Type II atrial flutter interruption with transesophageal pacing: use of propafenone and possible change of the substrate.

Type II atrial flutter (AFII) is an arrhythmia which usually cannot be interrupted by atrial pacing: the underlying mechanism is considered to be a leading circle without an excitable gap. We investigated whether the administration of propafenone, an antiarrhythmic drug, which primarily decreases conduction velocity, has a beneficial effect on AFII interruption using transesophageal pacing. Twelve patients with an AFII were randomized into 2 groups in which pacing was performed without treatment (group A) or two hours after the administration of 600 mg of oral propafenone (group B). Sinus rhyt

In vitro and in vivo results of transcatheter microwave ablation using forward-firing tip antenna design.

This study was designed to test a microwave (MW) ablation system using approximately 2,450 MHz of energy and a deflectable catheter with forward-firing tip antenna, an early clinical prototype system. In vitro three-dimensional thermal mapping of single and double helix antenna designs was performed. Quantitative measurements of antenna radiation were recorded on tissue phantoms equipped with temperature sensors distributed radially and outwardly. In vivo testing consisted of closed-chest AV junction ablation in three dogs. Thermal mapping showed hemispherical heat distribution from the tip an

Complete heart block in nonfamilial hyperekplexia.

An infant with nonfamilial hyperekplexia had multiple episodes of tonic spasms that mimicked tonic seizures. They were accompanied by complete heart block and apnea. These episodes did not correlate with electroencephalographic epileptiform changes and were partially responsive to clonazepam, valproic acid, and cardiac pacemaker. Sudden death in hyperekplexia may be related to complete heart block and apnea during seizurelike episodes.

Fetal heart block: a new experimental model to assess fetal pacing.

Epicardial fetal pacing via thoracotomy has the potential of being a safer and more reliable procedure to treat congenital complete heart block (CHB) associated with fetal hydrops refractory to medical therapy. To assess the acute electrophysiological characteristics of two ventricular epicardial leads, a new experimental model of fetal heart block induced by cryosurgical ablation of the AV node without the need for fetal cardiac bypass was performed in 12 pregnant ewes at 110-115 days gestation. A modified screw-in lead (1 1/2 turns) was used in six fetal lambs and a stitch-on lead in the oth

Diagnosis and surgical treatment of ectopic atrial tachycardia.

Eighty-two patients with ectopic atrial tachycardia (EAT) were subjected to radical closed heart surgery (without cardiopulmonary bypass). The age of the patients ranged from 1 to 51 years. Permanent EAT was present in 19 patients, incessant EAT in 14, and paroxysmal EAT in 49 patients. Preoperative electrophysiological study included computed analysis of the P wave vector. Ectopic foci were established in the right atrium in 34 patients, in the left atrium in 11, in the interatrial septum in 32, and extracardially in 5 patients. For ablation or isolation of the foci, the cryogenic technique w

Clinical efficacy and electrophysiologic effects of intravenous and oral encainide in patients with accessory atrioventricular pathways and supraventricular arrhythmias.

The electrophysiologic effects and clinical efficacy of intravenous (i.v.) and oral encainide were studied in 13 patients with accessory atrioventricular (AV) pathways (7 overt, 1 intermittent and 5 concealed) and drug-resistant supraventricular arrhythmias (5 paroxysmal atrial fibrillation, 1 atrial tachycardia and 7 with orthodromic circus movement tachycardia). Previously, therapy had failed with a mean of 3 conventional antiarrhythmic agents. In 5 patients, amiodarone administration had also been unsuccessful. All patients underwent programmed electrical stimulation of the heart before and

The p.Arg52Leu variant in GNB2 causes a gain-of-function due to an increase of IK, ACh in stimulated hiPSC-CMs.

BACKGROUND: Initially discovered and published in 2017, the heterozygous mutation (p.Arg52Leu) in the GNB2 gene is associated with sinus node dysfunction (SND) and atrioventricular block (AVB). Electrophysiological characterization using heterologous expression systems has demonstrated that this R52L GNB2 variant results in a larger G-protein-activated K+ (GIRK/IK, ACh) current. So far, this has not been functionally validated in patient-derived disease modelling using human induced pluripotent stem cell-derived cardiomyocytes (CM). METHODS: Here, we differentiated CMs derived from human indu

Predictors of pacing-induced cardiomyopathy in patients undergoing AV nodal ablation: insights from a Delphi process and retrospective cohort study.

AIMS: Pacing-induced cardiomyopathy (PICM) is a recognized complication of pace-and-ablate therapy. Early identification of patients at risk may allow tailored strategies, including upfront biventricular or conduction system pacing. PURPOSE: This study aimed to identify predictors of PICM in patients undergoing pace-and-ablate therapy, combining expert consensus with clinical validation. METHODS AND RESULTS: A three-round Delphi process involving 12 electrophysiologists prioritized candidate predictors of PICM. Consecutive patients undergoing atrioventricular node ablation with non-cardiac r…

Electrophysiological observations during and after transcatheter aortic valve replacement.

BACKGROUND: Conduction disturbances requiring permanent pacing frequently complicate transcatheter aortic valve replacement (TAVR). The understanding of mechanisms causing conduction block is incomplete. OBJECTIVE: This study aimed to characterize the acute and delayed electrophysiological (EP) effects of TAVR on the atrioventricular (AV) conduction system. METHODS: We conducted a single-center prospective cohort study of 409 patients undergoing TAVR. All patients underwent 12-lead electrocardiography and EP study (EPS) immediately before and after valve implantation, with continuous electro

The Burden of Arrhythmias and Conduction Abnormalities Among Patients Presenting With Palpitations at a Cardiac Specialist Care Center in Ghana: A Retrospective Cross-Sectional Study.

BACKGROUND AND AIMS: Palpitation is one of the commonest presenting complaints to the hospital. The underlying problem may or may not be cardiac. Symptoms may not be present at the time of hospital presentation, and the patient's resting electrocardiogram (ECG) may be normal. In such cases, Holter ECG becomes a valuable tool for evaluating these patients. METHODOLOGY: This 12-year retrospective cross-sectional study determined the prevalence of arrhythmias and conduction abnormalities among patients who presented to a specialist cardiac clinic with palpitations and underwent 24-h Holter ECG m

Regulation of Pacemaker Current in the Sinoatrial Node by Zonula Occludens-1.

BACKGROUND: In addition to lethal ventricular arrhythmias, arrhythmogenic cardiomyopathy (ACM) is associated with conduction abnormalities, bradycardias, and reduced expression of the scaffolding junctional protein zonula occludens-1 (ZO-1). Reduced ZO-1 expression is also seen in dilated cardiomyopathy, which is far more common than ACM. Conduction abnormalities are likewise a feature of ZO-1 cardiac-specific knockout (ZO-1cKO) mice. However, the role of ZO-1 in sinoatrial node (SAN) automaticity has not been studied. OBJECTIVE: To investigate the role of ZO-1 in SAN automaticity and elucida

Simplified 3-ganglionated plexus ablation for the treatment of recurrent vasovagal syncope and functional bradyarrhythmia.

BACKGROUND: Cardioneuroablation (CNA) is an emerging treatment for recurrent vasovagal syncope (VVS), extrinsically mediated atrioventricular block (AVB), and sinus node dysfunction (SND). However, there is ongoing debate regarding the number of ganglionated plexuses (GPs) to ablate. OBJECTIVE: To evaluate differences in the degree of autonomic denervation and clinical outcomes between a simplified 3-GP ablation strategy and an extensive ablation approach. METHODS: This is a prospective, observational study of patients with recurrent VVS, extrinsic AVB, or SND. Patients were categorized into

Dihydrotestosterone Pretreatment Diminishes the Severity of Drug-Induced Torsades de Pointes.

Testosterone and dihydrotestosterone (DHT) attenuate drug-induced lengthening of ventricular repolarization, but it is unknown whether they influence drug-induced torsades de pointes (TdP). We tested the hypothesis that DHT reduces the incidence and severity of drug-induced TdP. Male New Zealand white rabbits underwent orchiectomy and were implanted with 2 subcutaneous sustained-release pellets containing DHT 50 mg (100 mg, n = 23) or placebo (n = 20). After 7 days, hearts were excised, mounted, and perfused with modified Krebs-Henseleit solution. The atrioventricular node was destroyed manual…

Closed loop stimulation reduces the incidence of atrial high-rate episodes compared with conventional rate-adaptive pacing in patients with sinus node dysfunctions.

AIMS: Subclinical atrial fibrillation (AF) is associated with increased risk of progression to clinical AF, stroke, and cardiovascular death. We hypothesized that in pacemaker patients requiring dual-chamber rate-adaptive (DDDR) pacing, closed loop stimulation (CLS) integrated into the circulatory control system through intra-cardiac impedance monitoring would reduce the occurrence of atrial high-rate episodes (AHREs) compared with conventional DDDR pacing. METHODS AND RESULTS: Patients with sinus node dysfunctions (SNDs) and an implanted pacemaker or defibrillator were randomly allocated to

Continuous ECG monitoring versus mobile telemetry: A comparison of arrhythmia diagnostics in human- versus algorithmic-dependent systems.

BACKGROUND: Clinicians rarely scrutinize the full disclosure of a myriad of FDA-approved long-term rhythm monitors, and they rely on manufacturers to detect and report relevant rhythm abnormalities. OBJECTIVE: The objective of this study is to compare the diagnostic accuracy between mobile cardiac telemetry (MCT), which uses an algorithm-based detection strategy, and continuous long-term electrocardiography (LT-ECG) monitoring, which uses a human-based detection strategy. METHODS: In an outpatient arrhythmia clinic, we enrolled 50 sequential patients ordered to wear a 30-day MCT, to simultan

The efficacy of pacemaker implantation for extracardiac total cavopulmonary connection in a pediatric patient with bradycardia-tachycardia syndrome.

A 9-year-old boy, diagnosed with double outlet right ventricle after birth, suffered sinus node dysfunction and non-sustained junctional tachycardia after an extracardiac total cavopulmonary connection (TCPC). Spontaneous atrial tachycardia appeared 3 years after an extracardiac TCPC. Sotalol was administered but the bradycardia was obvious. It was difficult to increase sotalol and atrial tachycardia was uncontrollable. Atrial tachycardia continued with symptoms; direct current (DC) cardioversion was frequently required. Five years after extracardiac TCPC, we implanted a pacemaker with atrial

Clinical analysis of cardiac autonomic ganglion plexus ablation for bradyarrhythmia: Research protocol for an intervention study.

OBJECTIVE: This study aimed to explore the safety and effectiveness of selective cardiac autonomic ganglion plexus (GP) ablation on patients with bradyarrhythmia. The heart is controlled by its own intrinsic and central autonomic nerves. Increased cardiac vagal tone leads to sinus node dysfunction and atrioventricular conduction disorders, resulting in bradyarrhythmia. Pacemaker implantation can relieve the symptoms of arrhythmia caused by bradycardia, but it is not easy for patients to accept a pacemaker implantation as a form of treatment. Therefore, more and more attention has been paid to
