Integrin expression on neutrophils in a rabbit model of Group B Streptococcal meningitis.

Products released by polymorphonuclear cells (PMNs) during an acute inflammatory response can result in diffuse tissue injury. Integrins are cell surface adhesion proteins that play a pivotal role in inflammation by allowing PMNs to adhere to the endothelium and migrate through the extracellular matrix. We examined the expression of beta1 and beta2 integrins on neutrophils from blood and cerebrospinal fluid (CSF) in an animal model of Group B Streptococcal meningitis. We further evaluated whether integrin expression correlates with pathophysiologic markers of central nervous system inflammatio

Pathological aspects of Australian Stringhalt.

Nine horses with clinical signs of Australian Stringhalt were killed and tissues collected for a detailed pathological study. Lesions were limited to peripheral nerves and muscles. The most severely affected nerves were the superficial and deep peroneal, distal tibial, plantar digital, volar and recurrent laryngeal nerve with changes characterised by a selective loss of large diameter myelinated fibres with various degrees of demyelination, fibrosis, Schwann cell proliferation and onion-bulb formation. A routine evaluation of the brain and spinal cord by light microscopy failed to reveal any c

The systemic pathology of Venezuelan equine encephalitis virus infection in humans.

The histopathology of fatal Venezuelan equine encephalitis (VEE) in humans has not been well documented. To evaluate the spectrum of disease in man, the histologic slides of the 21 autopsied patients who died with documented VEE infection during the 1962-63 VEE epidemic in Zulia, Venezuela were reviewed. The main histopathologic lesion observed in multiple organs and tissues, especially the brain, gastrointestinal tract, and lungs, was moderate to marked diffuse congestion and edema with hemorrhage. In the central nervous system (CNS), mild or focal mixed inflammatory cell infiltrates were pre

The use of novel diffuse optical spectroscopies for improved neuromonitoring during neonatal cardiac surgery requiring antegrade cerebral perfusion.

BACKGROUND: Surgical procedures involving the aortic arch present unique challenges to maintaining cerebral perfusion, and optimal neuroprotective strategies to prevent neurological injury during such high-risk procedures are not completely understood. The use of antegrade cerebral perfusion (ACP) has gained favor as a neuroprotective strategy over deep hypothermic circulatory arrest (DHCA) due to the ability to selectively perfuse the brain. Despite this theoretical advantage over DHCA, there has not been conclusive evidence that ACP is superior to DHCA. One potential reason for this is the i

MRI and Clinical Variables for Prediction of Outcomes After Pediatric Severe Traumatic Brain Injury.

IMPORTANCE: Traumatic brain injury (TBI) is a leading cause of death and disability in children, and predicting functional outcome after TBI is challenging. Magnetic resonance imaging (MRI) is frequently conducted after severe TBI; however, the predictive value of MRI remains uncertain. OBJECTIVES: To identify early MRI measures that predict long-term outcome after severe TBI in children and to assess the added predictive value of MRI measures over well-validated clinical predictors. DESIGN, SETTING, AND PARTICIPANTS: This preplanned prognostic study used data from the Approaches and Decisio

Accuracy of diffusion-weighted imaging in the diagnosis of stroke in patients with suspected cerebral infarct.

BACKGROUND AND PURPOSE: The accuracy of diffusion-weighted imaging (DWI) for the diagnosis of acute cerebral ischemia among patients with suspected ischemic stroke arriving to an emergency room has not been studied in depth. METHODS: DWI was performed in 712 patients with acute or subacute focal symptoms that suggested an acute ischemic stroke (AIS), 609 of them with AIS. RESULTS: DWI demonstrated a sensitivity of 90% and specificity of 97%, a positive likelihood ratio of 31 and a negative likelihood ratio of 0.1 for detecting AIS. The overall accuracy was 95%. Of those patients who demonstr

Tc-99m ECD neuro-SPECT and diffusion weighted MRI in the detection of the anatomical extent of subacute stroke: a cautionary note regarding reperfusion hyperemia.

We present a case of subacute middle cerebral artery infarct, which demonstrates restricted diffusion on MRI and reperfusion hyperemia in the posterior half of the lesion on angiography. Tc-99m ethyl cysteinate dimer (ECD) SPECT obtained shortly after the MRI failed to demonstrate perfusion defects in the regions demonstrating reperfusion hyperemia on angiography, underestimating the true size of the infarct. Crossed cerebellar diaschisis is, however, present. SPECT studies obtained over the following weeks demonstrated gradual enlargement of the lesion to approximate the MRI signal changes ov

The Impact of Helmet Use on Head Injury Characteristics and Medical Management Following Helmet-Relevant Activity-Associated Traumatic Brain Injury: A 2014-2021 National Trauma Data Bank Retrospective Cohort Analysis.

Traumatic brain injury (TBI) is a major global health burden, with sports and recreational activities accounting for approximately 10% of all TBI cases in the United States. Helmets are widely used to mitigate head injury risk, yet prior studies have reported conflicting associations between helmet use and specific head injury patterns, medical management, and outcomes across different helmet-relevant activities. The objective of this study is to examine the associations between helmet use and head injury characteristics, injury severity, and emergency department (ED) disposition among patient

Hypertensive disorders of pregnancy and neuroimaging markers of dementia risk: A pilot study.

BACKGROUND AND OBJECTIVES: Hypertensive disorders of pregnancy (HDP) are associated with a long-term risk of maternal cognitive decline. Limited data exist regarding maternal brain structure in midlife after HDP. We examined the association between prior HDP and neuroimaging markers associated with microvascular brain injury, detected on high-resolution brain MRI. METHODS: We conducted a pilot study embedded within the nuMoM2b Heart Health Study, a multicenter prospective cohort of nulliparous US individuals recruited between 2010 and 2013 in the first trimester of pregnancy. Participants wit

Modulation of TLR4-mediated inflammatory pathways and oxidative stress by cerium oxide nanoparticles in traumatic brain injury.

OBJECTS: Traumatic brain injury (TBI) induces secondary neurodegeneration by interdependent inflammatory and oxidative processes. While cerium oxide nanoparticles (CeO2) exhibit antioxidant potential, regulation of Toll-like receptor 4 (TLR4)-driven neuroinflammation remains to be disclosed. The current study investigates the neuroprotective function of CeO2 in rat models of diffuse TBI focusing on the modulation of the TLR4 signaling pathway. METHOD: Thirty-six male Wistar rats (n = 6/group) were randomly assigned to six groups: Sham, DMSO, TBI, and three TBI groups receiving CeO2 at 0.1, 0.

The structural and functional mechanisms of motor recovery: complementary use of diffusion tensor and functional magnetic resonance imaging in a traumatic injury of the internal capsule.

OBJECTIVES: Recovery from focal motor pathway lesions may be associated with a functional reorganisation of cortical motor areas. Previous studies of the relation between structural brain damage and the functional consequences have employed MRI and CT, which provide limited structural information. The recent development of diffusion tensor imaging (DTI) now provides quantitative measures of fibre tract integrity and orientation. The objective was to use DTI and functional MRI (fMRI) to determine the mechanisms underlying the excellent recovery found after a penetrating injury to the right caps

Decoding lower-limb movement attempts from electro-encephalographic signals in spinal cord injury patients.

Restoring lower-limb function in patients with severe spinal cord injury (SCI) remains challenging. Spinal cord stimulation may enhance and reinstate lower-limb movements, but it is either used in open-loop control or its control depends upon residual motor functions, limiting its applicability in severely paralyzed individuals. The decoding of motor intentions from cortical signals may provide an interesting alternative in such cases. Electroencephalography (EEG) is an ideal solution since it is noninvasive and has been employed diffusely in the past to decode upper-limb movement intentions.

Neuroimaging spectrum of immune-mediated central nervous system disorders in children following SARS-CoV-2 infection: A case series.

ObjectiveSevere acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has been associated with neurological complications beyond respiratory involvement. Although children usually experience mild systemic disease, immune-mediated central nervous system (CNS) disorders have emerged as an important cause of morbidity. Neuroimaging plays a central role in detecting these complications and supporting diagnostic decisions. This study aims to describe neuroimaging patterns in children who developed immune-mediated CNS disorders following SARS-CoV-2 infection.Materials and methodsWe retrosp

Human parechovirus infection in neonates and young infants: Clinical features, diagnostic challenges, and neurodevelopmental outcomes.

Human parechoviruses (HPeVs) are increasingly recognized as viral pathogens in neonates and young infants. Among the 18 identified genotypes, HPeV-3 is most strongly associated with severe disease during early infancy, including sepsis-like illness and central nervous system (CNS) infection. Clinical recognition can be challenging because typical laboratory features of CNS infection are frequently absent: cerebrospinal fluid (CSF) pleocytosis is often lacking, and inflammatory markers are usually normal or only mildly elevated. Severe cases may present with marked hyperferritinemia and systemi

Disrupted Maturation of White Matter Microstructure After Concussion Is Associated With Internalizing Behavior Scores in Female Children.

BACKGROUND: Some children who experience concussions, particularly females, develop long-lasting emotional and behavioral problems. Establishing the potential contribution of preexisting behavioral problems and disrupted white matter maturation has been challenging due to a lack of preinjury data. METHODS: From the Adolescent Brain Cognitive Development cohort, 239 (90 female) children age 12.1 ± 0.6 years who experienced a concussion after study entry at 10.0 ± 0.6 years were compared to 6438 (3245 female) children without head injuries who were age 9.9 ± 0.6 years at baseline and 12.0 ± 0.6

Tetramethylpyrazine promotes axonal remodeling and modulates microglial polarization via JAK2-STAT1/3 and GSK3-NFκB pathways in ischemic stroke.

Ischemic stroke results in demyelination that underlies neurological disfunction. Promoting oligodendrogenesis will rescue the injured axons and accelerate remyelination after stroke. Microglia react to ischemia/hypoxia and polarize to M1/M2 phenotypes influencing myelin injury and repair. Tetramethylpyrazine (TMP) has neuroprotective effects in treating cerebrovascular disorders. This study aims to evaluate whether TMP promotes the renovation of damaged brain tissues especially on remyelination and modulates microglia phenotypes following ischemic stroke. Here magnetic resonance imaging (MRI)
