A Landscape of Recent Literature on the Predictors of Success and Failure in Medial Patellofemoral Ligament Reconstruction.

Medial patellofemoral ligament reconstruction is a widely used surgical technique to address patellar instability. This procedure is influenced by several key factors, including patient-specific factors, anatomical considerations, graft selection, tunnel placement, tensioning, fixation methods, postoperative rehabilitation, and potential complications. Various graft options, including autografts and allografts, have been studied, with research indicating no significant differences in pain reduction, return to activity, or failure rates between the two. However, some studies suggest allografts

Update on Shoulder Arthroplasties with Emphasis on Imaging.

Shoulder pain and dysfunction may significantly impact quality of life. If conservative measures fail, advanced disease is frequently treated with shoulder arthroplasty, which is currently the third most common joint replacement surgery following the hip and knee. The main indications for shoulder arthroplasty include primary osteoarthritis, post-traumatic arthritis, inflammatory arthritis, osteonecrosis, proximal humeral fracture sequelae, severely dislocated proximal humeral fractures, and advanced rotator cuff disease. Several types of anatomic arthroplasties are available, such as humeral

Medial patellofemoral ligament reconstruction in children: do osseous abnormalities matter?

PURPOSE: Management of post-traumatic patellar instability in children with osseous abnormalities is challenging because of the presence of an open physis. The aim of our study was to compare the rate of recurrence after isolated reconstruction of the medial patellofemoral ligament (MPFL) in children with or without osseous abnormalities. METHODS: The medical records of 25 children (27 knees) with recurrent patellar dislocation were reviewed. Each child underwent an isolated reconstruction of the MPFL using a hamstring graft. At the last follow-up, a clinical and radiographic evaluation was p

Investigating the Voce Faringea: Physiological and Acoustic Characteristics of the Bel Canto Tenor's Forgotten Singing Practice.

Several historical sources from the first half of the 19th century mention a distinct third register mechanism particular to tenor voices of that period. This so-called voce faringea-often described as an "intermediate" register-is a virtually forgotten historical singing practice used to extend the upper range of the voice, where the singer modifies falsetto, typically a weak and often feminine sound, into a more powerful, tenor-like vocal quality. Based on an evaluation of historical voice register theories, training strategies, and the sound ideals of the historical period, an informed disc

Applications in hip pathology.

Everyday clinical practice frequently leads us to suspect a close relationship between the lumbar spine and the hip-joints. Sagittal balance fundamentally expresses a postural strategy mobilizing the dynamic structure of the lumbar-pelvic-femoral complex in an authentic balance by which obligatory coupled movements transmit stresses in a single structure, the spine, to the two-part structure of the lower limbs, and vice-versa. Flexion contracture is a frequent hip pathology, but congenital dislocation and ankylosis of the hip have the greatest impact on the spine, due to excessive mechanical s

Clinical classification of patellofemoral pain and dysfunction.

The patellofemoral joint is a complex articulation based on its dependence on both dynamic and static restraints for stability. Classification of patellofemoral disorders has been frought with confusion. However, progress has been made in the classification and understanding of these disorders by improved understanding of the biomechanics of the joint and by clarification of the terminology to describe patellofemoral pathology. The term chondromalacia patella, although once used as an all-inclusive term for anterior knee pain, is now widely accepted as a term used to describe pathologic lesion

[Arthroscopic shoulder surgery].

In the field of orthopedic surgery, arthroscopic surgery has become widespread since its first development by Dr. Watanabe in 1950. Arthroscopic surgery for the knee joint is now a common procedure, but this surgery is not so common when the procedure involves the shoulder joint. Although using arthroscopic surgery is not so common on the shoulder joint, there are several benefits by utilizing arthroscopy in such cases. Some of the benefits are; minimal scar formation, no functional limitation of the deltoid muscle, and a quick recovery time. So it is believed that due to these benefits, shoul

Dynamic patellar brace: a new orthosis in the management of patellofemoral disorders. A preliminary report.

The diagnosis and treatment of patellofemoral dysfunction, especially subluxation of the patella, remains controversial. Many authors feel conservative treatment will give better long-term results than operative treatment. To aid in nonoperative treatment of patellofemoral conditions a dynamic patellar stabilizing brace has been developed. The brace applies an active, medially displacing force to the lateral border of the patella and maintains constant pressure during flexion, extension, and rotation of the knee. This brace consists of an elastic sleeve with a patellar cutout and two circumfer

The effect of native knee rotation on the tibial-tubercle-trochlear-groove distance in patients with patellar instability: an analysis of MRI and CT measurements.

BACKGROUND: This study aimed to quantify the effect of lower limb rotational parameters on the difference in the tibial-tubercle-trochlear-groove (TTTG) distance when assessed with magnetic resonance imaging (MRI) and computed tomography (CT) in patients with patellar instability. It was hypothesized that an increased native knee rotation angle significantly contributes to an underestimation of TTTG by MRI. METHODS: Forty patients with patellar instability who had undergone standard radiographs, MRI and CT scans were included in this retrospective study. A musculoskeletal radiologist assessed

Patella alignment abnormality in patients with developmental dysplasia of the hip.

BACKGROUND: The purpose of this study was to assess the influence of developmental dysplasia of the hip (DDH) on patella alignment. METHODS: The radiographic data of all the patients who met the inclusion criteria from January 2015 to July 2017 were reviewed, including the quadriceps angle (QA), patellar lateral tilt (PTA), sulcus angle (SA), lateral shift of patella (LSP), and lower limbs alignment. The patients were divided into three groups in accordance with lateral center-edge angle (LCE) and femoral neck torsion angle (FNTA): group A (LCE > 25° and FNTA < 40°), group B (LCE < 20° and FN

Lateral patellar maltracking due to trochlear dysplasia: A computational study.

BACKGROUND: The study focuses on the influence of trochlear dysplasia on patellar tracking related to patellar instability. METHODS: Knee extension against gravity and dual-limb squatting were simulated with seven models representing knees being treated for recurrent instability. Trochlear depth was altered to represent lateral trochlear inclination (LTI) values of 6°, 12° and 24°. Repeated measures analyses compared patellar lateral shift (bisect offset index) across different LTI values. Peak bisect offset index during extension and squatting was correlated with patella alta (Caton-Deschamp

Medial Patellofemoral Ligament Injuries in Children With First-Time Lateral Patellar Dislocations: A Magnetic Resonance Imaging and Arthroscopic Study.

BACKGROUND: A lateral patellar dislocation (LPD) is the most common knee injury in children with traumatic knee hemarthrosis. The medial patellofemoral ligament (MPFL), the important passive stabilizer against LPDs, is injured in more than 90% of cases. The MPFL injury pattern is most often defined in adults or in mixed-age populations. The injury pattern in the skeletally immature patient may be different. PURPOSE: To describe MPFL injuries in the skeletally immature patient by magnetic resonance imaging (MRI), and to compare the results with the injury pattern found at arthroscopic surgery.

Weight-bearing osteochondral lesions of the lateral femoral condyle following patellar dislocation in adolescent athletes.

In patients with patellar dislocation, osteochondral injury is often an indication for early surgical intervention. However, no studies have identified a relationship between injury to the weight-bearing surface of the lateral femoral condyle following a patellar dislocation and the eventual need for surgical treatment. The authors hypothesized that a significant number of patients sustain injury to the weight-bearing surface of the lateral femoral condyle following an acute patellar dislocation.Radiographs and magnetic resonance images were retrospectively reviewed and the patterns of injury
