Sympathetic Blockade for the Management of Refractory Ventricular Tachycardia: A Case Report.

A 64-year-old man with a history of nonischemic cardiomyopathy (NICM) presented with electrical storm (ES). Episodes of ventricular tachycardia (VT) persisted despite endocardial catheter ablations and exhaustive pharmacotherapy. We used alternating regional anesthesia techniques, left stellate ganglion block, and proximal intercostal block to reduce sympathetic input to the heart, resulting in a significant decrease in VT burden. By using alternating catheter locations, we were able to maintain continuous sympathetic blockade for 31 days and bridge the patient to a successful orthotopic heart

Stellat ganglion blockage in the treatment of chronic refractory angina pectoris.

In addition to medical approaches, laser revascularization, transmyocardial laser revascularization, angiogenesis, growth factor gene therapy, thoracic epidural anesthesia, and spinal cord simulation are used to treat chronic refractory angina pectoris. A unilateral left stellate ganglion block is another alternative. It may particularly be considered as a palliative intervention in patients with a short life span. Described here is the case of a 66-year-old male patient for whom a unilateral left stellate ganglion blockade was used to treat chronic refractory angina pectoris.

Stellate ganglion blockade with percutaneous bupivacaine injection followed with surgical denervation in the management of electrical storm.

Electrical storm is a life-threatening medical emergency that requires immediate diagnosis and treatment. It can remain a clinical challenge despite anti-arrhythmic drugs and catheter ablation therapies. Autonomic modulation techniques have gained increased recognition in the treatment of refractory electrical storm cases. In our case, we present a patient with recurrent ventricular tachycardia/fibrillation episodes one week after a myocardial infarction. Patient's arrhythmia was refractory to antiarrhythmic drugs and hemodynamical status was unstable, thus catheter ablation under temporary me

Sonographically guided coeliac plexus block.

A sonographically guided anterior approach to block the coeliac plexus with local anaesthetic was attempted on supine patients before hepatobiliary interventional procedures. A satisfactory block was achieved in 8/9 patients and the minor complication of transient hypotension was encountered in two which were managed conservatively. Comparison with controls showed the coeliac block patients to require significantly less intravenous sedation and analgesia to control pain during these procedures.

[Complications and side effects of stellate ganglion blockade. Results of a questionnaire survey].

Stellate ganglion blockade (SGB) is an established method in the therapy of chronic pain syndromes. Complications are rare but can be life-threatening (inadvertant subarachnoid or intra-arterial injection). Since no data are currently available as to the incidence of complications, we sent questionnaires to 76 departments of anaesthesiology in West Germany to evaluate this issue. RESULTS. Thirty-nine questionnaires (51%) were returned, representing approximately 45,000 SGBs; 82% of the departments prefer bupivacaine for SGB. The incidence of severe complications was 1.7 in 1000 blockades. Most

[The monitoring of sympathetic activity following stellate ganglion block].

Variations in skin resistance are an indicator of a patient's sympathetic nerve activity. To record these variations, a mobile measuring system was developed. Even if this method is not yet used as a clinical routine, it could be helpful in studying the effects of nerve blocks in anesthesia. It seems to be advantageous for estimating the effectiveness of a block, the stress of the patient, and for recognizing very early reactions of a patient's sympathetic system. METHOD. An objective sign of a successful and sufficient block should be diminishing and finally the total lack of skin reaction am

Lambert-Eaton syndrome.

LES is an autoimmune disorder of the neuromuscular junction in which autoantibodies directed against voltage-dependent Ca2+ channels block nerve-evoked Ca2+ entry at the motor nerve terminal. The pathogenic IgG is likely to produce a similar inhibitory effect on the Ca2+ channel function in other cholinergic synapses of the autonomic nervous system. This pathophysiology is sufficient to account for the distinctive clinical, immunologic, and electrophysiologic manifestations in patients with LES. Etiology of this disease is uncertain but in view of its frequent association with small cell lung

Sustained Relief of Complex Regional Pain Syndrome (CRPS) Pain Following a 60-Day Peripheral Nerve Stimulation: A Report of Three Cases.

Patients who present to pain clinics with complex regional pain syndrome (CRPS) typically have debilitating pain, including hyperalgesia and allodynia, and additional substantial quality-of-life concerns related to the motor and autonomic-related symptoms of CRPS. Present treatments for CRPS such as neuropathic pain medications and sympathetic blocks are often unsatisfactory for managing symptoms. The present cases highlight the use of a 60-day percutaneous peripheral nerve stimulation (PNS) treatment for three patients with CRPS Type I affecting the foot. In all three patients, the tibial and

Complex regional pain syndrome should be aggressively treated as soon as it is diagnosed.

Complex regional pain syndrome is a chronic pain condition characterized by regional pain without a dermatomal distribution. The primary goal of treatment is to alleviate pain and restore function in the affected limb. Multimodal therapeutic methods are adopted, including stellate ganglion block, with favorable outcomes such as pain score reduction and increased mobility in affected patients. This case presentation aims to describe the importance of early stellate ganglion block in the management of CRPS.

Sympathetic nerve blockade for ventricular arrhythmias: stellate ganglion and beyond.

PURPOSE OF REVIEW: To review and summarize the current clinical evidence supporting sympathetic neural blockade for recurrent and refractory ventricular arrhythmias and electrical storm. These conditions continue to be associated with substantial morbidity and mortality, despite advances in antiarrhythmic therapy, catheter ablation, and mechanical circulatory support. Sympathetic overactivity is a key driver of ventricular arrhythmogenesis, and cardiac autonomic neuromodulation should be recognized as an important tool in the therapeutic algorithms. RECENT FINDINGS: Clinical evidence and acce

Ultrasound-Guided Modified Out-of-Plane Approach With Micro-Convex Probe for Stellate Ganglion Block in Patient With Refractory Ventricular Tachycardia With Anticoagulation: A Case Report.

Stellate ganglion block (SGB) is effective in treating refractory ventricular tachycardia. The most common approach to ultrasound-guided SGB is in-plane with a liner probe. However, SGB is prone to hematoma formation as a result of vascular injury, particularly when anticoagulation is used. By performing a modified out-of-plane approach with a microconvex probe and separating the common carotid artery and thyroid cartilage, we were able to shorten the distance to the longus colli muscle. Furthermore, by performing compression with a microconvex probe while viewing an ultrasound image, we were

Conversion of sustained supraventricular tachycardia into sinus rhythm after stellate ganglion block: A case report and pathophysiological considerations.

Considering the anatomy, physiology, and pathophysiology of the autonomic nervous system of the heart, therapy by means of stellate ganglion block may be a logical additional treatment option in certain cases of heart rhythm disorders. A woman in her late 60s presented with sustained paroxysmal supraventricular tachycardia and a heart rate of 170 beats per minute that had persisted for 2 h despite performing vagal maneuvers and orally administering 40 mg of verapamil. She had a history of several episodes of paroxysmal supraventricular tachycardia with spontaneous conversion. We performed a st

Cease the storm - Successful stellate ganglion block in terminating refractory electrical storm.

An electrical storm also known as a ventricular tachycardia storm (VT storm) tends to recur and form a vicious cycle, eventually leading to a refractory electrical storm, refractory to electrical and pharmacological cardioversion. The treatment of refractory VT storm is challenging. Here we discuss the case of a middle-aged gentleman who presented to our emergency department 6 months apart with a refractory VT storm. When all the anti-arrhythmic agents and multiple cardioversion attempts failed in terminating the storm, we attempted ultrasound-guided stellate ganglion block. On both occasions,
