Non-Tuberculous Mycobacterial isolates from Panama: A retrospective 5-year analysis (2017-2021).

BACKGROUND: The genus Mycobacterium includes well-known bacteria such as M. tuberculosis causing tuberculosis and M. leprae causing leprosy. Additionally, various species collectively termed non-tuberculous mycobacteria (NTM) can cause infections in humans and animals, affecting individuals across all age groups and health conditions. However, information on NTM infection prevalence in Panama is limited. METHODS: This study conducted a retrospective analysis of clinical records from 2017 to 2021, specifically focusing on patients with NTM isolates. Data were categorized by variables like sex,

Prevalence, Clinical Presentation, and Outcome of Tuberculosis in Patients with Chronic Kidney Disease at a Tertiary Care Hospital in Nepal.

BACKGROUND: Tuberculosis (TB) is a serious public health threat in low- and middle-income countries like Nepal. Chronic kidney disease (CKD) patients are at higher risk of developing new infection as well as reactivation of TB. We aimed to determine the prevalence, clinical presentations, and outcome of TB in patients with CKD in Nepal. METHODS: A hospital-based cross-sectional study was performed at Tribhuvan University Teaching Hospital (TUTH), a tertiary level referral centre in Kathmandu, Nepal. We included patients older than 16 years with the diagnosis of CKD stage 3, 4, 5, and 5D (CKD

[Computer tomography aspects of benign intrathoracic space-occupying lesions].

We examined 55 patients with different, rare, benign intrathoracic lesions. Special computer-tomographic characteristics of the different tumours are demonstrated to help to differentiate from other malignant and benign tumours. In 37 of 55 tumours (67%) typical signs for the benign lesions could be shown. In 25 patients (46%) the preoperative diagnosis proved to be correct. While cystic, calcified and fat containing processes were assessed correctly by computed tomography, solid non calcified mediastinal and intrapulmonary tumours could not be classified as well.

Local production and localization of transforming growth factor-beta in tuberculous pleurisy.

Transforming growth factor-beta (TGF-beta) is one of the cytokines which play an immunosuppressive role in an inflammatory process. To investigate the local production of TGF-beta, we evaluated the levels of TGF-beta in tuberculous pleural effusions (TBPE) and non-tuberculous benign pleural effusions (non-TBPE) by the growth inhibition assay with Mv1Lu mink lung epithelial cells. The mean level of TGF-beta in TBPE (46.1 +/- 31.5 pM; mean +/- s.d.) was higher than in non-TBPE (21.7 +/- 12.3 pM) (P < 0.05). Although the level of interferon-gamma (IFN-gamma) in TBPE measured by ELISA was signific

Radiological difference between new sputum-positive and sputum-negative pulmonary tuberculosis.

INTRODUCTION: The current guidelines for the diagnosis of pulmonary tuberculosis (PTB) are based primarily on the demonstration of acid-fast bacilli (AFB) on sputum microscopy and chest radiograph. Knowing various radiological manifestations and their association with sputum microscopy findings can allow for early diagnosis and early initiation of treatment. AIMS: This study was performed to compare the chest radiograph features seen in sputum-positive and sputum-negative tuberculosis patients, respectively. MATERIALS AND METHODS: It was a prospective observational study, which included 147

Evaluation of different laboratory methods for rapid diagnosis of tuberculous pleurisy.

BACKGROUND/OBJECTIVE: Tuberculous pleurisy is a diagnostic challenge due to its nonspecific clinical presentation, paucibacillary nature of the effusion together with the inefficiency of conventional laboratory methods motivating the evaluation of variable diagnostic strategies. METHODS: Using thoracoscopy, the pleural cavity of 50 patients with undiagnosed exudative pleural effusion were fully examined and biopsy specimens of affected parietal pleura were taken under direct vision. Pleural fluid and biopsy specimen were subjected to microscopic examination (direct and after cytocentrifugatio

Different morbidity after pneumonectomy: multidrug-resistant tuberculosis versus non-tuberculous mycobacterial infection.

To assess whether there is any difference in postoperative morbidity and mortality after pneumonectomy between patients with multidrug-resistant tuberculosis (MDR-TB) and patients with non-tuberculous mycobacterial (NTM) infections. Between January 2000 and December 2007, 61 patients with MDR-TB and 60 patients with NTM infections underwent 66 and 64 pulmonary resections, respectively. Of these, 33 patients were analyzed who underwent a pneumonectomy, including 22 patients with MDR-TB (seven right, 15 left) and 11 patients with NTM infections (seven right, four left). All bronchial stumps were

[The specific features of immunity and immunological responsiveness in elderly and senile patients with tuberculous pleurisy].

The paper gives the results of immunological and clinical studies made in 162 elderly and senile patients with tuberculous exudative pleurisy and a control group of 162 tuberculous pleurisy aged 20-40 years. The elderly and senile patients were found to have more significant immunological responsiveness impairments, suppressed T-cell immunity, and inhibited pleural granulomatous reactions than in the young patients. These alterations substantially worsen the course and outcome of the disease and require im munological correction.

[Assessment of homeostasis by body's adaptative responses in patients with tuberculous pleurisy].

The types of adaptative responses were defined, by analysing leukograms in 126 patients with tuberculous pleurisy. Assessing homeostasis by proteinograms, cellular and humoral immunity verified adaptative responses as an integral index of responsiveness. This identified 5 types of responsiveness. Abnormal (hyperactive, hyporeactive, paradoxical, and areactive) responsiveness was found in 78.6% of patients with tuberculous pleurisy. All outcomes with significant residual pleural changes were observed in patients with abnormal responsiveness. Adaptative responses may be used in the clinical sett

Multifactorial Alterations in Lacosamide Exposure in an Elderly Woman with Symptomatic Epilepsy and Tuberculous Pleurisy: A Case Report.

An elderly woman taking lacosamide (LCM) for epilepsy was diagnosed with tuberculous pleurisy. In the hospital, she received a four-drug combination therapy with rifampicin (RFP), isoniazid, pyrazinamide, and ethambutol for tuberculous pleurisy, in addition to medications for symptomatic epilepsy. At admission, her renal function was mildly impaired but remained stable throughout hospitalization. Serum albumin levels were slightly below the normal range. C-reactive protein was markedly elevated on day 4 but returned to normal by the time of discharge. On day 4, therapeutic drug monitoring (TDM

Anti-tuberculosis drug-induced acute liver failure requiring transplantation in the second trimester of pregnancy: a case report.

BACKGROUND: Treatment of tuberculosis (TB) during pregnancy can reduce maternal and foetal complications. However, it may also induce fatal liver injury. CASE PRESENTATION: We present a case of a 26-year-old pregnant woman who underwent orthotopic liver transplantation for anti-TB drug-induced fulminant hepatic failure (FHF). Her tuberculous pleurisy was treated with rifampin, isoniazid and pyrazinamide. An artificial liver support system (ALSS) was unable to reverse the liver injury while serving as a bridge to liver transplantation. She had a successful liver transplantation operation at 17

Dysphagia due to isoniazid therapy for tuberculosis in a patient with Lewy body dementia.

Dementia with Lewy bodies (DLB) potentially causes dysphagia; however, the features and underlying mechanism causing dysphagia have still not been clarified. We are the first to report a case of dysphagia resulting from isoniazid therapy for tuberculosis in a DLB patient. A 74-year-old woman with DLB developed dysphagia during treatment for tuberculous pleurisy. Oral videoendoscopic and videofluorographic swallowing examinations showed oropharyngeal dysphagia. The increased administration of levodopa successfully ameliorated dysphagia. Therefore, dysphagia was ascribed to diminished levodopa e

Surgical management of resistant mycobacterial tuberculosis and other mycobacterial pulmonary infections.

Between August 1983 and October 1990, 42 patients with resistant Mycobacterium tuberculosis underwent 44 pulmonary resections. During the same time, 38 patients with mycobacterial infections other than tuberculosis had 41 pulmonary resections. All patients either were poor candidates for medical therapy alone or had existing complications requiring surgical intervention. There was one operative death in each group, both from adult respiratory distress syndrome (postpneumonectomy pulmonary edema). Complications were high, with bronchopleural fistula most commonly occurring after right pneumonec

Leucocyte migration inhibition and rosette tests in pleural effusion.

Lymphocytes from pleural effusions have been examined with leucocyte migration inhibition tests (LMIT) and the numbers of B and T lymphocytes estimated. In cases of tuberculous pleurisy a positive LMIT was more frequent than in those of neoplastic etiology. In cases of other etiology no difference has been observed. The proportion of T lymphocytes in pleural effusions from patients with neoplasia was significantly higher (P < 0.01) than in other diseases. The number of B lymphocytes was also lower than in cases of other etiology (P < 0.01). The per cent distribution of T and B lymphocytes in t
