Characterisation of vitreous derived small extracellular vesicles in diabetic retinopathy.

Diabetic retinopathy (DR) is a progressive retinal disease driven by microvascular dysfunction and inflammation, ultimately leading to vision loss. Early detection is essential for preventing irreversible damage; however, the asymptomatic nature of early-stage DR limits the effectiveness of current screening methods. Emerging evidence indicates that inflammatory processes, particularly activation of the nucleotide-binding oligomerization domain-like receptor protein 3 (NLRP3) inflammasome, precede clinical manifestations. Small extracellular vesicles (sEVs), which mediate intercellular communi

Expansion of the ABCA4-Associated Retinopathy Spectrum: Severe Variants Can be Associated With Early-Onset Severe Retinal Dystrophy.

PURPOSE: Leber congenital amaurosis (LCA) and early-onset severe retinal dystrophy (EOSRD) are inherited retinal diseases that are characterized by severe visual loss very early in life. Whereas LCA is characterized by loss of vision in the first year of life, nystagmus, and absent or abnormal electrical signals on electroretinogram, persons with EOSRD show onset of disease between 1 and 5 years of age, with better preserved visual acuity and some signals on an electroretinogram. We investigated the genetic cause of disease and clinical characteristics in three probands with EOSRD. METHODS: A

Pituitary Apoplexy Following Gonadotropin-Releasing Hormone Agonist Therapy: A Rare and Life-Threatening Complication.

Pituitary apoplexy is a rare but potentially life-threatening endocrine emergency characterized by acute hemorrhage or infarction of the pituitary gland. While often associated with pre-existing adenomas, it may also occur in previously normal glands. Risk factors include surgery, pregnancy, and medications such as gonadotropin-releasing hormone (GnRH) agonists, which are rarely implicated in pituitary apoplexy. We present the case of a 59-year-old male with a history of type 2 diabetes mellitus, hypertension, hyperlipidemia, and prostate cancer who developed intractable headache, nausea, vomi

Mendelian Randomisation Analysis of Causal Association between Lifestyle, Health Factors, and Keratoconus.

Keratoconus (KC), a leading cause of vision impairment, has an unclear aetiology. This study used Mendelian randomization (MR) to explore the causal links between various factors (smoking, asthma, Down syndrome, inflammatory bowel disease, atopic dermatitis, and serum 25-hydroxyvitamin D levels) and KC. A two-sample MR design, grounded in genome-wide association study (GWAS) summary statistics, was adopted using data from FinnGen, UK Biobank, and other GWAS-related articles. The inverse-variance weighted (IVW) method was employed, complemented by the Wald ratio method for factors with only one

Early diagnosis and treatment by newborn screening (NBS) or family history is associated with improved visual outcomes for long-chain 3-hydroxyacylCoA dehydrogenase deficiency (LCHADD) chorioretinopathy.

Long chain 3-hydroxyacyl-CoA dehydrogenase (LCHADD) is the only fatty acid oxidation disorder to develop a progressive chorioretinopathy resulting in vision loss; newborn screening (NBS) for this disorder began in the United States around 2004. We compared visual outcomes among 40 participants with LCHADD or trifunctional protein deficiency diagnosed symptomatically to those who were diagnosed via NBS or a family history. Participants completed ophthalmologic testing including measures of visual acuity, electroretinograms (ERG), fundal imaging, contrast sensitivity, and visual fields. Records

The case for investment in eye health: systematic review and economic modelling analysis.

OBJECTIVE: To assess how the returns on investment from correcting refractive errors and cataracts in low- and middle-income countries compare with the returns from other global development interventions. METHODS: We adopted two complementary approaches to estimate benefit-cost ratios from eye health investment. First, we systematically searched PubMed® and Web of Science™ on 14 August 2023 for studies conducted in low-and-middle-income countries, which have measured welfare impacts associated with correcting refractive errors and cataracts. Using benefit-cost analysis, we compared these impa

Lesion detection with fine-grained image categorization for myopic traction maculopathy (MTM) using optical coherence tomography.

BACKGROUND: Myopic traction maculopathy (MTM) are retinal disorder caused by traction force on the macula, which can lead to varying degrees of vision loss in eyes with high myopia. Optical coherence tomography (OCT) is an effective imaging technique for diagnosing, detecting and classifying retinopathy. MTM has been classified into different patterns by OCT, corresponding to different clinical strategies. PURPOSE: We aimed to engineer a deep learning model that can automatically identify MTM in highly myopic (HM) eyes using OCT images. METHODS: A five-class classification model was develope

Dual Consistency Enabled Weakly and Semi-Supervised Optic Disc and Cup Segmentation With Dual Adaptive Graph Convolutional Networks.

Glaucoma is a progressive eye disease that results in permanent vision loss, and the vertical cup to disc ratio (vCDR) in colour fundus images is essential in glaucoma screening and assessment. Previous fully supervised convolution neural networks segment the optic disc (OD) and optic cup (OC) from color fundus images and then calculate the vCDR offline. However, they rely on a large set of labeled masks for training, which is expensive and time-consuming to acquire. To address this, we propose a weakly and semi-supervised graph-based network that investigates geometric associations and domain
