrinolysis is associated with high mortality. The protease ADAMTS13 (a disintegrin and metalloprotease with thrombospondin type 1 motifs 13) is cleaved after tissue injury. We analysed the underlying mechanism of ADAMTS13 proteolytic cleavage and the effect of ADAMTS13 on trauma-induced hyperfibrinolysis. METHODS: Thirty-nine trauma patients with shock were stratified based on ADAMTS13 conformation. Mechanisms of ADAMTS13 cleavage were explored in vitro using blood from healthy volunteers. The role of ADAMTS13 in hyperfibrinolysis was examin

[Guidelines for severe multiple and multiorgan traumatic injuries].

Traumatic injuries have been described as the largest epidemic of the 20th century. In view of the number of victims and the associated costs, they have been also called the most severe and longest war of the contemporary world. According to Lipiński, every year every tenth Pole is involved in an accident and every one hundred-thirteenth Polish citizen requires specialist medical care. Thus, the general incidence is approximately 750 accidents per 100,000 Poles per year. 300,000 of them need hospital treatment, 30,000 die from accidents. Despite considerable progress in medical sciences and pr

Altered interstitial fluid space dynamics and postresuscitation hypertension.

Hypertensin occurred 24 to 48 hours after resuscitation in 35 of 86 injured patients, who had combined systolic and diastolic hypertensin (150/100 mmHg) for six or more consecutive hours. Plasma volume (PV), RBC volume, extracellular fluid (ECF) volume by the inulin dilution technique, renal plasma flow, glomerular filtration rate, and peripheral renin levels were measured in hypertensive and nonhypertensive patients an average of 40 hours after injury. The hypertensive patients had an average mean arterial pressure (MAP) of 114 mmHg, compared with 95 mmHg in the nonhypertensive patients. The
